Long-Term Results of Immediate vs Delayed Implant Loading for Full-arch Rehabilitation: Systematic Review

Abstract

Purpose: To compare the long-term outcomes of immediate (IL) and delayed (CL) implant loading for full-arch rehabilitation in both the maxilla and mandible. Thereby verifying if a shorter loading protocol could be utilized to reduce the time of decreased masticatory performance, without jeopardizing treatment outcomes.

Materials and methods: An electronic search was performed using PubMed and Cochrane Library. The search results were limited to clinical trials published between the 1st of January 2014 to the 1st of January 2024. Only randomized controlled trials were included for full-text reading.

Results: Of the 155 articles identified using the Boolean search string and the search filters, nine of them were considered eligible at the conclusion of the screening process. Only one study, using a single implant-retained overdenture, found a statistically significant higher survival rate in the CL group, while the remaining studies showed no significant difference. No clear superior tendencies were found in either of the groups in terms of peri-implant tissue parameters, implant stability, or patient satisfaction.

Conclusions: The findings suggest comparable long-term clinical outcomes for IL and CL. Apart from single implant-retained overdentures, IL seems appropriate for a range of clinical cases and implant systems. The choice of the loading protocol should be based on proper patient evaluation. More studies are needed for the maxilla and single implant-retained overdentures to determine their efficacy.

Tikslas: Palyginti ilgalaikius tiesioginio (IL) ir uždelsto (CL) implantų apkrovimo rezultatus viso lanko reabilitacijai tiek viršutinėje žandikaulyje, tiek apatinėje dalyje. Taip patikrinama, ar galima naudoti trumpesnį įkėlimo protokolą, kad sutrumpėtų kramtymo efektyvumo laikas, nepakenkiant gydymo rezultatams.

Medžiagos ir metodai: Elektroninė paieška atlikta naudojant PubMed ir Cochrane Library. Paieškos rezultatai buvo apriboti klinikiniais tyrimais, paskelbtais nuo 2014 m. sausio 1 d. iki 2024 m. sausio 1 d. Viso teksto skaitymui buvo įtraukti tik atsitiktinių imčių kontroliuojami tyrimai.

Rezultatai: Iš 155 straipsnių, nustatytų naudojant Būlio paieškos eilutę ir paieškos filtrus, devyni iš jų buvo laikomi tinkamais atrankos proceso pabaigoje. Tik viename tyrime, naudojant vieną implanto paliktą protezą, nustatytas statistiškai reikšmingas didesnis išgyvenamumas CL grupėje, o kiti tyrimai reikšmingo skirtumo neparodė. Nei vienoje grupėje neaptikta jokių ryškių pranašumų, susijusių su periimplanto audinių parametrais, implanto stabilumu ar pacientų pasitenkinimu.

Išvados: Išvados rodo panašias ilgalaikes IL ir CL klinikines baigtis. Be pavienių implantų išlaikomų protezų, IL atrodo tinkama įvairiems klinikiniams atvejams ir implantų sistemoms. Pakrovimo protokolo pasirinkimas turėtų būti pagrįstas tinkamu paciento įvertinimu. Norint nustatyti jų veiksmingumą, reikia atlikti daugiau tyrimų, kad būtų galima nustatyti viršutinio žandikaulio ir vieno implanto paliktus protezus.
