Lietuvos gydytojų ir pacientų žinios apie arterinę hipertenziją
Tikslas – įvertinti gydytojų ir pacientų žinias apie arterinę hipertenziją (AH) bei nustatyti veiksnius, darančius įtaką AH gydymo kokybei ir vaistų vartojamumui. Tyrimo medžiaga ir metodai. Moksliniai projektai: „Visuomenės žinios apie AH“ (2009 m. lapkričio–2010 m. vasario mėn., dalyvavo 534 pacientai), „AH gydymo ypatybės Lietuvoje“ (2011 m., dalyvavo 235 gydytojai ir 764 jų pacientai, sergantys AH). Vienmomenčiai analitiniai anketiniai tyrimai. Vertinant žinias apie AH apskaičiuotas suminio balo vidurkis (SBV). Vaistų vartojamumo ir gydymo rekomendacijų laikymasis vertintas remiantis Hill-Bone skale. Rezultatai. Vidurinį išsilavinimą įgijusių respondentų (n = 192) SBV buvo 6,69 ± 2,14, aukštesnįjį (n = 160) – 7,21 ± 2,14, aukštąjį (n = 180) – 7,94 ± 2,04 (p = 0,034). Moterų SBV buvo didesnis nei vyrų (7,51 ± 2,19 vs 6,91 ± 2,08; p = 0,002). AH sergančių > 10 metų asmenų (n = 119) SBV didesnis nei sergančiųjų < 10 metų (n = 193) (7,77 ± 1,76 vs 7,13 ± 2,03; p = 0,005). Tyrimo dalyvių, kurie vienu iš informacijos apie AH šaltinių pasirinko gydytoją (n = 278), SBV buvo didesnis, lyginant su tais pacientais, kurie gydytojo, kaip informacijos apie AH šaltinio, nenurodė (7,47 ± 1,94 vs 7,10 ± 2,38; p = 0,048). Nekontroliuojamos AH priežastimi nenuoseklų vaistų vartojimą nurodė 71,1 proc. gydytojų (n = 167) ir 40,5 proc. pacientų (n = 309), antsvorį ir nutukimą – atitinkamai 51,5 proc. (n = 121) ir 54,9 proc. (n = 419), prastą nemedikamentinio gydymo rekomendacijų laikymąsi – 68,5 proc. (n = 161) ir 37,6 proc. (n = 287). 336 pacientai (44,8 proc.) teigė, kad kyla sunkumų vartojant didelį paskirtų tablečių skaičių. AH trukmė koreliavo su gydymo rekomendacijų laikymusi (Pirsono (Pearson) koreliacijos koeficientas – 0,09, p = 0,014). [...].
Summary Objective. To evaluate patients and physicians knowledge about arterial hypertension (AH) and to determine factors that influence treatment adherence. Materials and methods. In this cross-sectional survey participants were enrolled from two separate studies that included 534 patients with arterial hypertension (project “Knowledge about arterial hypertension in society”, 2009 November – 2010 February) and 235 doctors and 764 patients (project “Arterial hypertension treatment features in Lithuania“), respectively. Knowledge on arterial hypertension was quantified using mean cumulative score (MCS). Treatment adherence and compliance with treatment recommendations was evaluated by means of Hill-Bone scale. Results. Patients with secondary educational level (n=192) achieved MCS of 6.69±2.14 (means ± SD), compared with 7.21±2.14 and 8.94±2.04 in patients with college education (n=160) or university degree (n=180), respectively (p=0.038). MCS was significantly higher in women vs. men (7.51 ±2.19 vs. 6.91±2.08; p=0.002). Higher MCS was reported in patients with long-standing AH (n=119), compared to respondents with less than 10 year history of AH (n=193) (7.77±1.76 vs. 7.13±2.03, p=0.005). MCS was higher if doctors were chosen as one of the primary information source (n=278) compared to individuals who did not list doctor as primary information source (7.47±1.94 vs. 7.10±2.38; p=0.048). 71.1 % of doctors (n=167) and 40.5 % of patients (309) agreed that poor arterial hypertension control was caused by nonadherence to medication use.
