Darbo pavadinimas: Pacientų, kuriems implantuotas EKS, veiksnių įtakojančių jų gyvenimo kokybę ir reabilitacijos efektyvumą vertinimas II reabilitacijos etape Darbo tikslas: Ištirti ir įvertinti pacientų, kuriems implantuotas EKS, kardiovaskulinės rizikos veiksnius, kurie daro įtaką mirtingumui nuo ŠKL, gyvenimo kokybę ir reabilitacijos efektyvumą, atsižvelgiant į lytį ir amžių. Darbo uždaviniai: 1. Įvertinti pacientų, kuriems implantuotas EKS, rizikos veiksnius, darančius įtaką mirtingumui nuo ŠKL, naudojant SCORE rizikos schemą, atsižvelgiant į lytį, amžių. 2. Įvertinti pacientų, kuriems implantuotas EKS, gyvenimo kokybės indeksą, pagal lytį ir amžių. 3. Įvertinti ir palyginti pacientų, kuriems implantuotas EKS, fizinio pajėgumo pokytį taikant šešių minučių testą, atsižvelgiant į lytį, amžių. 4. Įvertinti pacientų GKI sąsajas atsižvelgiant į lytį, amžių, funkcinį pajėgumą bei kardiovaskulinę riziką. Tyrimo metodai: tyrimas atliktas LSMUL Kauno klinikose, Kulautuvos reabilitacijos ligoninėje 2022-2023 metais. Buvo apklausta 20 pacientų, kuriems pirmą kartą implantuotas EKS. Buvo vertinami rizikos veiksniai, darantys įtaką mirtingumui nuo ŠKL, naudojant SCORE rizikos schemą, gyvenimo kokybė, naudojant standartizuotą Ferrans and Powers gyvenimo kokybės indekso širdies versijos klausimyną, reabilitacijos efektyvumas buvo vertinamas pagal fizinio pajėgumo pokytį, taikant šešių minučių ėjimo testą. Buvo ieškoma sąsajų tarp šių rodiklių ir atlikta gautų duomenų statistinė analizė, duomenų palyginimas tarp grupių. Tyrimo rezultatai: Tyrime dalyvavo 20 tiriamųjų - 14 vyrų ir 6 moterys. Amžiaus vidurkis buvo 77,15 (7,842) metai. SCORE balų vidurkis 5,2 (3,139). Rastas statistiškai reikšmingas skirtumas tarp SCORE balų lyginant pagal lytį (p=0,046) ir amžių (p = 0,002). Nustatytas GKI vidurkis 22,9 (3,086). Statistiškai reikšmingo skirtumo tarp lyties ir GKI nerasta (p = 0,09), tačiau rastas statistiškai reikšmingas skirtumas tarp amžiaus grupių ir GKI (p = 0,034). 6 minučių ėjimo testo rezultatai statistiškai reikšmingai skiriasi (T- = 1; T+ = 170; p-reikšmė < 0,001). Statistiškai reikšmingo skirtumo, lyginant pagal amžių (U=19, p-reikšmė = 0,279) ir lytį (U=19, p-reikšmė = 0,279) tarp nueitų metrų skirtumo, rasta nebuvo. GKI, SCORE rizika ir reabilitacijos efektyvumas šiame tyrime tarpusavyje nekoreliavo. Išvados: 1) Bendras visų tiriamųjų SCORE balų vidurkis buvo 5,2 (3,139). Moterys ir jaunesnio amžiaus pacientai turi mažesnę tikimybę numirti nuo ŠKL per 10 metų. 2) Nustatytas GKI vidurkis 22,9 (3,086). Lytis nedaro įtakos gyvenimo kokybei, o jaunesnis amžius lemia aukštesnį GKI indeksą. 3) Vertinant reabilitacijos efektyvumą, taikant šešių minučių ėjimo testą, nustatėme, kad fizinė būklė pagerėjo – nueitas atstumas išaugo. Amžius ir lytis daro vienodą įtaką reabilitacijos efektyvumui. 4) GKI, SCORE skalės balai ir reabilitacijos efektyvumas tarpusavyje, šiame tyrime nekoreliavo.
Research title: Assessment of Factors Affecting Patient’s Quality of Life and Rehabilitation Efficiency After Pacemaker Implantation in the II Rehabilitation Stage Research aim: To investigate and evaluate the cardiovascular risk factors of patients who have been implanted with pacemakers, which influence the mortality from CVD, the quality of life and the effectiveness of rehabilitation, taking into account gender and age. Research objectives: 1. To evaluate the risk factors affecting the mortality from CVD in patients implanted with a pacemaker using the SCORE table, taking into account gender and age. 2. To evaluate the quality of life index of patients implanted with a pacemaker, according to gender and age. 3. To evaluate and compare the change in the physical capacity of patients implanted with a pacemaker using the six minutes walking test, taking into account gender and age. 4. To evaluate the correlations between QLI of patients considering gender, age, functional capacity and cardiovascular risk. Research methods: The research was performed at LSMU Kaunas clinics, Kulautuva Rehabilitation Hospital in 2022-2023. 20 patients who had a peacemaker for the first time were interviewed. Quality of life was assessed using a standardized heart rate questionnaire for the Ferrans and Powers Quality of Life Index, and rehabilitation efficiency was evaluated by the change in physical capacity using a six-minute walk test. Correlations between these indicators were sought and statistical analysis of the obtained data was performed, data were compared between groups. Research results: there were 20 participants in this study - 14 men and 6 women. The average age of the trial was 77.15 (7.842) years. For this study sample, the average score of SCORE was 5.2 (3.139) - a high risk of dying from CVD within 10 years, which is 5-9 percent. A statistically significant difference was found between male and female SCORE scores (p=0.046). When comparing the SCORE between age groups, a statistically significant difference was found (p = 0.002). The QLI average was determined to be 22.9 (3.086). Thus, it can be said that the quality of life of the subjects is quite good. No statistically significant difference was found between genders in QLI (p = 0.09), but a statistically significant difference was found between age groups and QLI (p = 0.034). The results of the 6 minutes walking test are statistically significantly different (T- = 1; T+ = 170; p-value < 0.001). There was no statistically significant difference between the distance travelled by age (U=19, p-value = 0.279). The difference in meters walked between men and women is not different (U = 20.5; p-value = 0.151). QLI, SCORE risk and rehabilitation efficiency were not correlated in this study. Conclusions: 1) The overall SCORE mean for all subjects was 5.2 (3.139). Women and younger patients are less likely to die from CVD within 10 years. 2) The QLI average was determined to be 22.9 (3.086). Gender does not affect the quality of life, and younger age leads to a higher QLI. 3) When assessing the effectiveness of rehabilitation, using the six-minute walk test, we found that the physical condition improved - the distance walked increased. Age and gender have an equal influence on the effectiveness of rehabilitation. 4) QLI, SCORE scale and rehabilitation efficiency did not correlate in this study.
