Tyrimo tikslas: Įvertinti bei palyginti atviro chirurginio ir endovaskulinio neplyšusios pilvo aortos aneurizmos gydymo metodų rezultatus Lietuvos sveikatos mokslų universiteto ligoninės Kauno klinikų Širdies, krūtinės ir kraujagyslių chirurgijos klinikoje. Tyrimo uždaviniai: 1. Išanalizuoti pacientų, gydytų dėl neplyšusių pilvo aortos aneurizmų, demografinius, atviro chirurginio ir endovaskulinio gydymo duomenis. 2. Įvertinti ir palyginti šių gydymo metodų komplikacijas ir jų dažnį. 3. Įvertinti ir palyginti mirčių dažnį taikant šiuos gydymo metodus. Tyrimo metodai: Gavus Lietuvos sveikatos mokslų universiteto Bioetikos centro leidimą (BEC-MF-246), buvo atliktas retrospektyvinis tyrimas. Tyrime išanalizuotos 118 pacientų, gydytų LSMUL KK Širdies, krūtinės ir kraujagyslių chirurgijos klinikoje dėl neplyšusios pilvo aortos aneurizmos 2018-2021 laikotarpiu, ligos istorijos bei atlikta statistinė analizė. Tyrimo rezultatai: Tyrimo imtį sudarė 118 pacientų, iš kurių 83% (n=97) buvo vyrai ir 17% (n=21) – moterys. Amžiaus vidurkis buvo 73 metai (SN=7,66). 53% (n=63) pacientų buvo operuoti taikant atvirą operaciją, o EVAR buvo taikytas 47% (n=55) pacientų. Perioperacinių komplikacijų dažnis atvirų operacijų grupėje buvo 36,5% (n=23), o EVAR grupėje – 16,2% (n=9). Buvo rastas statistiškai reikšmingas silpnas ryšys tarp gydymo metodo ir perioperacinių komplikacijų dažnio (Cramér's V – 0,249; df=1; p=0,007). Pooperacinių komplikacijų dažnis atvirų operacijų grupėje buvo 10,3% (n=6), o EVAR grupėje – 32,1% (n=17). Buvo rastas statistiškai reikšmingas silpnas ryšys tarp gydymo būdo ir pooperacinių komplikacijų dažnio (Cramér's V – 0,249; df=1; p=0,009). 7 atvejai pasibaigė letaliai. Atvirų operacijų grupėje mirštamumas buvo 7,9% (n=5), EVAR grupėje - 3,8% (n=2). Dažniausia mirties priežastis – generalizuotos infekcijos sukeltas sepsinis šokas. Tyrimo išvados: 1. Tarp 118 pacientų, gydytų 2018-2021 dėl neplyšusios pilvo aortos aneurizmos, didžiausią dalį sudarė vyresnio amžiaus vyrai. 2. Dažniausios perioperacinės komplikacijos buvo generalizuota infekcija ir kraujavimas atvirų operacijų grupėje bei postimplantacinis sindromas EVAR grupėje. Perioperacinių komplikacijų dažnis buvo didesnis taikant atvirą operaciją ir buvo rasta statistiškai reikšminga silpna koreliacija (r=0,249; df=1; p=0,007). Dažniausia pooperacinė komplikacija atvirų operacijų grupėje – pooperacinė išvarža, o EVAR grupėje – endoleak II. Pooperacinių komplikacijų dažnis buvo didesnis taikant EVAR ir buvo rasta statistiškai reikšminga silpna koreliacija (r=0,249; df=1; p=0,009). 3. Bendras mirštamumas buvo 5,9%.
Author: Verner Šneider Title : Comparison of surgical methods and results in the treatment of unruptured abdominal aortic aneurysms. Aim of study: To evaluate and compare the results of open surgical and minimally invasive treatment of unruptured abdominal aortic aneurysm in the Department of Cardiac, Thoracic and Vascular Surgery, Lithuanian University of Health Sciences Hospital Kaunas Clinics (LUHSH KC). Objectives: 1. To analyse the demographics of patients surgically treated for unruptured abdominal aortic aneurysms. 2. To assess and compare the complications and their incidence of these treatment modalities. 3. To assess and compare the incidence of death after treatment of unruptured abdominal aortic aneurysms. Methods: A retrospective study was conducted with the permission of the Bioethics Centre of the Lithuanian University of Health Sciences (BEC-MF-246). The statystical analysis of the medical records of 118 patients treated at the Cardiac, Thoracic and Vascular Surgery Clinic of LSMUL KK for unruptured abdominal aortic aneurysm between 2018 and 2021 was performed. Results: The study sample consisted of 118 patients, 83% (n=97) were male and 17% (n=21) were female. The mean age was 73 years (SD=7,66). 53% (n=63) of patients underwent open surgery and EVAR was performed in 47% (n=55) of cases. The rate of perioperative complications was 36.5% (n=23) in the open surgery group and 16.2% (n=9) in the EVAR group with statistically significant weak association found between the treatment modality and the rate of perioperative complications (Cramér's V - 0.249; df=1; p=0.007). The rate of postoperative complications was 10.3% (n=6) in the open surgery group and 32.1% (n=17) in the EVAR group with statistically significant weak association found between the treatment modality and the rate of postoperative complications (Cramér's V - 0.249; df=1; p=0.009). In the open surgery group, the mortality rate was 7.9% (n=5), and 3.8% (n=2) in the EVAR group (7 cases in total). The most common cause of death was septic shock due to generalised infection. Conclusions: 1. Among the 118 patients treated for unruptured abdominal aortic aneurysm in 2018-2021, the majority were older men. 2. The most common perioperative complications were generalised infection and bleeding in the open surgery group and post-implantation syndrome in the EVAR group. The incidence of perioperative complications was higher in the open surgery group and a statistically significant weak correlation was found (r=0.249; df=1; p=0.007). The most common postoperative complication in the open surgery group was postoperative hernia, whereas in the EVAR group it was endoleak II. The rate of postoperative complications was higher with EVAR and a statistically significant weak correlation was found (r=0.249; df=1; p=0.009). 3. The overall mortality rate was 5.9%.
