Probiotikų reikšmė atopinio dermatito prevencijai ir gydymui

Darbo tikslas – apžvelgti mokslinėse publikacijose aprašomą probiotikų reikšmę atopinio dermatito prevencijai ir probiotikų efektyvumą, gydant vaikus, sergančius atopiniu dermatitu. Uždaviniai: 1. Įvertinti probiotikų, skiriamų prenataliai ir postnataliai, didelės atopinio dermatito rizikos šeimose, efektyvumą atopinio dermatito prevencijai. 2. Įvertinti probiotikų skyrimo naudą vaikams, sergantiems įvairaus sunkumo atopiniu dermatitu. 3. Įvertinti specifinių probiotikų efektyvumą, gydant vaikus, sergančius atopiniu dermatitu. Metodika. Tyrimo rūšis – sisteminė literatūros apžvalga. Mokslinių straipsnių paieška atlikta per PubMed paieškos sistemą LSMU prenumeruojamoje elektroninėje Medline duomenų bazėje. Atrinkti ir išanalizuoti 2011 01 01 – 2021 12 31 laikotarpiu publikuoti atsitiktinių imčių kontroliuojami tyrimai, kuriuose vertinama probiotikų reikšmė atopinio dermatito prevencijai ir atopiniu dermatitu sergančių vaikų gydymui. Rezultatai. Remiantis numatytais straipsnių atrankos kriterijais, į sisteminę literatūros apžvalgą įtraukta 12 atsitiktinių imčių placebu kontroliuojamų tyrimų: šešiuose nagrinėjama probiotikų įtaka atopinio dermatito prevencijai, kituose šešiuose – gydymui. Tyrimuose dalyvavo 2697 tiriamieji. Vertinant probiotikų veiksmingumą, teigiamu prevenciniu efektu laikytas statistiškai reikšmingai mažesnis atopinio dermatito atvejų skaičius probiotikų grupėje, lyginant su placebu, o teigiamu gydomuoju efektu laikytas statistiškai reikšmingas SCORAD balo sumažėjimas po intervencijos, lyginant su placebu. Visuose analizuotuose tyrimuose gauta stipresnė ar silpnesnė probiotikų skyrimo nauda atopinio dermatito prevencijai ar gydymui, bet tik pusėje tyrimų priminių pasekmių vertinimo rezultatai buvo statistiškai reikšmingi (p < 0,05). Nustatyta, kad kombinuotų probiotikų preparatai yra veiksmingesni nei pavienių mikroorganzimų preparatai tiek atopinio dermatito prevencijos, tiek ir gydymo tikslais. Dažniausiai tyrinėjamas probiotikas buvo Lactobacillus rhamnosus. Tik šios rūšies bakterijos buvo efektyvios, kai skirtos ne derinyje su kitu probiotiniu organizmu. Išnagrinėjus tyrimuose aprašytas antrines probiotikų vartojimo pasekmes, gauta probiotikų nauda vietinių gliukokortikoidų poreikio sumažejimui bei gyvenimo kokybės vertnimui. Išvados. 1. Nenustatyta reikšmingos probiotikų vartojimo įtakos atopinio dermatito atvejų skaičiaus sumažėjimui, lyginant su placebu. Tačiau iš kelių mikroorganizmų sudaryti probiotikų preparatai buvo veiksmingi, siekiant išvengti atopinio dermatito. 2. Nenustatyta reikšmingos probiotikų vartojimo įtakos SCORAD indekso sumažėjimui, lyginant su placebu. 3. Nustatyta, kad keli skirtingi probiotikai, skirami kartu, yra veiksmingesni nei vieno probiotiko preparatai. Tarpusavyje lyginant vienos probiotikų rūšies preparatus, teigiamas efektas stebėtas tik skiriant Lactobacillus rhamnosus.
Aim: to review the role of probiotics in the prevention of atopic dermatitis and the effectiveness of probiotics in the treatment of children with atopic dermatitis described in scientific publications. Objectives. 1. To evaluate the efficacy of probiotics administered prenatally and postnatally in families at high risk of atopic dermatitis for the prevention of atopic dermatitis. 2. To evaluate the benefits of prescribing probiotics in children with atopic dermatitis of varying severity. 3. To evaluate the effectiveness of specific probiotics in the treatment of children with atopic dermatitis. Methodology. Type of research – systematic literature review. The search for scientific articles was performed via PubMed search system in the electronic database subscribed by LUHS. Randomized controlled trials evaluating the role of probiotics in the prevention of atopic dermatitis and in the treatment of children with atopic dermatitis was selected and analyzed between 31 January 2011 and 31 December 2021. Results. Based on the described article selection criteria, 12 randomized placebo-controlled trials were included in the systematic review: 6 examined the effect of probiotics on the prevention of atopic dermatitis and the other 6 on treatment. 2697 subjects participated in the study. In terms of probiotic efficacy, a statistically significantly lower incidence of atopic dermatitis in the probiotic group was considered to be a positive preventive effect compared to placebo. A statistically significant reduction in the SCORAD score compared to placebo was considered to be a positive therapeutic effect. The analyses showed a stronger or weaker benefit of prescribing probiotics for the prevention or treatment of atopic dermatitis, but only in half of the studies the results of the assessment of primary endpoints were statistically significant (p < 0.05). Combined probiotics have been shown to be more effective than single strains in both the prevention and treatment. The most frequently studied probiotic was Lactobacillus rhamnosus. Only this strain was effective prescribed alone. After examining the secondary outcomes, the benefits of probiotics for reducing the need for local glucocorticoids and assessing the quality of life were obtained. Conclusions. 1. There was no significant effect of probiotic use on the reduction of the prevalence of atopic dermatitis compared with placebo. However, probiotic preparations composed of several microorganisms were effective in preventing atopic dermatitis. 2. There was no significant effect on the reduction of the SCORAD index after intervention compared to placebo. 3. Several different probiotics administered together have been found to be more effective than single strains. When comparing single strains, a positive effect was observed only with Lactobacillus rhamnosus.
