Klinikinis atvejis: paciento, sergančio šizofrenija ir turinčio transplantuotą inkstą, gydymo ypatumai

Abstract

Aprašomas paranoidinės šizofrenijos atvejis vyrui, sergančiam lėtiniu inkstų funkcijos nepakankamumu, esant transplantuotam inkstui. Pacientui paranoidinė šizofrenija diagnozuota 18 metų, skirtas gydymas įvairiais antipsichotiniais vaistais. Vėliau susirgimas vertintas kaip rezistentiškas gydymui, skirta klozapino. Laikui bėgant, 27 metų pacientui nustatytas aktyvus židininis pusmėnulinis IgA glomerulonefritas, įvertintas kaip gretutinė liga, nepriklausoma nuo psichotropinių vaistų vartojimo. Ligai progresuojant, išsivystė lėtinis inkstų funkcijos nepakankamumas. Po 4 metų atlikta inksto tranplantacija, taikytas imunosupresinis gydymas. Esant sunkiai psichikos ligai, pacientui buvo indikuotini ir antipsichotiniai vaistai. Gydant somatinė ir psichikos būklė stabilizavosi. Vėliau pacientas reguliariai lankėsi pas gydytojus nefrologus, vartojo vaistus, skirtus po inksto transplantacijos bei psichikos ligai gydyti. We present a case of paranoid schizophrenia of a man, suffering from kidney failure, who also has a kidney transplant. The patient was diagnosed with paranoid schizophrenia at the age of 18 and was treated with several antipsychotics. Later the disease was considered to be treatment resistant and Clozapine was prescribed. Later in life, at the age of 27, the patient was diagnosed with active focal cresent IgA glomerulonephritis, which was confirmed to be an independent disease and not related to the use of psichotropic medication. The disease progressed until it reached kidney failure. After 4 years the patient received a kidney transplant, along with immunosuppressive therapy. Because the patient‘s psychiatric illness was severe, antipsychotic medication was indicated. During treatment, the patient‘s mental and somatic status stabilised. Afterwards, the patient continued to visit nephrologists regularly, adhered to his medication recommended after transplantation and for the treatment of his psychiatric illness.
