Lietuviško rekombinantinio žmogaus augimo hormono somatogeno-L klinikinio efektyvumo įvertinimas pirmais gydymo metais pacientams su augimo hormono nepakankamumu

Abstract

Pateikti lietuviško rekombinantinio augimo hormono (AH) Somatogeno-L (AB "Biofa", Vilnius) klinikinio efektyvumo rezultatai, gydant sergančiuosius hipopituitarizmu pirmais gydymo metais. Šis klinikinis tyrimas vykdytas 1996-1997 metais Endokrinologijos institute. 17 pacientų su patvirtinta augimo hormono deficito (AHD) diagnoze ir atviromis kaulinėmis augimo zonomis metus laiko buvo gydomi Somatogenu-L, kurio vidutinė dozė buvo 0,6 TV/kg/sav. Tyrėme 11 berniukų ir 6 mergaites, kurių amžiaus vidurkis buvo 9,12±4,49 metų. Augimo greitis statistiškai patikimai (p=0,00029) padidėjo nuo 3,98?1,85 prieš gydymą iki 7,17±2,55 (cm/m) per gydymo Somatogenu-L kalendorinius metus. Taip pat statistiškai patikimai (p=0,001) sumažėjo augimo greičio atsilikimas, išreikštas standartinio nukrypimo reikšme (SDS) pagal tarptautinius Tanner ir Whitehouse standartus, nuo -2,23±2,13 prieš gydymą iki 0,78±2,44 gydymo laikotarpiu. Po metų gydymo sumažėja ūgio deficitas, išreikštas SDS, o galutiniai apskaičiuoti ūgiai padidėja, tačiau šie duomenys yra statistiškai nepatikimi. Tyrimo metu Somatogenas-L pasireiškė kaip saugus ir efektyvus preparatas, gydant vaikus su hipopituitarizmu. Results of clinical efficacy of Lithuanian recombinant human growth hormone (rhGH) Somatogen-L during the first year of treatment in growth hormone deficient (GHD) patients are presented. The study was performed at the Institute of Endocrinology, Kaunas Medical University during 1996-1997. 17 GHD patients with open epiphyseal growth plates were treated with rhGH for one year at a mean dose of 0.6 IU/kg body weight per week. 11 boys and 6 girls with a mean age of 9.12±4.49 years were enrolled in this study. Growth velocity significantly (p=0.00029) increased from 3.98±1.85 before therapy to 7.17±2.55 (cm/yr) after one year of treatment with rhGH. Growth velocity standard deviation score (SDS) according to Tanner  Whitehouse international standards for chronological age significantly (p=0.001) reduced from -2.23±2.13 at baseline to 0.78±2.44 after one year of therapy. Patients height SDS and predicted adult heights increased after one year of treatment with rhGH, but these changes are not statistically significant. Somatogen-L is both effective and safe during the first year treatment in children with hypopituitarism.
