Uterine smooth muscle tumor of unknown malignant potential: A case report

Introduction Uterine smooth muscle tumor of unknown malignant potential is one of the rarest gynaecological tumors diagnosed after treatment and histological analysis. Case presentation 58 y/o patient arrived to Kaunas Clinics due to bloating, abdominal pain, polyuria and postcoital bleeding. For the last 15 years she had been monitored for leiomyoma that increased in size from 4 cm to 11 cm in the past year. TVS revealed a 111x 95 mm subserosal uterine fibroid, so hysterectomy with bilateral salpingoophorectomy was performed with histological result of leiomyoma with necrosis. 3 years later this patient came back to the hospital due to nausea, vomiting, epigastric pain and episodic rectal bleeding. Abdominal and pelvic CT scans showed numerous nodules that were intensively accumulating contrast. TVS presented 117x77x95 mm solid heterogenous mass that was reaching the aortic bifurcation and compressing the bladder. Even though the biopsy results reported leiomyoma, tumor reduction surgery revealed this was a smooth muscle tumor of uncertain malignant potential (STUMP). Later CT scans showed multiple metastatic nodules in the lungs, liver, pelvic muscles and in the subcutaneous layer. Some nodules were cryoablated and some were removed by laparotomic surgery.
