The Impact of Diabetes Mellitus on the Success, Osseointegration and Peri-implant Health of Dental Implants: a Scientific Literature Review

Aktualumas ir tikslas: Cukrinis diabetas yra rimta pasaulinė sveikatos problema, galinti neigiamai paveikti burnos sveikatą ir trukdyti dantų implantų sėkmei. Šio tyrimo tikslas buvo įvertinti implantų sėkmę, oseointegraciją ir periimplantinę sveikatą diabetu sergantiems pacientams, palyginus su nesergančiais diabetu, taip pat įvertinti glikeminės kontrolės (pagal HbA1c lygį) poveikį.
Medžiaga ir metodai: Buvo atlikta mokslinės literatūros apžvalga vadovaujantis PRISMA gairėmis. Atrinkti 2015–2025 m. publikuoti tyrimai iš PubMed, Google Scholar ir ResearchGate duomenų bazių. Buvo atrinkta vienuolika tyrimų, kuriuose lyginta implantų sėkmė, oseointegracija ir periimplantinė sveikata tarp diabetu nesergančių ir sergančių pacientų, ypatingą dėmesį skiriant gerai ir blogai kontroliuojamam diabetui.
Rezultatai: Diabetu sergančiųjų sėkmės rodikliai svyravo nuo 76,7 % iki 100 %. Diabetu nesergantys ir gerai kontroliuojami diabetikai turėjo panašius rezultatus, tačiau blogai kontroliuojami diabetikai pasižymėjo didesniu nesėkmių skaičiumi. Aukšti HbA1c lygiai buvo siejami su prasta oseointegracija, padidėjusiu kaulo nykimu ir periimplantiniu uždegimu. Įdomu tai, kad kai kuriuose tyrimuose buvo nustatyta, jog implantų įsriegimas gali teigiamai paveikti glikemijos rodiklius.
Išvada: Prasta glikeminė kontrolė padidina implantų komplikacijų riziką. Nepaisant to, gerai kontroliuojamas diabetas neturi neigiamo poveikio implantų rezultatams. Be to, yra duomenų, rodančių galimą dvikryptį ryšį tarp glikeminės kontrolės ir implantų gydymo.
Relevance and Aim: Diabetes mellitus is a serious global health problem that can negatively affect the oral health and hinder the success of dental implants. The aim of the current study was to evaluate implant success, osseointegration, and peri-implant health in diabetics compared to non-diabetic patients and assess the effect of glycaemic control by HbA1c levels.
Material and Methods: A scientific literature review was conducted as per PRISMA guidelines. Studies published from 2015 to 2025 were selected from PubMed, Google Scholar, and ResearchGate. Eleven studies that compared implant success, osseointegration, and peri-implant health among non-diabetic and diabetic patients, with emphasis given in well-controlled and poorly controlled diabetics, were selected.
Results: Success rates ranged between 76.7% and 100% in diabetics. Both non-diabetics and well-controlled diabetics performed similarly, however poorly controlled diabetics had higher rates of failure. High levels of HbA1c were associated with impaired osseointegration, and increased bone loss and peri-implant inflammation. Interestingly, there was evidence in some studies suggesting that glycaemic levels were positively enhanced by implant placement.
Conclusion: Poor glycaemic control increases the risk of implant complications. Despite this, well-controlled diabetes will not negatively impact the implant outcomes. Additionally, there is evidence towards a potential bidirectional relationship between glycaemic control and implant treatment.
