Tyrimo tikslas: Nustatyti prognozinius psichosocialinius ir biocheminius diabetinės retinopatijos veiksnius. Tyrimo uždaviniai: 1) Įvertinti pacientų, sergančiųjų 2 tipo cukriniu diabetu ir diabetine retinopatija, sampratą apie cukrinį diabetą ir diabetinę retinopatiją. 2) Išanalizuoti cukrinio diabeto kontrolės ir diabetinės retinopatijos išreikštumo sąsajas. 3) Palyginti regos funkcijas bei akių morfologinių pokyčius tarp sergančiųjų paprastąja, preproliferacine ir proliferacine diabetine retinopatija. 4) Palyginti sergančiųjų diabetine retinopatija ir nesergančiųjų cukriniu diabetu glikozilinto nago proteino tyrimo rezultatus. 5) Įvertinti ryšį tarp glikozilinto nago proteino sugerties ir glikozilinto hemoglobino kiekio. Tyrimo metodika: Įtraukti LSMUL KK Akių ligų klinikos pacientai, sergantys 2 tipo cukriniu diabetu (CD) su išreikšta diabetine retinopatija (n=43) ir pagal amžių ir lytį tiriamųjų grupę atitinkanti kontrolinė grupė (n=28). Apklausos būdu išsiaiškinta tiriamųjų samprata apie CD ir diabetinę retinopatiją (DR), surinktiišsamaus oftalmologinio ištyrimo duomenys. Glikozilinimo procesas vertintas nagų nuokarpose atliekant in vitro glikacijos ir deglikacijos etapus. Ištyrus nuokarpas ATR-FTIR spektroskopu apskaičiuota infraraudonųjų spindulių sugertis dominančiame regione, o nagų nuokarpų tirpalų sugertis vertinta NanoDrop spektrofotometru. Sugerties skirtumai prieš ir po paveikimo fermentu palyginti tarp sergančiųjų DR ir kontrolinės grupių. Tyrimo rezultatai: Pateikdami atsakymus apie diabeto sampratą, 28 pacientai (65,12 %) bent du atsakymus iš trijų atsakė teisingai, šių pacientų regėjimo aštrumas buvo geresnis (0,40±0,29 vs. 0,71±0,27, p=0,008), o centrinės tinklainės dalies paburkimas mažesnis (253±24,62 vs. 393±109,08, p=0,001) nei teisingai neatsakiusiųjų ar teisingai atsakiusiųjų į vieną klausimą. Aukštesnįjį ar aukštąjį išsilavinimą turinčių pacientų grupėje hipoglikemijos epizodai buvo retesni nei pagrindinį ar vidurinį išsilavinimą turinčiųjų grupėje (39,1 % vs. 50 %, χ2=0,21, p=0,58); glikozilinto hemoglobino reikšmės proliferacinės DR grupėje buvo aukštesnės nei paprastos ir preproliferacinės DR grupėse, tačiau skirtumai nebuvo statistiškai reikšmingi (χ2=0,07, p=0,79). Tinklainė ties fovea ir parafovea buvo storesnė tarp sergančiųjų proliferacine DR forma, lyginant su sergančiais paprastąja (p=0,04) ir preproliferacine DR (p<0,001). Nustatytos slenkstinės reikšmės, kurioms esant galima prognozuoti proliferacinę DR: 267 µm storis ties fovea, 305,5 µm – ties parafovea (GS 5,182, 95 %, PI: 1,512–17,761; p=0,006). Glikozilinto nago proteino sugerties reikšmė didesnė nei 83,00 % didina CD ir DR pasireiškimo galimybę (GS=11,4, 95 %, PI: 3,296–39,425, p<0,001). Sergančiųjų CD ir DR vidutinė glikozilinto nago proteino sugertis buvo didesnė nei kontrolės grupėje (88,11±4,60 vs. 81,60±6,98 %, p<0,001). Nago proteino sugerties reikšmė nekoreliavo su glikozilinto hemoglobino HbA1c kiekiu. Tyrimo išvados: 1) Sergantieji CD ir DR pacientai, turintys geresnę sampratą apie ligą, pasižymėjo palankesniais DR išreikštumo rodikliais. 2) Statistiškai reikšmingų CD kontrolės ir DR išreikštumo sąsajų nenustatėme. 3) Sergančiųjų proliferacine DR diabetinis geltonosios dėmės paburkimas buvo statistiškai reikšmingai didesnis nei sergančiųjų paprastąja ir preproliferacine ligos forma. Nustatytas prognozinis proliferacinės DR veiksnys – slenkstinė 267 m tinklainės storio ties fovea reikšmė, leidžiantis teisingai prognozuoti proliferacinę DR 85 % jautrumu ir 75 % specifiškumu. 4) Sergančiųjų CD ir DR vidutinė glikozilinto nago proteino sugertis buvo statistiškai reikšmingai didesnė nei kontrolės grupėje. Prognozinis CD ir DR veiksnys – slenkstinė 83,00 % glikozilinto nago sugerties reikšmė, 11,4 karto didinanti CD ir DR pasireiškimo galimybę. 5) Glikozilinto nago proteino sugerties reikšmės nekoreliavo su glikozilinto hemoglobino kiekiu.
Aim: Determine the predictive psychosocial and biochemical factors of diabetic retinopathy. Objectives: 1) Assess the comprehension of diabetes mellitus and diabetic retinopathy in patients with type 2 diabetes and diabetic retinopathy. 2) Analyze the link between management of diabetes and severity of diabetic retinopathy. 3) Compare visual functions and morphological changes of eyes between patients with simple, pre-proliferative and proliferative diabetic retinopathy. 4) Compare results of glycated nail protein tests between patients with diabetic retinopathy and people with no history of diabetes. 5) Evaluate the relationship between absorption of glycated nail protein and amount of glycated hemoglobin. Methods: Study group consisted of patients with type 2 diabetes (CD) with expressed diabetic retinopathy (n=43) and age and gender matched control group (n=28). Each patient filled in a questionnaire to assess their understanding about diabetes and diabetic retinopathy, and each patient had undergone a comprehensive opfthalmologic examination. Glycation process in nail clippings was evaluated in stages of in vitro glycation and deglycation. ATR-FTIR spectroscopy was used to calculate infrared absorption in the region of interest, and the absorption of solutions with nail clippings was evaluated by NanoDrop spectrophotometry. Absorption differences before and after exposure to the enzyme were compared between patients with DR and control groups. Results: When responding to the concept of diabetes, 28 patients (65,12 %) answered at least two of the three responses correctly, their visual acuity was better (0,40±0,29 vs. 0,71±0,27 p=0,008), and the edema of the central retina was lower (253±24,62 vs. 393±109,08 p=0,001) than those who did not answer correctly or correctly answered only one question. Hypoglycaemic episodes were less common in patients with upper or higher education than in the group with primary or secondary education (39,1 % vs. 50 %, χ2=0,21, p=0,58). The glycated hemoglobin values in the proliferative DR group were higher than in the normal and pre-proliferative DR groups, but the differences were not statistically significant (χ2=0,07, p=0,79). The retina at fovea and parafovea was thicker in patients with proliferative DR compared to those with plain (p=0,04) and pre-proliferative DR (p<0,001). The threshold values for predicting proliferative DR: 267 µm thickness at fovea, 305,5 µm for parafovea (GS 5,182, 95 % CI: 1,512–17,761; p=0,006) have been set. The absorption of glycated nail protein in excess of 83,00 % increases the chance of developing DM and DR (GS=11,4, 95 %, PI: 3,296–39,425, p<0,001). The absorption of glycated nail protein in patients with DM and DR was higher than in the control group (88,11±4,60 % vs. 81,60±6,98 %, p<0,001). The absorption of nail protein did not correlate with the level of glycated hemoglobin HbA1c. Conclusions: 1) Patients with DM and DR who had a better understanding about their disease, had less severe form of DR. 2) We did not establish a statistically significant relationship between DM control and DR severity. 3) Macular edema had a statistically significant increase in proliferative DR diabetic patients compared to those with plain and pre-proliferative disease forms. The prognostic factor for proliferative DR was determined – a threshold value of 267 μm retinal thickness at fovea, which allows accurate prediction of the proliferative DR of 85 % sensitivity and 75 % specificity. 4) The average absorption of glycated nail protein in patients with DM and DR was statistically significantly higher than in the control group. The predictive factor for DM and DR is the threshold value of 83,00 % of glycated nail protein absorption, which increases the chance of developing DM and DR by 11,4 times. 5) The absorption of glycated nail protein did not correlate with amount of glycated hemoglobin.
