Polymorphisms of Matrix Metalloproteinase Genes (MMP1 and MMP3) in Patients with BCR-ABL1-negative Myeloproliferative Neoplasms

Tikslai:
- Nustatyti MMP1 ir MMP3 genų polimorfizmų paplitimą pacientams, sergantiems BCR-ABL1 neigiamais mieloproliferaciniais navikais.
- Ištirti MMP1 ir MMP3 genų polimorfizmų ryšį su venų ir arterijų trombozės rizika.
- Išanalizuoti MMP1 ir MMP3 genų polimorfizmų ryšį su BCR-ABL1 neigiamų mieloproliferacinių navikų klinikinėmis charakteristikomis. Metodika: Tyrimo metu buvo tiriami matricos metaloproteinazės genų (MMP1 ir MMP3) polimorfizmai pacientams, sergantiems BCR-ABL1 neigiamais mieloproliferaciniais navikais (MPN). Tyrime dalyvavo 88 pacientai, kuriems diagnozuota pirminė mielofibrozė, esminė trombocitemija ir vera policitemija. Genotipų nustatymas buvo atliktas naudojant genominę DNR, išskirtą iš periferinio kraujo leukocitų. PGR-RFLP metodu nustatyti MMP1 g.-1607dupG (rs1799750) ir g.-MMP3 -1171dupA (rs3025058) polimorfizmai, vėliau įvertintas jų ryšys su trombozės rizika ir MPN laboratorinėmis charakteristikomis. Statistinė analizė atlikta naudojant SPSS (20.0 versija). Rezultatai: Nustatyta, kad MMP1 g.-1607dupG genotipų 1G/1G, 1G/2G ir 2G/2G dažniai buvo atitinkamai 29,60%, 42,00% ir 28,40%. MMP3 g.-1171dupA genotipų 5A/5A, 5A/6A ir 6A/6A dažniai buvo atitinkamai 22,70%, 50,0% ir 27,30%. Šis tyrimas nustatė MMP1 ir MMP3 genų polimorfizmų sąsajas su trombozės išsivystymo rizika tarp MPN pacientų. Reikšmingo ryšio tarp MMP1 ir MMP3 polimorfizmų ir trombozinių įvykių nepastebėta. Tačiau buvo tendencija tarp analizuotų polimorfizmų ir padidėjusios arterijų trombozės rizikos. Galima pridurti, kad statistiškai reikšmingo ryšio tarp analizuotų polimorfizmų ir MPN laboratorinių charakteristikų nenustatyta. Išvados: Šie stebėjimai rodo, kad MMP1 ir MMP3 polimorfizmai gali prisidėti prie trombozės rizikos BCR-ABL1 neigiamuose MPN. Turime pripažinti, kad vienas iš šio bandomojo tyrimo apribojimų yra nedidelė tiriamų pacientų grupė. Todėl reikia atlikti gilesnę analizę su didesne MPN pacientų grupe ir atskira kontroline grupe.

Objectives:
- To determine the prevalence of MMP1 and MMP3 gene polymorphisms in patients with BCR-ABL1-negative myeloproliferative neoplasms.
- To investigate the association of MMP1 and MMP3 gene polymorphisms with the risk of venous and arterial thrombosis.
- To analyze the association of MMP1 and MMP3 gene polymorphisms with the clinical characteristics of BCR-ABL1-negative myeloproliferative neoplasms. Methodology: The study researched polymorphisms of matrix metalloproteinase genes (MMP1 and MMP3) in patients with BCR-ABL1-negative myeloproliferative neoplasms (MPNs). The study consists of 88 patients diagnosed with primary myelofibrosis, essential thrombocythemia, and polycythemia vera. Genotyping was performed using genomic DNA isolated from peripheral blood leukocytes. MMP1 g.-1607dupG (rs1799750) and g.-MMP3 -1171dupA (rs3025058) polymorphisms were determined using the PCR-RFLP method, and later, their association with thrombosis risk and laboratory characteristics of MPNs were assessed. Statistical analysis was performed using SPSS (version 20.0). Results: It was found that the frequencies of the MMP1 g.-1607dupG genotypes 1G/1G, 1G/2G, and 2G/2G were 29.60%, 42.00%, and 28.40%, respectively. The frequencies of MMP3 g.-1171dupA genotypes 5A/5A, 5A/6A, and 6A/6A were 22.70%, 50.0%, and 27.30%, respectively. This research established the associations of MMP1 and MMP3 gene polymorphisms with the risk of thrombosis development among MPN patients. Significant association between MMP1 and MMP3 polymorphisms, and thrombotic events was not observed. However, there was a tendency between analyzed polymorphisms and increased risk of arterial thrombosis. It can be added that no statistically significant relationship was determined between analyzed polymorphisms and laboratory characteristics of MPNs. Conclusions: These observations suggest that MMP1 and MMP3 polymorphisms could contribute to thrombotic risk in BCR-ABL1-negative MPNs. We must admit that one of the limitations of this pilot study is a small group of analyzed patients. Therefore, there is a need for deeper analysis with a larger group of MPN patients as well as a control individual group.
