Išsiaiškinti skirtingų sarginio limfmazgio žymėjimo metodų privalumus ir trūkumus. 3. Ištirti skirtingų sarginio limfmazgio žymėjimo metodų saugumą ir pritaikymą klinikinėje praktikoje. Tyrimo metodai. Atlikta sisteminė mokslinės literatūros apžvalga ir analizė remiantis PRISMA (angl. Preferred Reporting Item for Systematic Review and Meta-Analyses: The PRISMA Statement) metodika. Paieška atlikta „PubMed“ elektroninėje duomenų bazėje naudojant raktinius žodžius: (Fluorescent OR new methods OR alternative methods OR novel techniques OR SPIO OR magnetic method OR superparamagnetic iron oxide) AND (sentinel lymph node biopsy breast cancer). Į sisteminę literatūros apžvalgą įtrauktos ne senesnės nei 10 metų publikacijos, kurios atrinktos pagal iš anksto sudarytus įtraukimo ir atmetimo kriterijus. Tyrimo rezultatai. Pasitelkiant raktinius žodžius ir atlikus pirminę paiešką mokslinėje duomenų bazėje ,,PubMed“ buvo rasta 1610 publikacijų. Pritaikius papildomus filtrus buvo atmesta 1555 straipsniai. Perskaičius publikacijų pavadinimus ir santraukas iš viso buvo atrinkta 12 mokslinių publikacijų, kurios atsakė į iškeltus sisteminės literatūros apžvalgos uždavinius ir atitiko visus paieškos kriterijus. Sisteminėje literatūros apžvalgoje ištirti sarginio limfmazgio žymėjimo metodų efektyvumas, privalumai, trūkumai ir jų saugumas ankstyva krūties vėžio stadija sergantiems pacientams. Išvados. a) Indocianino žaliojo dažo fluorescencija (ICG-F) ir superparamagnetinis geležies oksidas (SPIO) pasižymėjo neprastesniais rezultatais nei technecis ar jo derinys su mėlynu dažu aptinkant SLM ankstyva krūties vėžio stadija sergantiems pacientams, b) ICG ir SPIO yra saugesni apšvitos atžvilgiu ir pigesni lyginant su radioizotopu techneciu, kainos skirtumas tarp ICG ir radioizotopo ypač didelis, siekiantis iki 5 kartų, c) ICG ir SPIO yra saugios SLM žymėjimui taikomos medžiagos, kadangi tarp apžvelgtų tyrimų ICG dažo injekcija nepasireiškė jokiomis komplikacijomis, o SPIO vienintelė nepageidaujama reakcija buvo ilgalaikis odos nudažymas ruda spalva injekcijos vietoje. Rekomendacijos: Naujų technikų ir technologijų pritaikymas medicinos praktikoje yra svarbus, siekiant geresnių pooperacinių rezultatų. Šio tyrimo rezultatai rodo, kad tiek ICG-F, tiek SPIO pasižymi neprastesniais rezultatais nei radioizotopas. Abu minėti žymėjimo metodai sukelia mažiau komplikacijų, tuo pačiu yra pigesni nei technecis. Todėl gydytojams vertėtų apžvelgti ICG-F ir SPIO žymėjimo metodų įtraukimo kaip tinkamų alternatyvų galimybes. Author. Rokas Šadauskas Title. Alternative Methods for Sentinel Lymph Node Biopsy in Breast Cancer Patients Aim of the study. To perform a systematic review of the literature, to review and summarize alternative methods of sentinel lymph node biopsy in breast cancer patients. Tasks. 1. To investigate which sentinel lymph node biopsy method has diagnostic advantage for early breast cancer patients. 2. To investigate whether a specific method for sentinel lymph node biopsy has an advantages and/or disadvantages over the other methods in breast cancer patients. 3. To investigate the safety and implementation of different sentinel lymph node biopsy methods in breast cancer patients Methods: A systematic review and analysis of the literature was performed according to the PRISMA methodology. The search was performed in the PubMed electronic database using the keywords: (fluorescent OR blue dye OR radioactive isotope OR new methods OR alternative methods) AND (sentinel lymph node biopsy breast cancer). The systematic literature review includes publications up to 10 years old, based on a preestablished inclusion and exclusion criteria Results: Using keywords and conducting a preliminary search in the scientific database "PubMed," a total of 1610 publications were found. After applying additional filters, 1555 articles were rejected. Upon reading the titles and abstracts of the publications, a total of 12 scientific articles were selected, which addressed the research questions of the systematic literature review and met all search criteria. The systematic literature review investigated the effectiveness, advantages, disadvantages, and safety of sentinel lymph node detection methods in patients with early-stage breast cancer. Conclusions: a) Fluorescence of Indocyanine green (ICG) and superparamagnetic iron oxide (SPIO) showed noninferior results to technetium or its combination with blue dye in detecting SLN in patients with early-stage breast cancer, b) ICG and SPIO are safer in terms of radiation exposure and cheaper compared to technetium radioisotope, the price difference between ICG and radioisotope is particularly large, reaching up to 5 times cheaper cost of ICG compared to technetium, c) ICG and SPIO are safe materials for SLN labeling, as among the reviewed studies, injection of ICG dye did not result in any complications, and the only adverse reaction of SPIO was long-term skin discoloration at the injection site. Recommendations: The application of new techniques and technologies in medical practice is important for achieving better postoperative outcomes. The results of this study indicate that both ICG-F and SPIO show noninferior results compared to the radioisotope. Both of these marking methods result in fewer complications and are also cheaper than technetium. Therefore, it is worthwhile for physicians to consider the inclusion of ICG-F and SPIO marking methods as viable alternative options.
