Įgimta angioedema yra reta liga, paveldima autosominiu dominantiniu būdu. Ilgą laiką Lietuvoje nebuvo atliekami šios ligos genetiniai tyrimai. Genetinės mutacijos gali nulemti šią ligą, tačiau gali būti geno defektas be klinikinės ligos išraiškos. Molekulinės genetikos tyrimu yra nustatomos genetinės mutacijos- SERPING1, SERPING2 ar XII faktoriaus, angiopoetino 1 ir plazminogeno ir tai gali padėti išsiaiškinti sergančių įgimta angioedema pacientų ligos paveldėjimo ypatumus tarp šeimos narių. Imuniniai rodmenys vertinami pagal C1INH, C3, C4 specifinių baltymų kiekius, tai paprastas metodas leidžiantis patvirtinti įgimtos angioedemos diagnozę ir ją diferencijuoti tarp trijų skirtingų jos tipų. Tyrimo tikslas: ištirti sergančiųjų įgimta angioedema genetinius, klinikinius ir imuninius žymenimis.
Tyrimo uždaviniai: 1) Nustatyti sergančiųjų įgimta angioedema demografinius duomenis, klinikinius ligos variantus ir skirtumus tarp lyčių. 2) Nustatyti sergančiųjų įgimta angioedema imuninius žymenis ir skirtumus tarp lyčių. 3) Palyginti klinikinius ir imuninius žymenis sergant įvairiomis įgimtos angioedemos formomis. 4) Nustatyti sergančiųjų įgimta angioedema genetinius veiksnius, ligos paveldėjimo tendencijas tarp šeimos narių. Tyrimo metodika: Analizuojami 17-os pacientų ligos istorijų duomenys, kuriems buvo diagnozuota įgimta angioedema Lietuvos sveikatos mokslų universiteto Kauno klinikų Imunologijos ir Alergologijos klinikoje. Vertinami genetinių mutacijų ir imuninių rodmenų rezulatai (C1INH, C3, C4). Tyrimo rezultatai: Tyrime dalyvavo 17 pacientų, 8 moterys (47 proc.) ir 9 vyrai (53 proc.). Pirmųjų simptomų pradžios metų vidurkis moterims nustatytas- 15,38 ± 6,63, o vyrams- 13,11 ± 7,67. Laikas iki diagnozės patvirtinimo moterims buvo- 6,13 ± 3,72 metai, o vyrams- 5,44 ± 3,17 metai, tačiau reikšmingo skirtumo tarp lyties ir simptomų pradžios amžiaus bei diagnozės patvirtinimo laiko nebuvo. Nustatyta, kad dauguma pacientų 15 (88 proc.) turi šeiminę šios ligos anamnezę. Priepuolių dažnio vidurkis – 11,77 ± 9,56 k./metus. Dažniausia nustatyta klinikinė forma- odos ir poodžio edema- 4 (29 proc.), gerklų edema pasireiškė-3(2 proc.), gerklų ir odos ir poodžio edema- 3 (21 proc.), virškinamojo trakto angioedema ir odos ir poodžio edema- 2 (15 proc.), odos, gerklų ir virškinamojo trakto angioedema- 1 (7 proc.), virškinamojo trakto angioedema- 1 (7 proc.). Pastebėta tendencija, kad daugiau vyrų serga gerklų edemos forma- 4 (66,7 proc.), nei moterys- 3 (37,5 proc.). Moterų imuninių rodmenų vidutiniai kiekiai: C1INH- 0,067 ± 0,021 g/l, C3- 1,25 ± 0,658 g/l, C4 - 0,04 ± 0,017 g/l, o vyrų C1INH - 0,057 ± 0,018 g/l, C3- 0,75 ± 0,415 g/l, C4- 0,06 ± 0,033 g/l, tačiau statistiškai reikšmingo skirtumo nebuvo. Mažiausias vidutinis C1INH kiekis (0,05 ± 0,008) g/l buvo sergant odos ir poodžio edema, o didžiausias C1INH kiekis (0,12 ± 0,082) g/l sergant odos ir gerklų edema, tačiau statistiškai reikšmingai nesiskyrė. Mažiausias C3 kiekis (0,68 g/l) pasireiškus kelioms klinikinėms formomis- virškinamojo trakto angioedema, odos ir gerklų edema, o didžiausias C3 kiekis (1,15 ±0,659) g/l sergant odos ir poodžio edema, tačiau statistiškai reikšmingai nesiskyrė. Mažiausias C4 kiekis (0,04 ± 0,017) g/l sergant odos ir poodžio edema ir pasireiškus kelioms klinikinėms formomis- virškinamojo trakto angioedema, odos ir gerklų edema, o didžiausias C4 kiekis (0,098 ± 0,088) g/l sergant gerklų edema, tačiau statistiškai reikšmingai nesiskyrė. Tyrimo metu dviems pacientams (tėvui ir sūnui) buvo nustatyta genetinė SERPING1 mutacija. Išvados: Nustatyta tendencija, kad moterims angioedemos simptomų pradžia pasireiškė vėliau-15,38 metai, nei vyrams- 13,11 metai, tačiau statistiškai reikšmingo skirtumo nebuvo. Nustatyta tendencija, kad moterų diagnozės patvirtinimas nuo pirmųjų ligos simptomų vėlesnis- 6,13 metai, nei vyrų-5,44 metai, tačiau statistiškai reikšmingo skirtumo pagal lytį ir diagnozės nustatymo laiką nebuvo. Dažniausiai pasireiškusi klinikinė išraiška- odos ir ir poodžio edema- 4 (28,6 proc.), o rečiausios virškinamojo trakto angioedema - 1 (7,1 proc.), odos, gerklų ir virškinamojo trakto angioedema forma- 1 (7,1 proc.). Stiprių priepuolių dažnio vidurkis – 11,77 k./metus. Įvertinus ligos paveldėjimo tendencijas, nustatyta, kad dauguma pacientų- 15 (88 proc.) turi šeiminę anamnezę sirgti šia liga. Šeiminę anamnezę turi visos moterys- 8 (100 proc.) ir 7 (77 proc.) vyrai, tačiau reikšmingo skirtumo tarp pacientų, kurie turėjo šeiminę anamnezę, pagal lytį nenustatyta. Imuninių rezultatų (C1INH, C3, C4) rodmenys neturėjo statistiškai reikšmingo skirtumo tarp lyčių ir reikšmingai nesiskyrė sergant įvairiomis angioedemos klinikinėmis formomis. Įvertinus dviejų pacientų genetinius duomenis buvo nustatyta abiems pacientams SERPING1 de novo mutacija, kurią tėvas perdavė sūnui autosominiu dominantiniu būdu.
Hereditary angioedema is a rare disease inherited in an autosomal dominant manner. Genetic testing for this disease has not been performed in Lithuania for a long time. Genetic mutations can cause this disease, but there may be a gene defect without clinical expression of the disease. Molecular genetic testing identifies genetic mutations - SERPING1, SERPING2 or factor XII, angiopoietin 1 and plasminogen may help to elucidate the inheritance of the disease in patients with hereditary angioedema. Immune markers are assessed by the levels of C1INH, C3, C4- specific proteins, a simple method to confirm the diagnosis of hereditary angioedema and to differentiate it between three different types. The aim of the study: to investigate the genetic, clinical, and immune markers of patients with hereditary angioedema. Objectives of the study: 1) To determine the demographics, clinical variants and gender differences in patients with hereditary angioedema. 2) To identify immune markers and gender differences in patients with hereditary angioedema. 3) To compare clinical and immune markers in various forms of hereditary angioedema. 4) To determine the genetic factors of patients with hereditary angioedema, the inheritance trends of the disease among family members. Methodology: Data from the medical histories of 17 patients diagnosed with hereditary angioedema at the Clinic of Immunology and Allergology of Kaunas University of Health Sciences are analyzed. The results of genetic mutations and immune markers (C1INH, C3, C4) are evaluated. Results of the study: The study included 17 patients, 8 women (47%) and 9 men (53%). The mean year of onset of the first symptoms was 15.38 ± 6.63 years for women and 13.11 ± 7.67 years for men. The time to confirmation of the diagnosis was 6.13 ± 3.72 years for women and 5.44 ± 3.17 years for men, but there was no significant difference between gender and age at onset of symptoms and time to diagnosis. The majority of patients (15%) were found to have a family history of the disease. The average frequency of hereditary angioedema attack was 11.77 ± 9.56 times per year. The most common clinical forms were skin and subcutaneous edema- 4 (29%), then edema of the larynx- 3 (21%), skin and subcutaneous edema, larynx edema- 3 (21%), then skin and subcutaneous edema and gastrointestinal tract aangioedema- 2 (15%), skin and subcutaneous edema, larynx edema and gastrointestinal tract angioedema- 1 (7%), gastrointestinal tract angioedema-1 (7%). There is a tendency that more men suffer from laryngeal edema - 4 (66.7%) than women - 3 (37.5%). Average levels of immune markers in women: C1INH- 0,067 ± 0,021 g/l, C3- 1,25 ± 0,658 g/l, C4 - 0,04 ± 0,017 g/l and in men C1INH - 0,057 ± 0,018 g/l, C3- 0,75 ± 0,415 g/l, C4- 0,06 ± 0,033 g/l, there was no statistically significant difference. The lowest mean C1INH content (0.05 ± 0.008) g/l was in skin and subcutaneous edema and the highest C1INH content (0.12 ± 0.082) g/l was in skin and subcutaneous edema and laryngeal edema, but these results were not statistically significant. The lowest C3 content (0.68 g/l) was in several clinical forms - gastrointestinal tract angioedema, skin and subcutaneous edema and larynx edema, and the highest C3 content (1.15 ± 0.659) g/l in skin and subcutaneous edema, but did not differ statistically significantly. The lowest C4 content (0.04 ± 0.017) g/l was in skin and subcutaneous edema and several clinical forms - gastrointestinal tract angioedem, skin and subcutaneous edema and laryngeal edema, and the highest C4 content (0.098 ± 0.088) g/l in laryngeal edema also did not differ statistically. In the study, two patients (father and son) were found to have a genetic mutation in SERPING1. Conclusions: The onset of angioedema symptoms in women was found to be later than 15.38 years than in men at 13.11 years, but there was no statistically significant difference. There was a tendency for women to be diagnosed later than the first symptoms after 6.13 years than for men 5.44 years, but there was no statistically significant difference between gender and time of diagnosis. The most common clinical manifestations were skin and subcutaneous edema- 4 (28.6%) and the least common were gastrointestinal tract angioedema- 1 (7.1%), skin and subcutaneous edema, larynx edema and gastrointestinal tract angioedema- 1 (7.1%). Assessing the inheritance trends of the disease, it was found that the majority of patients (88%) have a family history of the disease. All women have a family history - 8 (100%) and 7 (77%) men, but there was no significant difference in gender between patients with a family history. Immune results (C1INH, C3, C4) did not differ statistically significantly between the sexes and did not differ significantly between different clinical forms of angioedema. Genetic data from two patients were evaluated in both patients with a de novo mutation in SERPING1 transmitted from father to son in an autosomal dominant manner.
