Abstract
Baigiamojo magistro darbo autorius: Goda Gudaitytė Baigiamojo magistro darbo vadovė: prof. dr. Erika Skrodenienė Baigiamojo magistro darbo tema: Spermos kokybės ir vyrų amžiaus sąsajos Tiriamojo darbo tikslas: Įvertinti spermos kokybės ir vyrų amžiaus sąsajas. Darbo uždaviniai:
- Įvertinti vyrų amžiaus ir spermos tūrio sąsajas.
- Įvertinti spermatozoidų koncentracijos ir judrumo pokyčius skirtingose vyrų amžiaus grupėse.
- Įvertinti spermos rodiklių (spermos tūrio, spermatozoidų koncentracijos, bendro spermatozoidų judrumo, progresyviai ir vietoje judančių spermatozoidų, nejudančių spermatozoidų, leukocitų ir apvalių ląstelių kieko, spermos pH) ryšį su vyrų amžiumi. Tyrimo objektas ir metodai: Tyrime retrogradiškai nagrinėti vyrų spermos tyrimų, atliktų LSMU ligoninės Kauno klinikų, Laboratorinės medicinos klinikos, Hematologijos ir bendrosios citologijos laboratorijoje, rezultatų duomenys. Į tyrimą buvo įtraukti 349 vyrų, tirtų nuo 2017 iki 2020 metų, spermos tyrimų rezultatai. Jauniausio tyrime dalyvavusio vyro amžius – 21 metai, o vyriausio – 54 metai. Analizuojant duomenis, tiriamieji buvo suskirstyti į tris amžiaus grupes: vyrai iki 30 metų amžiaus, vyrai nuo 30 iki 40 metų ir vyrai virš 40 metų amžiaus. Jauniausioje tyrimo dalyvių grupėje iki 30 metų buvo 71 pacientas (amžiaus Md (Q1; Q3) – 28 (27; 28) metai), 30–40 metų amžiaus grupę sudarė 218 pacientų (amžiaus Md (Q1; Q3) – 33 (31; 35) metai), o vyriausiajai amžiaus grupei virš 40 metų priskirta 60 pacientų (amžiaus Md (Q1; Q3) – 44 (42; 46,25) metai. Tyrimo duomenų statistinei analizei atlikti buvo naudojama IBM SPSS programos 24.0 versija. Statistiniam patikimumui nustatyti buvo pasirinktas reikšmingumo lygmuo p<0,05. Tyrimo rezultatai: Tyrime skirtingo amžiaus grupių vyrų spermos kiekio Md (Q1; Q3) buvo 3 (2; 4) ml, tarp grupių statistiškai reikšmingo skirtumo nebuvo (p=0,838). Tyrime dalyvavusių vyrų spermatozoidų koncentracijos mediana tarp skirtingo amžiaus grupių vyrų statistiškai reikšmingai nesiskyrė (p=0,063). Nustatyta, kad bendras spermatozoidų judrumas didžiausias buvo jauniausių vyrų spermoje, Md (Q1; Q3) – 67 (59,4; 66,6) proc., tuo tarpu 30–40 bei > 40 metų amžiaus grupių vyrų spermoje judrių spermatozoidų kiekis buvo mažesnis (Md (Q1; Q3) atitinkamai – 60,5 (53,4; 59,1) ir 56 (48,6; 58,4) proc.) (p=0,009). Vertinant progresyviai judančių spermatozoidų kiekį nustatyta, kad jauniausių vyrų spermoje progresyviai judančių spermatozoidų kiekis buvo didesnis, Md (Q1; Q3) – 33 (31,4; 39,7) proc., nei vidutinio ir vyriausio amžiaus tiriamųjų spermoje Md (Q1; Q3) atitinkamai – 27 (27,7; 32,7) ir 21,5 (23; 32,8) proc. (p=0,023). Atlikus mūsų tyrimo duomenų analizę buvo nustatyta, kad skirtingo amžiaus vyrų spermoje vietoje judančių spermatozoidų kiekis tarp amžiaus grupių statistiškai reikšmingai nesiskyrė (p=0,924). Tyrimo metu nustatyta, kad nejudrių spermatozoidų kiekis didėjo didėjant pacientų amžiui. < 30, 30–40 bei > 40 metų amžiaus grupėms priskirtų tiriamųjų spermoje nejudrių spermatozoidų kiekio Md (Q1; Q3) atitinkamai buvo 33 (33,4; 40,6) proc., 37 (35,4; 40,7) proc. bei 44 (41,6; 51,4) proc. (p=0,004). Tyrimo rezultatai parodė, kad skirtingo amžiaus grupių vyrų spermoje esančių leukocitų kiekis statistiškai reikšmingai nesiskyrė (p=0,666). Vertinant vyrų spermoje aptinkamų apvaliųjų ląstelių kiekį, skirtumo tarp amžiaus grupių taip pat neustatyta (p=0,096). Tyrime skirtingo amžiaus grupių vyrų spermos pH Md (Q1; Q3) – 8,5 (8; 8,5) ir tarp amžiaus grupių statistiškai reikšmingo skirtumo nebuvo (p=0,146). Buvo nustatytas statistiškai reikšmingas atvirkštinis ryšys tarp vyrų amžiaus ir bendro spermatozoidų judrumo (r=-0,125; p=0,020) bei progresyviai judančių spermatozoidų kiekio (r=-0,106; p=0,049). Taip pat nustatytas tiesioginis statistiškai reikšmingas ryšys tarp vyrų amžiaus ir nejudančių spermatozoidų kiekio (r=0,107; p=0,045). Išvados:
- Sąsajų tarp vyrų amžiaus ir išskiriamo spermos tūrio nebuvo nenustatyta.
- Vyrų amžius neturėjo įtakos spermatozoidų koncentracijos pokyčiams. Bendras spermatozoidų judrumas bei progresyviai judančių spermatozoidų kiekis buvo didesnis jaunesnių nei 30 metų vyrų amžiaus grupėje negu vyresnių nei 40 metų. Nejudančių spermatozoidų didžiausias kiekis buvo vyresnių nei 40 metų vyrų amžiaus grupėje. Vietoje judančių spermatozoidų kiekis nuo amžiaus nepriklausė.
- Didėjant vyrų amžiui bendras spermatozoidų judrumas ir progresyviai judančių spermatozoidų kiekis mažėja, o nejudančių spermatozoidų kiekis didėja.

Title of the final thesis: The Relationship Between the Sperm Quality and the Male Age
Aim of the study: To evaluate the effects of the quality of sperm and the males age.
Tasks of the study:
- To evaluate the interfaces of male age and volume of semen.
- To evaluate sperm concentration and motility changes in different age groups of men.
- To evaluate the relationship between semen parameters (smen volume, sperm concentration, sperm motility, progressive motility, non–progressive motility, immotile sperm, the number of leukocytes and the round cells, semen pH) and the age of males. Object and methods of the study: During the investigation, the data of male sperm examination were analyzed retrogradely, which were carried out in Kauno klinikos of the Lithuanian University of Health Sciences hospital, the Department of Laboratory Medicine, the Laboratory of Hematology and General Cytology. The data of 349 male sperm results from 2017 to 2020 were included. The youngest patient was 21 years old, while the oldest was 54. As the data was analyzed, the participants were divided into three groups: males under 30 years old, males from 30 to 40 years old, and males over 40 years old. In the youngest group, 71 patients (age Md (Q1; Q3) – 28 (27; 28) years), 218 patients were in the group of 30–40 years old (age Md (Q1; Q3) – 33 (31; 35) years), meanwhile, in the group of the oldest patients (over 40 years old), 60 patients were assigned (age Md (Q1; Q3) – 44 (42; 46.25) years). The study data were analyzed using the IBM SPSS program (Statistical Package for Social Science) version 24.0. Differences between groups were considered statistically significant when the p value was less than 0.05. Research findings: The Md (Q1; Q3) of semen volume in different male age groups was 3 ml (2; 4). The statistically significant differences were not found (p=0.838). The median sperm concentration in the study men did not differ statistically significantly between men in different age groups (p=0,063). We determined that the total sperm motility in the youngest group was the highest, Md (Q1; Q3) – 67 (59.4; 66.6) percent, meanwhile, in the 30–40 years old and over 40 years age group, the sperm motility was lower, their Md (Q1; Q3) – 60.5 (53.4; 59.10) and 56 (48.6; 58.4) percent respectively (p=0.009). We evaluated that progressive motility sperm in the youngest male group was higher, Md (Q1; Q3) – 33 (31.4; 39.7) percent, than in average and older age groups participants, Md (Q1; Q3) – 27 (27.7; 32.7) and 21.5 (23; 32.8) percent respectively (p=0.23). After studying our data analysis, we estimated that the quantity of non–progressive motility sperm did not differ statistically significantly (p=0.924) among different age groups. The investigation identified that the number of immotile spermatozoa increased consistently with the growing age. In the groups of assigned participants who were under 30, 30–40, over 40 year old groups, Md (Q1; Q3) of immotile spermatozoa quantity were 33 (33.4; 40.6) percent, 37 (35.4; 40.7) percent and 44 (41.6; 51.4) percent (p=0.004). Study results showed that the number of leukocytes in the semen of men of different age groups did not differ statistically significantly (p=0.666). The number of round cells in male sperm was not different in age groups (p=0.096). The Md (Q1; Q3) of pH was 8.5 (8; 8.5), with no statistically significant differences between age groups (p=0.146). Estimated statistically significant, reverse connection between the age of males and the total motility of sperm (r=-0.125; p=0.020) and the quantity of progressive motility sperm (r=-0.106; p=0.049). Also determined a positive statistically significant, connection between age of males and the quantity of immotile sperm (r=0.107; p=0.045). Conclusions:
- The link between the male age and the semen volume was not determined.
- The age did not influence the changes in sperm concentration. The total motility of sperm and number of progressive motility sperm was higher in the group under 30 years old than in the over 40 years old group. The highest number of immotile spermatozoa was estimated in the group over 40 years old. Age did not influence the number of non-progressive motility sperm.
- When males‘get older, the total motility and the number of progressive motility sperm decrease, whereas immotile sperm increase.
