Tyrimo uždaviniai: 1. Įvertinti pacienčių, operuotų dėl krūties vėžio ir turinčių BRCA1/BRCA2 genetinių mutacijų dažnį. 2. Įvertinti ir palyginti pacienčių su BRCA genų mutacijomis operacijų apimtis ir rezultatus skirtingose amžiaus grupėse (iki 50 m. ir virš 50 m.) 3. Nustatyti pacienčių, operuotų dėl krūties vėžio su BRCA1/BRCA2 mutacijomis pooperacinio gydymo rezultatus. 4. Įvertinti pacienčių su BRCA1 ir BRCA2 mutacijomis neoadjuvantinio gydymo įtaką pooperaciniams gydymo rezultatams. Tyrimo metodai: atliktas retrospektyvinis tyrimas LSMUL KK. Vertinta BRCA genų mutacijų dažnis, įtaka operacijos pasirinkimui ir pooperaciniams gydymo rezultatams, neoadjuvantinės chemoterapijos įtaka pooperaciniams gydymo rezultatams. Statistinė analizė atlikta naudojant IBM SPSS Statistics 29.0.0.0 programinę įrangą. Tyrimo dalyviai: 68 moterys, sergančios krūties vėžiu ir turinčios BRCA1 ir/ar BRCA2 genų mutacijas, kurios 2018-2021 metais operuotos LSMUL KK Chirurgijos klinikos krūties chirurgijos skyriuje. Tyrimo rezultatai: BRCA genų mutacijos nustatytos 3,93 % pacienčių. 63,2 % turėjo BRCA1, 35,3 % BRCA2, 1,5 % abiejų genų mutacijas. 72,1 % pacienčių jaunesnės, nei 50 m. 47,1 % pacienčių nustatytas TNKV ( tarp jų 71,9 % jaunesnės nei 50 m.). Jaunesnėms nei 50 m. pacientėms reikšmingai daugiau atlikta abipusių poodinių mastektomijų 30,6 %, vyresnėms nei 50 metų, reikšmingai daugiau vienpusių lumpektomijų (p = 0,047). Komplikacijų dažnis siekė 16,2 % ( seroma 8,8 %, hematoma 2,94 %, odos lopo kraujotakos sutrikimai 1,5 %, infekcija 2,9 %). NAC gavo 60,3 % ( tarp jų teigiamas pCR 23,7 %) Tyrimo išvados: 1. BRCA1 ir/ar BRCA2 genų mutacijos nustatytos 3,93 % moterų. Mutacijų dažnis: BRCA1 63,2 % atvejų, BRCA2 35,3 %, o abiejų genų 1,5 % tiriamųjų. 2. Pacientėms jaunesnėms nei 50 m. atlikta reikšmingai daugiau profilaktinių KV riziką mažinančių operacijų lyginant su vyresnėmis nei 50 m. (p = 0,007). 3. 83,8 % pacienčių pooperacinių komplikacijų nenustatyta. 4. TNKV geriausiai reagavo į NAC (p = 0,025). Nenustatyta, kad NAC turėtų reikšmingos įtakos pooperacinių komplikacijų rizikai (p = 0,672).
Author: Jonas Normantas Title: Analysis of Immediate Surgical Treatment Outcomes in Patients with Breast Cancer and BRCA Genes Mutation in 2018-2021Years Aim: To evaluate the surgical treatment approaches and the immediate results for patients who have breast cancer and possess mutations in the BRCA1 and BRCA2 genes. Objectives: 1. To determine the occurrence rate of BRCA1 and BRCA2 mutations in patients who underwent breast cancer surgery and had genetic mutations. 2. To assess and compare the extent and outcomes of surgical interventions in patients with BRCA genes mutations in groups: under 50 and those over 50. 3. To ascertain the outcomes of surgeries and postoperative therapies for patients who underwent breast cancer surgery and have BRCA1/BRCA2 mutations. 4. To evaluate the impact of neoadjuvant treatment on the outcomes of postoperative treatment for patients with BRCA genes mutations who underwent surgery. Methods: At LUHSH KK, a retrospective study was carried out to evaluate the occurrence rate of BRCA genes mutations, the impact of these mutations on the selection of surgical procedures and the results of postoperative treatment, as well as the effect of neoadjuvant chemotherapy on postoperative treatment outcomes. The data was analyzed using IBM SPSS Statistics 29.0.0.0 software. Patients: Between 2018 and 2021, 68 women with breast cancer and mutations in the BRCA1 and/or BRCA2 genes underwent surgery at the breast surgery department of LUHSH KK Surgery Clinic. Results: BRCA gene mutations were detected in 3.93% of cases, with 63.2% having mutations in BRCA1, 35.3% in BRCA2, and 1.5% in both genes. 72.1% of patients were younger than 50 years old, and 47.1% were diagnosed with TNKV, among whom 71.9% were younger than 50 years old. Patients younger than 50 years old were more likely to receive bilateral subcutaneous mastectomies (30.6%), patients older than 50 years old were more likely to undergo unilateral lumpectomies (p = 0.047). The frequency of complications was 16.2%: seroma (8.8%), bleeding (2.94%), skin flap circulatory disorders (1.5%), infection (2.9%). Neoadjuvant chemotherapy was administered to 60.3% of patients, and among them, 23.7% achieved a positive pCR Conclusions: In 3.93% of women, BRCA1 and/or BRCA2 gene mutations were detected with the majority having BRCA1 mutations (63.2%), followed by BRCA2 mutations (35.3%), and mutations in both genes (1.5%).2. Women under the age of 50 had a significantly higher incidence of prophylactic CVD risk-reducing operations than those over the age of 50 (p = 0.007). 3. Postoperative results were positive with 83.8% of patients not experiencing any complications. 4. TNKV responded best to neoadjuvant chemotherapy (p = 0.025), while there was no significant association found between neoadjuvant chemotherapy and the risk of postoperative complications (p = 0.672).
