Sąsajos tarp vaistų nuo epilepsijos dozės ir koncentracijos kraujyje, bei nepageidaujamų vaistų nuo epilepsijos poveikių

Darbo tikslas: Nustatyti sąsajas tarp epilepsijos gydymui vartojamų vaistų nuo epilepsijos paros dozės ir koncentracijos kraujyje bei nepageidaujamų reakcijų į VNE.
Darbo uždaviniai: 1. Išanalizuoti VNE paros dozes ir koncentraciją kraujyje tiriamiesiems VNE vartojant monoterapijoje, politerapijoje, su ar be kepenų fermentus indukuojančiu (CBZ) bei inhibuojančiu (VPA) VNE.
2. Nustatyti ryšį tarp tiriamųjų vartotų VNE paros dozių ir koncentracijos kraujyje, gydant VNE monoterapija bei politerapija.
3. Įvertinti kepenų fermentų (ALT, AST) ir Na kiekio kraujyje rodiklius tiriamiesiems, gydytiems VNE.
4. Nustatyti tiriamųjų nepageidaujamų reakcijų į VNE (kepenų fermentų (ALT, AST) kiekio padidėjimo, hiponatremijos) pasireiškimo dažnį ir dydį.
5. Nustatyti ryšį tarp VNE paros dozės, koncentracijos kraujyje ir kepenų fermentų (ALT, AST) bei Na kiekio kraujyje rodiklių.
Metodai: Atlikta retrospektyvinė 172 epilepsiją sergančių ir VNE vartojančių tiriamųjų demografinių (lytis, amžius), klinikinių duomenų (EP dažnis ir rūšis), VNE vartojamų dozių, koncentracijų, kepenų fermentų bei natrio koncentracijų duomenų analizė. Naudota Pearsono tiesinė koreliacija bei dviejų nepriklausomų imčių T kriterijus. Statistiškai patikimais laikyti rezultatai, kai p < 0,05.
Rezultatai: Tiriamųjų grupėje (n=172) buvo 86 (50 proc.) vyrai ir 86 (50 proc.) moterys. Vidutinis jų amžius – 38,2 ± 16,2 m., vyrų – 39,8 ± 17,0 m., moterų - 36,7 ± 15,3 m. (p > 0,05). LTG koncentracijos kraujyje vidurkis vartojant LTG politerapijoje su VPA buvo statistiškai reikšmingai didesnis, nei vartojant politerapijoje be VPA. Daugiau kaip dviems trečdaliams tiriamųjų, vartojusių mažą VPA dozę, buvo nustatyta ir maža koncentracija kraujyje, vidutinė paros dozė vidutinei koncentracijai kraujyje atitiko vienam trečdaliui tiriamųjų, o vartojusių didelę VPA paros dozę tiriamojo koncentracija kraujyje buvo maža. Tiriamųjų vartojusių mažą CBZ dozę ir turėjusių mažą koncentraciją taip pat buvo apie du trečdalius. Vidutinė dozė atitiko vidutinę koncentraciją vienam penktadaliui tiriamųjų, o vartojusių didelę dozę trečdaliui tiriamųjų buvo nustatyta didelė koncentracija.
Kuo didesnę VPA,CBZ, LEV ar OXC dozę vartojo tiriamieji, tuo didesnė šio vaisto koncentracija kraujyje buvo nustatyta, o vartojant LTG ir TPM tokio ryšio nustatyta nebuvo. Lyginant tiriamųjų natrio koncentraciją kraujyje, kuriems politerapijos metu buvo skiriami deriniai su CBZ ir be šio medikamento, buvo nustatytas statistiškai reikšmingai mažesnis kiekis politerapijos grupėje su CBZ.
Išvados: 1. Politerapijos grupėje tiriamieji vartojo didesnę VPA bei CBZ paros dozę nei šiuos VNE vartoję tiriamieji monoterapijos grupėje (p<0,05). Tarp VNE paros dozių, vartojant VNE derinyje su ar be kepenų fermentus indukuojančiu VNE (CBZ) bei su ar be kepenų fermentus inhibuojančiu VNE (VPA), statistiškai reikšmingo skirtumo nenustatyta (p>0,05). Vaistų nuo epilepsijos (VPA, CBZ, LTG, LEV, TPM, OXC) koncentracijos kraujyje reikšmės monoterapijos ir visose politerapijos grupėse statistiškai reikšmingai nesiskyrė (p>0,05), išskyrus LTG koncentraciją kraujyje, kurios vidurkis vartojant LTG derinyje su VPA buvo statistiškai reikšmingai didesnis (p<0,05), nei vartojant VNE derinį be VPA.
2. Tiek monoterapijos, tiek politerapijos grupėse, kuo didesnę VPA, CBZ, LEV ar OXC dozę vartojo tiriamieji, tuo didesnė buvo nustatyta ir šio VNE koncentracija kraujyje (p<0,05); vartojant LTG ir TPM, ryšio tarp vartojamos VNE paros dozės ir koncentracijos kraujyje nustatyta nebuvo (p>0,05).
3. Visose analizuotose tiriamųjų grupėse nustatytas ALT, AST kiekio kraujyje vidurkis atitiko normalią šių fermentų kiekio vertę. Bendroje tiriamųjų grupėje Na kiekio kraujyje vidurkis atitiko normalią Na kiekio kraujyje vertę. Vartojusiems VNE derinyje su CBZ, Na kiekis kraujyje buvo mažesnis, nei tiriamųjų, kurie buvo gydomi VNE deriniu be CBZ (p<0,05).
4. Bendroje tiriamųjų grupėje padidėjęs kepenų fermentų kiekis buvo nustatytas maždaug dešimtadaliui tiriamųjų (ALT – 16,4 proc., AST – 9,3 proc.), didžiąjai daliai – iki dviejų kartų viršijant normą. Tiriamųjų kiekis, kuriems buvo nustatytas kepenų fermentų padidėjimas, tarp skirtingus kepenyse metabolizuojamus VNE (VPA, CBZ, OXC) vartojančių tiriamųjų nesiskyrė (p>0,05). Bendroje tiriamųjų, vartojusių CBZ ir/ar OXC, grupėje hiponatremija nustatyta ~ ¼ tiriamųjų. Lengva hiponatremija pasireiškė ~ 1/5, vidutinio sunkumo - ~ 6 proc. tiriamųjų, sunkios hiponatremijos nebuvo nustatyta nei vienam tiriamajam. Tiriamųjų, kuriems nustatyta hiponatremija, vartojusių ir nevartojusių CBZ, OXC bei CBZ ir OXC grupėse statistiškai reikšmingai nesiskyrė (p>0,05).
5. Kuo gydomi tik VPA tiriamieji vartojo didesnę VPA paros dozę, tuo didesnis buvo ALT kiekis kraujyje (r = 0,371, p<0,05). Statistiškai reikšmingo ryšio tarp VPA, CBZ ar OXC, vartojamų monoterapijoje ar derinyje su kitu VNE, koncentracijos kraujyje ir kepenų fermentų kiekio kraujyje dydžio nenustatyta (p>0,05). Tiek CBZ, tiek OXC mono- ar politerapija gydytiems tiriamiesiems, statistiškai reikšmingo ryšio tarp CBZ ar OXC paros dozės, koncentracijos kraujyje ir natrio kiekio kraujyje nustatyta nebuvo (p>0,05).
Relation between dose and concentration of antiepileptic drugs and side effects of antiepileptic drugs Author: Tomas Šarnauskas Supervisor: prof. Giedrė Jurkevičienė Aim: To determine correlation between antiepileptic drugs dose and serum concentration and side effects. Objectives:
- To analyze the daily dose and blood concentration of AED in subjects during monotherapy, polytotherapy and treated with or without liver enzyme inducer (CBZ) and inhibitor (VPA).
- To determine the relationship between the daily doses and the blood concentration of AED administered to the subjects during monotherapy and polytherapy.
- To assess liver enzymes (ALT, AST) and sodium levels in subjects on antiepileptic drug treatment.
- To determine subjects antiepileptic drugs side effects (liver enzyme (ALT, AST) level elevation, hyponatremia) frequency of occurrence and level.
- To determine correlation between antiepileptic drugs dose, serum concentration and liver enzyme (ALT, AST) and sodium blood levels. Methods: We performed a retrospective analysis of demographic (sex, age), clinical (epileptic seizure frequency and type), antiepileptic drugs (AED) dose, serum concentration, liver enzyme and sodium levels data in 172 subjects with epilepsy, treated with antiepileptic drugs. We used Pearsons linear correlation and independent sample T test. Statistically reliable result was considered when p < 0,05. Results: There were 86 (50%) men and 86 (50%) women in the study group (n = 172). Their average age was 38.2 ± 16.2 years, men - 39.8 ± 17.0 years, women - 36.7 ± 15.3 years. (p> 0.05). Mean blood LTG concentrations were statistically significantly higher in LTG polytotherapy with VPA than without VPA. More than two-thirds of the subjects who received low-dose VPA also had low blood concentrations, the average daily dose corresponded to one-third of the average concentrations and those who received high-dose VPA had low blood concentrations. CBZ at low doses and at low concentrations were also about two-thirds. The average dose corresponded to the average concentration in one-fifth of the subjects, and high concentrations were found in one-third of the subjects at the high dose. The higher the dose of VPA, CBZ, LEV, or OXC taken by the subjects, the higher the blood levels of this drug was determined, and no such association was found with LTG and TPM. Comparing the blood sodium concentrations of subjects treated with CBZ in combination with and without this drug during polytherapy, statistically significantly lower levels were found in the polytherapy group with CBZ. There was a statistically significant relationship between VPA dose and ALT concentration during monotherapy (r = 0.371; p <0.05). Conclusions: 1. Subjects in the polytherapy group received higher daily doses of VPA and CBZ than subjects in the monotherapy group of these AEDs (p <0.05). There was no statistically significant difference (p> 0.05) between the daily doses of AED in combination with liver enzyme inducer AED (CBZ) and with or without liver enzyme inhibitor AED (VPA). Blood concentrations of antiepileptic drugs (VPA, CBZ, LTG, LEV, TPM, OXC) did not differ statistically significantly between monotherapy and all polytherapy groups (p> 0.05), except for LTG blood concentrations, where the mean LTG in combination with VPA was statistically significantly higher (p <0.05) than with AED combination without VPA.
- In both the monotherapy and polytotherapy groups, the higher the dose of VPA, CBZ, LEV, or OXC was administered to the subjects, the higher the blood concentration of this AED was found (p <0.05); no relationship was observed between the daily dose of AED administered and the blood concentration with LTG and TPM (p> 0.05).
- The mean of blood ALT, AST levels in all study groups analyzed corresponded to the normal levels of these enzymes. There was no statistically significant difference (p>0,05) between liver enzyme (ALT, AST) levels among different AED groups in the monotherapy and polytherapy and also between subjects with AED which are metabolised in liver (VPA, CBZ, OXC, LTG) and subjects with AED that main metabolisation system is not in liver (LEV, TPM). In the general study group, the mean blood Na level remained normal. Patients treated with AED in combination with CBZ had lower blood Na levels than subjects treated without CBZ (p <0.05).
- Hyponatremia was observed in ~ ¼ subjects in the general cohort of CBZ and / or OXC subjects. Mild hyponatremia occurred in ~ 1/5, moderate - ~ 6% subjects, severe hyponatremia was not observed in any of the subjects. There was no statistically significant difference (p> 0.05) between the CBZ, OXC, and CBZ and OXC groups of subjects with hyponatremia.
- The higher the daily dose of VPA in patients treated with VPA in monotherapy, the higher the blood ALT level was found (r = 0.371, p <0.05). There was no statistically significant association between VPA, CBZ or OXC, used alone or in combination with another AED and the level of liver enzymes in the blood (p> 0.05).
