Stevens-Johnson sindromas ir toksinė epidermio nekrolizė : imunomoduliuojančio gydymo naudos apžvalga

Darbo tikslas – apžvelgti mokslinėse publikacijose aprašomą sisteminio imunomoduliuojančio gydymo naudą vaikams, sergantiems Stevens-Johnson sindromu ir toksine epidermio nekrolize (SJS/TEN), vertinant jo poveikį mirtingumui ir lyginant su palaikomuoju gydymu. Uždaviniai: 1. Įvertinti sisteminių kortikosteroidų (prednizolono, metilprednizolono) skyrimo naudą vaikams, sergantiems SJS/TEN. 2. Įvertinti intraveninio imunoglobulino G skyrimo naudą vaikams, sergantiems SJS/TEN. 3. Įvertinti ciklosporino skyrimo naudą vaikams, sergantiems SJS/TEN. 4. Įvertinti etanercepto skyrimo naudą vaikams, sergantiems SJS/TEN. 5. Įvertinti kombinuoto imunomoduliuojančio gydymo naudą vaikams, sergantiems SJS/TEN. Metodika. Tyrimo rūšis – sisteminė literatūros apžvalga. Mokslinių publikacijų paieška atlikta LSMU prenumeruojamos PubMed paieškos sistemos pagalba elektroninėje Medline duomenų bazėje, analizuojant straipsnius, atitinkančius paieškos kriterijus. Paieškos metu buvo naudojami šie raktažodžiai: „immunomodulating therapy“, „corticosteroids“, „intravenous immunoglobulin G (IVIG)“, „cyclosporine“, „etanercept“ AND „Stevens-Johnson syndrome“ AND „Toxic epidermal necrolysis“ AND „children“. Paieškos laikotarpis – 2014.01.01 – 2024.12.31. Į sisteminę literatūros apžvalgą buvo įtraukti atsitiktinių imčių kontroliuojami tyrimai, kuriuose SJS/TEN sergantiems vaikams skirtingų grupių imunomoduliuojančiam gydymui skiriamų vaistų ir jų kombinacijų nauda buvo lyginama su palaikomuoju gydymu. Rezultatai. Pagal numatytus straipsnių atrankos kriterijus į sisteminę literatūros apžvalgą įtraukti 8 atsitiktinių imčių kontroliuojami tyrimai. Dauguma tyrimų buvo retrospektyviniai kohortiniai tyrimai. Tyrimuose dalyvavo 964 tiriamieji. Visuose straipsniuose buvo nagrinėta imunomoduliuojančio gydymo įtaka SJS/TEN. Vertinant skirtingų imunomoduliuojančių medikamentų efektyvumą, gydant vaikų SJS/TEN, dažniausiai buvo skiriamas sisteminis KKS – metilprednizolonas. Išvados. 1. Sisteminiai KKS gali būti veiksmingi ankstyvose ligos stadijose, tačiau jų nauda turi būti vertinama individualiai. 2. IVIG terapija laikoma saugesne alternatyva nei sisteminiai KKS, kadangi IVIG kelia mažesnę infekcijų riziką. 3. CS gali sumažinti ligos progresavimą, tačiau didina infekcijų riziką dėl imunosupresijos. 4. Etanercepto vartojimo nauda dėl riboto kiekio tyrimų, susijusių su etanercepto saugumu ir veiksmingumu naudojimo vaikams, nenustatyta. 5. Kombinuotas imunomoduliuojantis gydymas svarbus, kai vienas medikamentas nesuteikia pakankamo efekto.
The aim of the work is to review the benefits of systemic immunomodulatory therapy in children with Stevens-Johnson syndrome and toxic epidermal necrolysis (SJS/TEN) described in scientific publications, assessing its impact on mortality and comparing it with supportive care Tasks: 1. To evaluate the benefits of administering systemic corticosteroids (prednisolone, methylprednisolone) to children with SJS/TEN”. 2. To evaluate the benefits of intravenous immunoglobulin G administration in children with SJS/TEN 3. To assess the benefits of administering cyclosporine to children with SJS/TEN 4. To assess the benefits of administering etanercept to children with SJS/TEN. 5. To evaluate the benefits of combination immunomodulatory therapy in children with SJS/TEN. Methodology. The type of research was a systematic literature review. The search for scientific publications was carried out with the help of the LSMU-subscribed PubMed search system in the Medline electronic database, analyzing articles that met the search criteria. The search terms included: "immunomodulatory therapy", "corticosteroids", "intravenous immunoglobulin G (IVIG)", "cyclosporine", "etanercept" AND "Stevens-Johnson syndrome" AND "Toxic epidermal necrolysis" AND "children". Search period – 2014.01.01 – 2024.12.31. The systematic literature review included randomized controlled trials comparing the benefits of different groups of immunomodulatory drugs and their combinations with maintenance therapy in children with SJS/TEN. Results. Based on the specified article selection criteria, 8 randomized controlled trials were included in the systematic literature review: six of them examined the effect of immunomodulatory treatment in SJS/TEN. Most of the studies were retrospective cohort studies. The studies included 964 subjects. All articles examined the effect of immunomodulatory treatment in SJS/TEN. When evaluating the effectiveness of different medications, the most frequently studied and used immunomodulatory medication in the treatment of SJS/TEN in children was CCS - methylprednisolone. Conclusions. 1. Systemic CCS may be effective in the early stages of the disease, but their benefits must be assessed individually. 2. IVIG therapy is considered a safer alternative to systemic CCS, as IVIG carries a lower risk of infections. 3. CS may reduce disease progression, but increases the risk of infections due to immunosuppression. 4. The benefit of etanercept has not been established due to the limited number of studies related to the safety and efficacy of etanercept in children. 5. Combination immunomodulatory therapy is important when a single drug does not provide a sufficient effect.
