Darbo autorius: Ainas Beinakaraitis Darbo vadovė: prof. Giedrė Jurkevičienė Darbo tikslas: Nustatyti vaistų nuo epilepsijos (VNE) koncentracijos sąsajas su VNE vartojimo ypatumais Darbo uždaviniai:
- Nustatyti VNE koncentracijos kraujyje pasiskirstymą VNE terapinės, mažesnės bei didesnės nei terapinė koncentracijų grupėse.
- Nustatyti VNE koncentracijos kraujyje svyravimus.
- Nustatyti VNE koncentracijos kraujyje sąsajas su kūno masės indeksu.
- Nustatyti VNE koncentracijos kraujyje sąsajas su gydymu VNE (vartojant vieną VNE ar VNE derinį; su ar be kepenų fermentų induktoriaus (CBZ)/inhibitoriaus (VPA), VNE paros dozės).
- Nustatyti VNE koncentracijos kraujyje sąsajas su laiko tarp paskutinės VNE dozės išgėrimo ir VNE koncentracijos tyrimo atlikimo trukme. Metodai: Atlikta retrospektyvinė 86 epilepsiją sergančių tiriamųjų demografinių (lytis, amžius), antropometrinių (ūgis, kūno masė, kūno masės indeksas) rodiklių, vartojamo VNE, VNE kiekio (monoterapija, politerapija), VNE dozės, laiko nuo paskutinės VNE dozės išgėrimo ir VNE koncentracijos tyrimo atlikimo trukmės ir VNE koncentracijos kraujyje duomenų analizė. Analizuoti tiriamieji vartojantys lamotriginą (LTG), levetiracetamą (LEV), valproatą (VPA), karbamazepiną (CBZ), topiramatą (TPM) ir okskarbazepiną (OXC). Tiriamųjų grupei nustatyta n=180 koncentracijų ėminių. Duomenims palyginti naudota chi-kvadrato, Mann-Whitney U ir Kruskal-Wallis testai bei Spearmano tiesinė koreliacija. Statistiškai patikimais laikyti rezultatai, kai p < 0,05. Rezultatai: Tiriamųjų grupėje (n = 86) buvo 34 (39,5 %) vyrai ir 52 (60,5 %) moterys. Statistiškai reikšmingai nesiskyrė monoterapijos ir politerapijos tiriamųjų grupės pagal lytį, amžių, ūgį, kūno masę ir KMI (p > 0,05). Nuo ~2/3 (64,4 %) iki 100% tirtų VNE koncentracijų ėminių (n=180) pateko į terapines koncentracijų ribas skirtingose VNE grupėse. Mažesnė nei terapinė VNE koncentracija kraujyje buvo nustatyta nuo 0 iki 1/3 (33,3 %), didesnė nei terapinė nuo 0 iki ~1/10 (10,8 %) tirtų VNE koncentracijų ėminių grupėje vartojant skirtingus VNE. VNE koncentracijų ėminių (n=180) pasiskirstymas VNE koncentracijų grupėse statistiškai reikšmingai nesiskyrė vartojant konkrečias VNE paros dozes (p>0,05). Reikšmingai svyravo VNE koncentracijos kraujyje 53,9-75% tiriamųjų skirtingose VNE grupėse. Tiriamųjų VNE koncentracijų kraujyje svyravimai statistiškai reikšmingai nesiskyrė vartojant skirtingus VNE (p>0,05). Palyginus VNE koncentracijas kraujyje skirtingo KMI tiriamųjų grupėms nebuvo statistiškai reikšmingo skirtumo tarp vartojusių skirtingus VNE, tiek vartojusių VNE konkrečias paros dozes (p>0,05). Tirtų koncentracijų ėminių grupėje, vartojant vieną VNE, (n=73) nustatytas statistiškai reikšmingas ryšys tarp LTG koncentracijos kraujyje ir paros dozės (p<0,05). Kitų tirtų VNE koncentracijų ėminių grupėse ryšys tarp VNE koncentracijos kraujyje ir paros dozės nebuvo statistiškai reikšmingas (p>0,05). LTG koncentracijos kraujyje skyrėsi tirtų VNE koncentracijų ėminių grupėse: vartojant vieną VNE lyginant ir vartojant VNE derinį (U = 838,5, p < 0,05), vartojant VNE derinyje su ar be CBZ (U = 401, p < 0,05) ir vartojant VNE derinyje su ar be VPA (U = 423, p < 0,05). Beveik pusei (47,2%) tiriamųjų VNE koncentracijų kraujyje tyrimas atliekamas tarp 11 ir 13 valandų nuo paskutinės VNE dozės, o 16,7% – anksčiau nei 11 valandų ir 36,1% – vėliau nei 13 valandų. Nenustatytas statistiškai reikšmingas ryšys tarp VNE koncentracijos kraujyje ir trukmės nuo paskutinės VNE dozės išgėrimo iki VNE koncentracijos tyrimo atlikimo bendroje (n=180), vartojant vieną VNE ir vartojant VNE derinį VNE koncentracijų ėminių grupėse (p>0,05). Tirtų VNE koncentracijų ėminių grupėje (n=180) statistiškai reikšmingai skyrėsi LEV koncentracijos tirtos <11 valandų ir ≥11–≤13 valandų nuo paskutinės VNE dozės (Kruskal-Wallis H=10,532, p<0,05). Tirtų VPA koncentracijų ėminių grupėje vartojant 1000mg paros dozę statistiškai reikšmingai skyrėsi VNE koncentracijos tirtos anksčiau nei 11 valandų ir ≥11–≤13 valandų nuo paskutinės VNE dozės (Kruskal-Wallis H = 7,229, p<0,05). Išvados:
- Skirtingų VNE koncentracija kraujyje terapinėse koncentracijų ribose buvo nuo ~2/3 (64,4%) iki 100% tirtų VNE koncentracijų ėminių. Atitinkamai, mažesnė nei terapinė buvo nustatyta nuo 0 iki 1/3 (33,3%), didesnė nei terapinė - nuo 0 iki ~1/10 (10,8%) tirtų skirtingų VNE koncentracijų ėminių. VNE koncentracijų ėminių pasiskirstymas VNE koncentracijų grupėse statistiškai reikšmingai nesiskyrė vartojant konkrečias VNE paros dozes.
- VNE koncentracijos kraujyje reikšmingai svyravo nuo maždaug 1/2 (53,9%) iki 3/4 (75%) tiriamųjų, kuriems buvo atlikti bent du VNE koncentracijos kraujyje matavimai. Skirtingų VNE koncentracijų svyravimai statistiškai reikšmingai nesiskyrė.
- Ryšio tarp VNE koncentracijos kraujyje ir KMI tiriamųjų (tiek bendroje, tiek monoterapijos, tiek politerapijos) grupėse, nustatyta nebuvo. VNE koncentracija kraujyje skirtingose tiriamųjų KMI grupėse vartojant konkrečias VNE paros dozes statistiškai reikšmingai nesiskyrė.
- Nustatytas ryšys tarp LTG koncentracijos kraujyje ir LTG paros dozės (vartojant LTG monoterapijoje) (p<0,05). LTG koncentracija kraujyje buvo mažesnė:
- vartojant LTG monoterapijoje, nei LTG koncentracija, kai LTG buvo vartojamas derinyje su kitu (-ais) VNE;
- kai LTG vartojant derinyje su kitais VNE, vienas iš vartojamų VNE buvo kepenų fermentų (KF) induktorius (CBZ), lyginat, kai VNE derinyje KF induktoriaus nebuvo;
- kai LTG vartojant derinyje su kitais VNE, šiame derinyje KF inhibitoriaus (VPA) nebuvo, lyginant, kai VNE derinyje KF inhibitorius buvo. Atitinkamai, analogiškų sąsajų tarp kitų VNE koncentracijos kraujyje ir atitinkamo VNE paros dozės, nustatyta nebuvo.
- Beveik pusė (47,2%) VNE koncentracijų ėminių buvo tirta tinkamu laiku, t.y. praėjus ≥11–≤13 valandų nuo paskutinės VNE dozės išgėrimo. Maždaug 1/3 (36,1%) – vėliau nei 13 valandų ir 16,7% - anksčiau nei 11 valandų. Statistiškai reikšmingo ryšio tarp VNE koncentracijos kraujyje ir laiko nuo paskutinės VNE dozės išgėrimo iki VNE koncentracijos tyrimo atlikimo trukmės, nustatyta nebuvo.

Author: Ainas Beinakaraitis Supervisor: prof. Giedrė Jurkevičienė Aim: To determine associations between antiepileptic drug (AED) concentrations and AED consumption characteristics. Objectives:
- To determine the distribution of AED blood concentration across therapeutic, subtherapeutic, and supratherapeutic concentration groups.
- To determine the fluctuations in AED concentration levels in the blood.
- To determine associations between AED blood concentration and body mass index (BMI).
- To determine associations between AED blood concentration and AED treatment (monotherapy or combination therapy; with or without a liver enzyme inducer (CBZ) or inhibitor (VPA), and daily AED dose).
- To determine associations between AED blood concentration and the time interval between the last AED dose intake and the concentration measurement. Methods: A retrospective analysis was conducted on data from 86 subjects with epilepsy, including demographic (sex, age), anthropometric (height, body weight, body mass index) indicators, the type of AED used, AED regimen (monotherapy or polytherapy), AED dose, the time interval between the last AED dose intake and the blood sampling for AED concentration measurement, and the AED blood levels. The analysis included subjects using lamotrigine (LTG), levetiracetam (LEV), valproate (VPA), carbamazepine (CBZ), topiramate (TPM), and oxcarbazepine (OXC). A total of n=180 blood concentration samples were analyzed in the study group. For data comparison, the chi-square test, Mann-Whitney U test, and Kruskal-Wallis test, as well as Spearman's rank correlation, were used. Results were considered statistically significant when p < 0.05. Results: In the study group (n = 86), there were 34 men (39.5%) and 52 women (60.5%). There were no statistically significant differences between the monotherapy and polytherapy groups in terms of sex, age, height, body weight, and BMI (p > 0.05). Between approximately two-thirds (64.4%) and 100% of the analyzed AED concentration samples (n = 180) fell within the therapeutic range across the different AED groups. Subtherapeutic AED concentrations were observed in 0 to one-third (33.3%) of samples, while supratherapeutic levels were found in 0 to about one-tenth (10.8%) of the samples, depending on the AED used. The distribution of AED concentration samples (n = 180) across concentration ranges did not differ significantly according to specific daily AED doses (p > 0.05). AED concentration levels in the blood showed significant fluctuations in 53.9–75% of subjects across different AED groups. These fluctuations were not statistically significantly different between users of different AEDs (p > 0.05). When comparing AED concentrations across subject groups with different BMIs, there were no statistically significant differences, regardless of the AED used or the specific daily dose (p > 0.05). In the subgroup using a single AED (n = 73), a statistically significant correlation was found between LTG blood concentration and daily dose (p < 0.05). No statistically significant correlations were found between blood concentrations and daily doses for the other AEDs studied (p > 0.05). LTG concentrations differed significantly in the groups based on monotherapy vs. combination therapy (U = 838.5, p < 0.05), combination therapy with or without CBZ (U = 401, p < 0.05), and combination therapy with or without VPA (U = 423, p < 0.05). Nearly half (47.2%) of the AED blood concentration measurements were taken 11–13 hours after the last AED dose, while 16.7% were taken earlier than 11 hours, and 36.1% later than 13 hours. There was no statistically significant association between AED blood concentration and the time interval since the last dose in the overall sample (n = 180), as well as in monotherapy and combination therapy subgroups (p > 0.05). In the LEV group (n = 180), concentrations measured before 11 hours and between 11–13 hours after the last dose differed significantly (Kruskal-Wallis H = 10.532, p < 0.05). In the VPA group receiving a 1000 mg daily dose, concentrations measured before 11 hours and between 11–13 hours after the last dose also differed significantly (Kruskal-Wallis H = 7.229, p < 0.05). Išvados:
- The blood concentrations of different AEDs fell within the therapeutic range in approximately two-thirds (64.4%) to 100% of the analyzed AED concentration samples. Accordingly, subtherapeutic concentrations were found in 0 to one-third (33.3%) of samples, and supratherapeutic concentrations in 0 to about one-tenth (10.8%) of the samples for different AEDs. The distribution of AED concentration samples across the concentration groups did not differ statistically significantly based on specific daily AED doses.
- AED blood concentrations fluctuated significantly in approximately half (53.9%) to three-quarters (75%) of the subjects who had at least two blood concentration measurements. The fluctuations in concentrations of different AEDs did not differ statistically significantly.
- No association was found between AED blood concentration and BMI in any of the subject groups (overall, monotherapy, or polytherapy). AED blood concentrations did not differ statistically significantly across different BMI groups when specific daily AED doses were used.
- An association was found between LTG blood concentration and the daily LTG dose in cases of LTG monotherapy (p < 0.05). LTG blood concentration was lower in the following situations:
- when LTG was used as monotherapy compared to when it was used in combination with other AED(s);
- when LTG was used in combination with other AEDs and one of them was a liver enzyme inducer (CBZ), compared to combinations without an inducer;
- when LTG was used in combination with other AEDs and no liver enzyme inhibitor (VPA) was included, compared to combinations where an inhibitor was present. No similar associations were found between the blood concentrations and daily doses of the other AEDs studied.
- Nearly half (47.2%) of the AED concentration samples were tested at an appropriate time, i.e., 11 to 13 hours after the last AED dose. Approximately one-third (36.1%) were tested more than 13 hours after the last dose, and 16.7% were tested earlier than 11 hours. No statistically significant association was found between AED blood concentration and the time interval from the last AED dose to the concentration measurement.
