Darbo tikslas: įvertinti magnetinio rezonanso tomografijos diagnostinę vertę vertinant kepenų metastazių atsaką į chemoterapijos gydymą. Tyrimo uždaviniai: 1. Nustatyti magnetinio rezonanso tomografijos kepenų metastazių požymius prieš ir po chemoterapijos gydymo 2. Nustatyti magnetinio rezonanso tomografijos diagnostines vertes, vertinant kepenų metastazių atsaką į chemoterapijos gydymą 3. Apibendrinti ir palyginti skirtingų autorių tyrimų rezultatus. Tyrimo metodika: atlikta mokslinės literatūros sisteminė apžvalga, straipsnių paieška atlikta elektroninėje duomenų bazėje „PubMed“, pagal šiuos raktažodžius: „response“ AND „chemotherapy“ AND „liver metastasis“ AND „MRI“. Paieškos laikotarpis 2013- 2023m. Atrinktos publikacijos, kuriose vertinamas MRT diagnostikos metodų tikslumas, jautrumas, specifiškumas, imtis ne mažesnė 20, tiriamos kepenų metastazės prieš ar po chemoterapinio gydymo. Rezultatai: iš viso pagal kriterijus į sisteminę literatūros apžvalgą atrinkti 11 straipsnių, iš kurių 7 retrospektyviniai ir 4 prospektyviniai. Išvados: 1. Būdingiausi kepenų metastazių požymiai yra: hipointensinis židinys T1 sekoje, hiperintensinis židinys T2 sekoje ir ribotos difuzijos vaizdas DW-MRT sekoje. Tiriant kepenų metastazių atsaką į gydymą, padidėjusios ADC vertės po chemoterapinio gydymo yra geresnio metastazių atsako požymis. 2. MRT tyrimas, vertinant kepenų metastazių atsaką į chemoterapijos gydymą, pasižymi vidutiniu-aukštu jautrumu ir pakankamai aukštu specifiškumu. 3. Palyginus skirtingų autorių tyrimų rezultatus, naudojant DW-MRT ir Gd-EOB-DTPA-MRT tyrimų kombinaciją buvo nustatytas aukštas tyrimo jautrumas, atliekant tik DW-MRT tyrimą- jautrumas buvo žemas. Rekomendacijos: Dėl MRT po chemoterapijos tyrimų stokos yra sudėtinga nustatyti diagnostinę vertę. Siekiant gauti tikslesnius radiologinių tyrimų diagnostinius rezultatus, rekomenduojama atlikti platesnės apimties tyrimą ateityje.

Aim of the study: to assess the diagnostic value of magnetic resonance imaging in the evaluation of liver metastases response to chemotherapy. Objectives of the study: 1. To determine the diagnostic value of MRI in assessing the response of liver metastases to chemotherapy 2. To determine the diagnostic value of MRI in assessing the response of liver metastases to chemotherapy 3. To summarise and compare the results of studies by different authors. Methods: a systematic review of the scientific literature was performed according to the requirements, and articles were searched in the PubMed electronic database using the following keywords: "response" AND "chemotherapy" AND "liver metastasis" AND "MRI". The search period was 2013- 2023. The selected publications evaluated the accuracy, sensitivity and specificity of MRI diagnostic methods, with a sample size of at least 20, and investigated liver metastases before or after chemotherapy treatment. Results: a total of 11 articles were selected according to the criteria for inclusion in the systematic literature review, 7 retrospective and 4 prospective. Conclusions: 1. The most characteristic features of liver metastases are hypointense lesions on T1 sequence, hyperintense lesions on T2 sequence and diffusion-limited images on DW-MRI sequence. In the study of the response of liver metastases to treatment, increased ADC values after chemotherapy are a sign of a better metastatic response. 2. MRI has a moderate-high sensitivity and a sufficiently high specificity when assessing the response of liver metastases to chemotherapy treatment. 3. When comparing the results of the different authors' studies, the combination of DW-MRI and Gd-EOB-DTPA-MRI had the highest sensitivity, whereas the lowest sensitivity was observed with DW-MRI alone. Recommendations: The lack of MRI studies after chemotherapy makes it difficult to determine the diagnostic value. In order to obtain more accurate diagnostic results from radiological studies, a larger study volume is recommended in the future.
