Ištirti krūties vėžiu sergančių pacienčių NOXA geno polimorfizmus ir įvertinti jų sąsajas su krūties vėžio patomorfologinėmis charakteristikomis bei ligos eiga. Tyrimo uždaviniai. 1. Nustatyti NOXA geno polimorfizmus (rs78800940 ir rs1041978) krūties vėžiu sergančių pacienčių grupėje. 2. Įvertinti NOXA geno variantų sąsajas su klinikinėmis ir morfologinėmis krūties vėžio naviko charakteristikomis. 3. Išanalizuoti, kaip NOXA geno polimorfizmai daro įtaką krūties vėžio eigą. Tyrimo metodai. NOXA geno polimorfizmai rs78800940 ir rs1041978 buvo tirti naudojant tikro laiko polimerazės grandininės reakcijos (TL-PGR) metodą. Tyrimo atlikimui buvo naudojami DNR mėginiai (imtis 201), kurie buvo išskirti iš krūties vėžiu sergančių pacienčių kraujo. Sąsajų tarp NOXA geno polimorfizmų ir patomorfologinių savybių bei pacienčių ligos eigos įvertinimas atliktas naudojant „IBM SPSS Statistics“ programos paketą. Tyrimo rezultatai. Statistiškai reikšminga sąsaja nustatyta tarp NOXA rs78800940 polimorfizmo ir estrogenų receptorių (ER) raiškos, A alelio nešiotojoms dažniau nustatyti ER neigiami navikai. Daugialypės logistinės regresijos analizė parodė, kad ši sąsaja išliko reikšminga ir koregavus pagal amžių, naviko dydį bei kitus klinikinius veiksnius. Tarp rs1041978 polimorfizmo ir krūties vėžio patomorfologinių savybių reikšmingų nepriklausomų sąsajų nenustatyta. Taip pat statistiškai reikšmingų sąsajų nenustatyta tarp NOXA rs78800940 ir rs1041978 polimorfizmų bei ligos eigos, atkryčio, metastazių ar bendro išgyvenamumo. Literatūroje šie polimorfizmai krūties vėžio kontekste tirti labai mažai, todėl šis darbas gali būti laikomas vienu pirmųjų, vertinančių jų reikšmę krūties vėžio klinikinėms ir morfologinėms savybėms. Išvados. 1. NOXA rs1041978 polimorfizmo GG genotipas tarp tiriamųjų buvo dažniausias (77,1 %), TT – rečiausias (3,5 %), o G alelis nustatytas dažniau nei T (atitinkamai 86,8 % ir 13,2 %). NOXA rs78800940 polimorfizmo GG genotipas buvo dažniausias (49,8 %), AA – rečiausias (8,5 %), o G alelis nustatytas dažniau nei A (atitinkamai 70,6 % ir 29,4 %). 2. Nustatyta statistiškai reikšminga sąsaja tarp NOXA rs78800940 polimorfizmo ir estrogenų receptorių raiškos. A alelio nešiotojos dažniau turėjo ER neigiamus navikus lyginant su A alelio neturinčiomis pacientėmis (atitinkamai 62,5 % ir 37,5 %; p = 0,023). 3. Statistiškai reikšmingų sąsajų tarp NOXA rs78800940 ir rs1041978 polimorfizmų bei krūties vėžiu sergančių pacienčių ligos eigos nenustatyta.
Author of the thesis. Ieva Tveragaitė. Title of the thesis. „The impact of NOXA gene polymorphisms on breast cancer patients' clinical characteristics and disease progression.“ Aim of the study. To investigate NOXA gene polymorphisms in patients with breast cancer and to evaluate their associations with the pathomorphological characteristics of breast cancer and the course of the disease. Objectives of the study. 1. To identify NOXA gene polymorphisms (rs78800940 and rs1041978) in a group of patients with breast cancer. 2. To assess the associations between NOXA gene variants and the clinical and morphological characteristics of breast cancer tumors. 3. To analyze how NOXA gene polymorphisms influence the course of breast cancer. Methods. The NOXA gene polymorphisms rs78800940 and rs1041978 were analyzed using the real-time polymerase chain reaction (RT-PCR) method. DNA samples (n = 201), isolated from the blood of breast cancer patients, were used for the study. The assessment of associations between NOXA gene polymorphisms, pathomorphological characteristics, and the clinical course of the patients’ disease was performed using the IBM SPSS Statistics software package. Results. A statistically significant association was found between the NOXA rs78800940 polymorphism and estrogen receptor (ER) expression, with A allele carriers more frequently presenting with ER-negative tumors. Multiple logistic regression analysis showed that this association remained significant after adjustment for age, tumor size, and other clinical factors. No significant independent associations were found between the rs1041978 polymorphism and the pathomorphological characteristics of breast cancer. No statistically significant associations were found between the NOXA rs78800940 and rs1041978 polymorphisms and disease progression, recurrence, metastasis, or overall survival. These polymorphisms have been very rarely studied in the context of breast cancer, therefore this study may be considered one of the first to evaluate their significance in relation to the clinical and morphological characteristics of breast cancer. Conclusions. 1. The GG genotype of the NOXA rs1041978 polymorphism was the most frequent among the study participants (77.1%), while the TT genotype was the rarest (3.5%). The G allele was detected more frequently than the T allele (86.8% and 13.2%, respectively). For the NOXA rs78800940 polymorphism, the GG genotype was also the most frequent (49.8%), while the AA genotype was the rarest (8.5%). The G allele was detected more frequently than the A allele (70.6% and 29.4%, respectively). 2. A statistically significant association was found between the NOXA rs78800940 polymorphism and estrogen receptor (ER) expression. Carriers of the A allele more frequently had ER-negative tumors compared to patients without the A allele (62.5% vs. 37.5%; p = 0.023). 3. No statistically significant associations were found between the NOXA rs78800940 and rs1041978 polymorphisms and disease progression in patients with breast cancer.
