Tyrimo tikslas. Pateikti geros medicinos praktikos rekomendacijas, teikiant pagalbą artimiesiems, giminaičiams, atsižvelgiant į etines ir teisines nuostatas.
Tyrimo uždaviniai. 1. Nustatyti, ar sveikatos specialisto teikiama pagalba artimajam nepažeidžia etiškumo ir moralės principų. 2. Įvertinti, ar teikiama medicininė pagalba artimam asmeniui turi teisinių apribojimų. 3. Išsiaiškinti, kiek gydytojų anesteziologų-reanimatologų bendroje Kauno Klinikų Anesteziologijos klinikos gydytojų imtyje sutiko teikti pagalbą savo artimiems žmonėms. 4. Nustatyti, ar turi įtakos sveikatos specialistų klinikinės praktikos trukmė ir sutikimas padėti artimam asmeniui. 5. Nustatyti, kokią dažniausią medicininę pagalbą sveikatos specialistai yra suteikę savo artimiesiems. 6. Įvertinti dėl kokių priežasčių sveikatos specialistai yra linkę atsisakyti medicininės pagalbos teikimo. 7. Išsiaiškinti, ar sveikatos specialistai žino, jog yra rekomendacijos, galinčios padėti priimti sprendimus artimųjų gydyme.
Tyrimo metodai. Vykdyta anoniminė internetinė anketinė apklausa. Gauta bendra imtis – 128 respondentai. Surinkti sociodemografiniai duomenys, pateikti klausimai skirti išsiaiškinti, ar į sveikatos specialistus kreipiasi jų artimieji ir ar jie sutinka padėti iškilus sveikatos problemoms bei įvertinti medicininės pagalbos suteikimo pobūdį ir sveikatos specialistų požiūrį. Duomenų statistinė analizė atlikta naudojantis Microsoft Office Excel 2010 bei SPSS Statistics Subscribtion programomis. Statistinių hipotezių vertinimui naudotas reikšmingumo lygmuo p<0,05.
Tyrimo rezultatai. Tyrime dalyvavo 128 LSMUL Kauno klinikų sveikatos specialistai. Į anketoje pateiktus klausimus atsakė 51,6 proc. gydytojų, 16,4 proc. gydytojų-rezidentų ir 32 proc. slaugytojų. 95 proc. visų apklaustųjų gavo medicininės pagalbos prašymą iš savo artimųjų ir 99 proc. jų sutiko. 63,2 proc. iš visų apklaustų gydytojų buvo anesteziologai-reanimatologai. 46,2 proc. viso Anesteziologijos klinikos personalo sutiko padėti savo artimiesiems. 67 proc. visų apkaustų gydytojų išrašė receptus vaistams, tačiau tik 6 proc. buvo kaip pagrindiniai gydytojai. 69,5 proc. apklaustųjų sveikatos specialistų medicininę pagalbą teikė telefonu. Resondentų nuomone, atsisakyti paskatintų kompetencijos nebuvimas, negalėjimas būti objektyviu dėl per artimų santykių, jei tai būtų neetiška ar turėtų teisinių apribojimų. 73 proc. sveikatos specialistų nežino ar nėra įsitikinę, jog egzistuoja gairės, kurios gali padėti apsispręsti artimųjų gydymo klausimu. 44 proc. apklaustų sveikatos specialistų norėtų turėti etinį vadovą ar rekomendacijas.
Išvados. 1. Išsiaiškinta, jog pagal Lietuvos gydytojų profesinės etikos kodeksą gydytojui rekomenduojama vengti diagnozuoti ir gydyti ligas ar išrašyti vaistus savo šeimos nariams bei artimiesiems; 2. Lietuvoje pagal teisines normas įstatymo, draudžiančio sveikatos specialistams gydyti šeimos narius nėra; 3. Visi tyrime dalyvavę Anesteziologijos klinikos gydytojai ir gydytojai-rezidentai (46,2 proc. viso personalo) sutiko teikti medicininę pagalbą savo artimiesiems; 4. Nustatyta, jog stipraus ryšio tarp sveikatos specialistų klinikinės praktikos trukmės ir sprendimo teikti medicininę pagalbą nebuvo; 5. Dažniausia teikiama pagalba buvo receptų išrašymas (67 proc. visų apkaustų), tačiau tik 6 proc. buvo kaip pagrindiniai gydytojai; 6. 35,9 proc. respondentų pagrindine atsisakymo teikti priežastimi nurodė kompetencijos stygių, kiti 9,4 proc. negalėjimą būti objektyviais dėl per artimų santykių. 7. Nustatyta, jog 73 proc. nežino ar nėra įsitikinę, jog egzistuoja gairės, kurios gali padėti apsispręsti artimųjų gydymo klausimu. 44 proc. nurodė, jog tokios rekomendacijos gydymo įstaigose būtų reikalingos;

The aim of the research. To prepare recommendations for good medical practice in providing medical aid to health care specialist‘s family members and relatives, taking into account ethical and legal provisions. Objectives. 1. To determine whether the aid provided by a health professional to relatives does not violate the principles of ethics and morality. 2. To evaluate whether there are legal restrictions on the provision of medical care to family members and relatives. 3. To find out the incidence of health care provision for relatives of anesthesiologists-reanimatologists, Department of Anaesthesiology, LUHS Hospital Kaunas Clinics. 4.To determine whether acceptance to help for close family member or relative is affected by the duration of health professional‘s clinical practice. 5. To determine the most common medical aid provided by health care professionals to their family members and relatives. 6. To evaluate the reasons why health care professionals tend to refuse to provide medical care. 7. To find out if health care professionals are aware of recommendations that can help making decisions about the treatment of their relatives. Research methodology. The anonymous survey was conducted for 2.5 months. The study involved randomly selected physicians, resident physicians and nurses of different age and specialties. Socio-demographic data were collected. Questions were asked to find out the ratio of health care specialists who experienced their relatives‘ seek for help and their agreement to provide help. In addition, questions to evaluate the type of medical care and the health care specialist‘s attitude were asked. Statistical analysis of the data was performed using Microsoft Office Excel 2010 and SPSS Statistics Subscribtion programs, regarding p <0.05 as statistically significant. Research results. 128 health specialists participated in the study. The questionnaire was answered by 51.6% of physicians, 16.4% of resident physicians and 32% of nurses. 95 % of all respondents received a request for medical assistance from their relatives and 99 % of them agreed to help. 63.2% of all surveyed physicians were anesthesiologists-reanimatologists. The most frequent relatives seeking medical help were parents . 67% of respondents were inclined to prescribe medications, and 6% provided direct treatment. 69.5% of all respondents provided medical care by phone calls. The lack of competence, inability to be objective due to too close relationship, unethical acts or legal restrictions were named as principal reasons for refusal. 73% of health professionals do not know or are not sure that there are guidelines and 44% of them would request an ethical guide or recommendations for medical care of health care professionals. Conclusions. 1. According to the Lithuanian Code of Professional Ethics for Doctors, a doctor is recommended to avoid diagnosing and treating diseases or prescribing medicines to his family members and relatives; 2. According to Lithuanian legal norms, there is no law prohibiting health care professionals from treating family members. 3. All physicians and resident physicians of the Clinic of Anesthesiology who participated in the study (46.2% of the total staff) agreed to provide medical care to their relatives. 4. No strong correlation between the duration of clinical practice of health care professionals and the decision to provide medical care was found. 5. The most common help provided was prescription of medication (67% of cases), but direct medical treatment was provided in 6% of cases. 6. Reasons for refusal to provide health care for the relatives were: lack of competence in 35.9 % and inability to be objective due to too close relationship in 9.4% of cases, respectively. 7. 73% of health professionals were unaware about existance of guidelines regarding the treatment of relatives and 44% expressed a desire for the need of recommendations for provision of health care for the relatives.
