Darbo tikslas: nustatyti YAP1 geno amplifikacijos sąsają su pacientų, sergančių galvos ir kaklo navikais, klinikiniais duomenimis ir gydymo veiksmingumu. Darbo uždaviniai: 1. nustatyti YAP1 geno amplifikacijos dažnį pacientams, sergantiems galvos ir kaklo navikais, bei palyginti YAP1 geno amplifikacijos dažnį normaliame ir navikiniame audiniuose; 2. nustatyti YAP1 geno amplifikacijos sąsają su klinikiniais duomenimis: amžiumi, lytimi, naviko lokalizacija, T ir N būkle, diferenciacijos laipsniu (G); 3. įvertinti YAP1 geno amplifikacijos sąsają su gydymo atsaku bei įtaką laikui be ligos progreso. Tyrimo dalyviai ir metodika: tirti 27 pacientai, kurie nuo 2015 m. sausio mėn. iki 2019 m. lapkričio mėn. buvo gydyti LSMUL Kauno klinikos dėl galvos ir kaklo plokšialąstelinės karcinomos. Biopsijos metu navikiniai audiniai paimti iš pirminio naviko, normalūs audiniai – iš apatinės lūpos vidinės gleivinės. YAP1 geno kopijų skaičius nustatytas tikro laiko polimerazės grandininės reakcijos metodu. Statistinė duomenų analizė atlikta IBM SPSS Statistics 26.0 programa. Rezultatai: 18,5% pacientų nustatyta YAP1 geno amplifikacija, 74,1% pacientų – YAP1 geno kopijų skaičius normalus, o 7,4% pacientų – YAP1 geno delecija. YAP1 geno amplifikacija galvos ir kaklo navikiniame audinyje nebuvo reikšminga (p>0,05) lyginant su normaliu audiniu. YAP1 geno amplifikacijos sąsaja su pacientų amžiumi (p=0,242), lytimi (p=0,502), naviko lokalizacija (p=0,336), pirminiu naviku (p=0,277), sritiniais limfmazgiais (p=0,188) bei diferenciacijos laipsniu (p=0,179) nėra reikšminga. Vertinant gydymo atsaką tarp pacientų su nutatyta YAP1 geno amplifikacija ir tarp tų pacientų, kuriems nebuvo rasta, nėra reikšmingo skirtumo (p>0,05). Laikas be ligos progreso truko nuo 4 iki 18 mėn., vidutiniškai 9,67±7,37 mėn. YAP1 geno amplifikacijos koreliacija su laiku be ligos progreso yra stipriai neigiama ir nereikšminga (r=-0,866, p=0,333). Išvados: YAP1 geno amplifikacija nustatyta 18,5% pacientų, sergančių galvos ir kaklo navikais, ir nėra reikšminga lyginant navikinį audinį su normaliu. Nėra reikšmingos sąsajos tarp YAP1 geno amplifikacijos ir pacientų klinikinių duomenų. YAP1 geno amplifikacija neturi reikšmingos įtakos gydymo atsakui ir laikui be ligos progreso.
Purpose: to determine the association between YAP1 gene amplification and clinical data and treatment efficacy in patients with head and neck tumors. Objectives: 1. to determine the frequency of YAP1 gene amplification in patients with head and neck tumors and to compare the frequency of YAP1 gene amplification in normal and tumor tissues; 2. to determine the association between YAP1 gene amplification and clinical data: age, sex, tumor site, T and N status, degree of differentiation (G); 3. to evaluate the association of YAP1 gene amplification with treatment response and the effect on time without disease progression. Methods and study participants: 27 patients, who were treated at LUHS Hospital Kaunas Clinics for head and neck squamous cell carcinoma from January 2015 to November 2019. During the biopsy, tumor tissues were taken from the primary tumor, normal tissues - from the inner mucosa of the lower lip. YAP1 gene copy number was determined by the real-time polymerase chain reaction method. Statistical analysis was performed using IBM SPSS Statistics 26.0. Results: 18.5% of the patients had YAP1 gene amplification, 74.1% of the patients – normal YAP1 gene copy number and 7.4% of the patients – YAP1 gene deletion. YAP1 gene amplification in head and neck tumor tissue was not significant (p>0.05) compared to normal tissue. Association between YAP1 gene amplification and patients age (p=0.242), gender (p=0.502), tumor site (p=0.336), primary tumor (p=0.277), regional lymph nodes (p=0.188), and degree of differentiation (p=0.179) ) is not significant. There was no significant difference (p>0.05) in the treatment response between patients with YAP1 gene amplification and those without it. The time without disease progression lasted from 4 to 18 months, with a mean of 9.67±7.371 months. The correlation of YAP1 gene amplification with time without disease progression is strongly negative and insignificant (r=-0.866, p=0.333). Conclusions: YAP1 gene amplification was found in 18.5% of the patients with head and neck tumors and is not significant when comparing tumor tissue to normal. There is no significant association between YAP1 gene amplification and pacients clinical data. YAP1 gene amplification has no significant effect on treatment response and time without disease progression.
