Aistė Adakevičiūtė Karščiavimo metu atsitiktinai rastos neutropenijos įtaka vaikų infekcijų rizikai. Literatūra pateikia mažai duomenų apie bakterinių komplikacijų riziką karščiuojantiems vaikams su ne chemoterapijos sukelta neutropenija, todėl gydant vaikus su atsitiktinai rasta neutropenija kyla daug klausimų apie gręsiančias bakterines komplikacijas. Tikslas: Nustatyti ryšį tarp įgytos neutropenijos bei infekcinių komplikacijų karščiuojantiems vaikams. Uždaviniai: 1. Nustatyti neutropenijos dažnį vaikams karščiavimo metu. 2. Nustatyti neutropenijos sunkumą vaikams karščiavimo metu. 3. Nustatyti bakterinių komplikacijų dažnį karščiuojantiems vaikams. 4. Nustatyti neutrofilų sąsajas su kitomis kraujo ląstelėmis. Tyrimo metodai: Atliktas perspektyvinis stebėjimo tyrimas. Atrankos kriterijus atitikę asmenys priskirti tiriamųjų arba palyginamajai grupei, ištirtas ryšys tarp įgytos neutropenijos bei bakterinių komplikacijų karščiuojantiems vaikams, atlikta rezultatų analizė. Rezultatai: Tyrime dalyvavo 348 pacientai, 1 – 36 mėnesių amžiaus. Karščiavimo metu 85,3 % tirtų vaikų atsitiktinės neutropenijos nebuvo, o neutropenija nustatyta 14,7 % tiriamųjų. Iš jų 49,0 % sudarė lengva neutropenijos forma, 33,3 % – vidutinė ir 17,6 % – sunki. Neutropenijos sunkumo formos pagal vidutinį neutrofilų skaičių tarpusavyje skyrėsi statistiškai reikšmingai (p<0,05). Vyresni vaikai serga lengvesne neutropenijos forma nei jaunesni, o berniukams sunkesnė neutropenijos forma pasitaiko dažniau nei mergaitėms, tačiau skirtumas nėra statistiškai reikšmingas (p>0,05). Neutropenijos sunkumas nepriklauso nuo karščiavimo trukmės (p>0,05) bei atsitiktinė neutropenija neturi įtakos didesnei bakterinių komplikacijų rizikai (p>0,05). Karščiuojantiems vaikams, kuriems atsitiktinė neutropenija nenustatyta, dažniau būna CRB>50 mg/l (p<0,05), o mikrobiologinis pasėlis išauga nepriklausomai nuo neutrofilų skaičiaus sumažėjimo (p>0,05). Esant neutropenijai reikšmingų sąsajų su kitais kraujo parametrais nėra (p>0,05). Tačiau kai neutrofilų skaičius kraujyje pakankamas, teigiama koreliacija nustatyta su CRB, leukocitais ir trombocitais. Išvados: 1. Atsitiktinės neutropenijos dažnis karščiavimo metu nėra didelis (14,7 %), o lengva neutropenijos forma sudaro beveik pusę (49 %) visų nustatytų neutropenijos atvejų, tačiau šios formos pasireiškimo dažnis lyginant su sunkesnėmis ligos formomis nėra reikšmingas (p>0,05). 2. Lengva neutropenijos forma nustatyta 7,2 %, vidutinė – 4,9 %, o sunki – 2,6 % visų tirtų karščiuojančių vaikų. Neutropenijos sunkumo formos pagal vidutinį ANS skyrėsi statistiškai reikšmingai (p<0,05), o karščiavimo trukmė nepriklauso nuo neutropenijos sunkumo formos (p>0,05). 3. Bakterinių komplikacijų dažnis karščiuojantiems vaikams nėra didelis, o CRB>50 mg/l dažniau būna tiems karščiuojantiems vaikams, kuriems atsitiktinė neutropenija nenustatyta (p<0,05). 4. Esant atsitiktinei neutropenijai reikšmingų sąsajų su kitais kraujo parametrais nenustatyta (p>0,05). Kai neutropenijos nėra, teigiama koreliacija nustatyta su CRB, leukocitais ir trombocitais.
Aistė Adakevičiūtė The risk of infection in children with fever and randomly found neutropenia. Literature provides little information about the risk of bacterial complications of febrile children with non-chemotherapy induced neutropenia, therefore treatment of children with accidentally found neutropenia is always raising a lot of questions. Purpose: Establish connection between the acquired neutropenia and infectious complications of febrile children. Tasks: 1. Set the frequency of neutropenia of febrile children. 2. Set the severity of neutropenia of febrile children. 3. Set the frequency of bacterial complications of febrile children. 4. Set the connection between neutrophils and other blood cells. Research methods: A prospective observational study is completed. Children who fitted selection criteria are assigned to the test or comparison group, the relation between acquired neutropenia and bacterial complications of febrile children has been investigated, analysis of the results is completed. Results: The study included 348 patients, 1 - 36 months of age. During fever 85.3 % of investigated children had no random neutropenia, while in 14.7 % of targeted children neutropenia was found. Out of these, 49.0 % accounted for mild neutropenia, 33.3 % – moderate and 17.6 % - severe neutropenia. The severity of neutropenia shaped by the average number of neutrophils, differ in a statistically significant (p <0.05). Older children had less severe form of neutropenia than younger ones while more severe form of neutropenia occurs more often for boys than for girls, but the difference was not statistically significant (p> 0.05).The duration of fever does not depend from the severity of neutropenia (p <0.05) and random neutropenia does not affect the greater risk of bacterial complications (p> 0.05). Febrile children without found random neutropenia, more often has CRP> 50 mg/l (p <0.05), and microbiological culture grows regardless of the reduction in the number of neutrophils (p> 0.05). With occurred neutropenia there are no significant links to other blood parameters (p> 0.05). But when the neutrophil count is sufficient, a positive correlation was found with CRP, leukocytes, and platelets. Conclusions: 1. The frequency of random neutropenia of febrile children is not high (14.7 %), and mild form of neutropenia accounts for nearly half (49 %) of all identified cases of neutropenia, however incidence frequency of this form in comparison to more severe form of the disease is not significant (p > 0.05). 2. Mild form of neutropenia is determined for 7.2 %, moderate – 4.9 %, severe – 2.6 %. out of all investigated febrile children. The severity of neutropenia shaped according to the average ANC differed significantly (p <0.05), while the duration of fever does not depend from neutropenia severity form (p <0.05). 3. The frequency of bacterial complications of febrile children is not high, and CRP> 50 mg / l, more often for febrile children who has no random neutropenia (p <0.05). 4. In case of accidental neutropenia significant links to other blood parameters were not found (p> 0.05). When there is no neutropenia, a positive correlation was found with CRP, leukocytes, and platelets. Keywords: random neutropenia, fever, infectious complications, children.
