Aim: Conservative and surgical treatment of trophic ulcers, due to venous insufficiency: A Case Report. Method: Male of 51 years, suffers from venous insufficiency for 15 years. Trophic ulcers in the both calves that are not healing appeared 10 years ago. Wound is wet with fibrin coating with no signs of infection. The patient was consulted by a plastic surgeon regarding the further treatment of trophic ulcers. Surgery was recommended. First of all, lymphodrainage was applied for the patient to improve venous blood flow in the calves. Later, a surgical treatment – autodermolasty, and laser phlebectomy were performed. After the surgery, treatment lasted 12 days in the hospital, where wounds bandaging was applied. Outpatient treatment was recommended – wound bandaging with an ointment* every 2 to 3 days, and antibiotics – Cefadroxil 500 mg 2 times a day were prescribed for prevention of infection. Polyurethane wound dressing with a soft silicone foam dressing** was advised for the donor site wound. Also recommended for skin dryness to lubricate the legs with moisturizing cream, and apply compressive therapy with elastic bandage. 20 days after treatment trophic ulcers in the both calves healed completely, bloodflow and function fully restored, leg oedema and pain subsided. Results / Discussion: 20 days after treatment positive results were observed. Skin transplant was not rejected, no signs of infection were observed. Due to the application of a complex treatment trophic ulcers of the both calves healed completely. Leg oedema and pain subsided, bloodflow and function restored completely.
