Introduction Definite management of a patient‘s medication plan is an essential element of patient care. The prevention of medication related errors (MRE) is a significant public health concern, as MRE can cause avoidable harm for health, including avoidable hospitalizations, drug interactions, adverse drug reactions. Acquiring medication history from patients upon hospitalization is a process prone to errors. Errors in medication history may lead to potential or clinically significant drug-related problems or inappropriate pharmacotherapy. Scientific data confirms that medication errors during hospital admissions are prevalent and some of them have the potential to cause clinically significant harm for the patients. One of the most frequently noted errors in medication histories is the medication omission. Medication omission refers to the situation where medication taken by the patient is not documented in their medical records.
