Skirtingų dantų protezų konstrukcijų paplitimas ir pacientų nuomonė apie dantų protezavimo paslaugų poreikį ir prieinamumą

Problemos aktualumas ir darbo tikslas. Norint atkurti pusiausvyrą burnoje netekus dantų, yra reikalingas dantų protezavimas. Nors kai kuriose Europos Sąjungos šalyse, tarp jų ir Lietuvoje, pilnos ar dalinės adentijos paplitimas mažėja, ypač tarp jaunų žmonių, tyrimai rodo, kad dėl senstančios visuomenės dantų protezų paplitimas ir toliau didėja. Visuomenė nuolat keičiasi ir anksčiau Lietuvoje atliktų tyrimų duomenys ne iki galo atspindi šiuo metu vyraujančias tendencijas, be to COVID-19 pandemija, prasidėjusi nuo vieno ligos atvejo 2019 metų pabaigoje įnešė neišvengiamos sumaišties medicinos paslaugų sektoriuje. Šio tyrimo tikslas yra išanalizuoti protezų konstrukcijų paplitimą, dantų protezavimo paslaugų poreikį, prieinamumą ir jo pokytį COVID-19 metu pačių pacientų nuomone.
Medžiaga ir metodai. Tyrimui sukurta anoniminė anketa buvo platinta internetiniu ir popieriniais variantais pilnamečiams Lietuvos Respublikos gyventojams 2020 m. gruodžio – 2021 m. vasario mėnesiais. Tyrime išanalizuota tinkamai gauta 207 tiriamųjų nuomonė. Duomenys buvo suvesti į „Microsoft Excel 2016“ programą, statistinė analizė atlikta IBM SPSS 25.0 programos paketu, reikšmingumo lygmuo p=0.05.
Rezultatai. Pastebėta, kad nors statistiškai reikšmingo skirtumo tarp abiejų lyčių respondentų vertinant savo dantų sveikata nebuvo, tačiau respondentai vyrai išreiškė didesnį poreikį anketos pildymo metu dantų protezavimo paslaugoms nei moterys (p<0,05). 59,9% respondentų mano, kad žino, kas yra gydytojas odontologas ortopedas, o jo paslaugomis yra naudojęsi 29% apklaustųjų. 76,3% respondentų norėtų gauti visą informaciją apie galimus gydymo būdus ir patys pasirinkti protezavimo planą, tačiau tik 70,6% procentų atžymi turintys galimybę pasirinkti vieną gydymo planą iš kelių. Tik 5,3% respondentų yra nepatenkinti arba visiškai nepatenkinti dantų protezavimo paslaugų prieinamumu. COVID-19 pandemijos metu tik 39,5% dantų protezavimo paslaugomis besinaudojusių respondentų pastebėjo prieinamumo pasikeitimus.
Išvados. Apie pusei respondentų arba reikia dantų protezavimo paslaugų, arba jie yra jomis naudojęsi. Beveik pusė respondentų neskiria bendrinės kompetencijos gydytojo odontologo nuo gydytojo odontologo ortopedo, dalis jų pastarąjį tapatina su gydytoju odontologu ortodontu ar burnos chirurgu. Daugumai respondentų ypač svarbus gydytojo malonus bendravimas, dantų protezo kaina, daugiau nei pusei – norisi turėti svaresnį indėlį renkantis gydymo planą. COVID-19 pandemijos metu, mažiau nei pusei respondentų vizito tenka laukti ilgiau, dantų protezavimo poreikis sumažėjo dėl COVID-19 apsikrėtimo rizikos ir baimės.
Relevance of the problem and aim of the work. Prosthodontics are needed to restore balance in the mouth after tooth loss. Although the prevalence of full or partial edentulism in some European Union countries, including Lithuania, is declining, especially among young people, research shows that the prevalence of various types of dentures continues to increase due to an aging population. Society is constantly changing and previous research data do not fully reflect the current trends in Lithuania. The pandemic, which started with only one case of the disease at the end of 2019 caused confusion in the sector of medical services. The aim of this research is to analyze self-reported prevalence of different prosthesis designs, the need for dental prosthetic services, its availability, and change during COVID-19.
Material and the methods. The anonymous questionnaire created for the research was distributed in online and paper versions to adult residents of the Republic of Lithuania from December of 2020 to February of 2021. During the research fully filled questionnaires of 207 subjects were analyzed. The data were compiled into Microsoft Excel 2016 program, statistical analysis was performed with the IBM SPSS 25.0 software package, significance level p = 0.05.
Results. It was observed that although there was no statistically significant difference between the respondents of both genders in assessing their dental health, at the time survey was performed male respondents expressed a greater need than female respondents for prosthetic services (p<0.05). 59.9% of respondents think they know who prosthodontists are, and 29% of respondents have used their services. 76.3% of the respondents would like to receive all the information about the possible prosthodontic treatment methods and choose it themselves, but only 70.6% state that they usually have the opportunity to do so. Only 5.3% of respondents were dissatisfied or completely dissatisfied with the availability of dental prosthetic services. During the COVID-19 pandemic, only 39.5% of respondents who used dental prosthetic services noticed changes in accessibility.
Conclusions. About half of the respondents either need dental prosthetic services or have used them. Almost half of the respondents do not distinguish between a dentist of general competence and a prosthodontist, and some of them identify the latter with an orthodontist or oral surgeon. For most respondents, the pleasant communication of the dentist and the price of the dental prosthesis is especially important, more than half of them want to have a more significant contribution when choosing a prosthodontic treatment plan. During the COVID-19 pandemic, less than half of all the respondents waited longer for a visit, the need for dental prosthetic services decreased due to the risk and fear of COVID-19 infection.
