A Rare Cause of Focal Hepatic Lesions: a Case Report

Introduction. T-cell lymphoma is a rare type of cancer and is categorized under Non Hodgkin Lymphoma. It may be either indolent or aggressive and can develop in lymphoid tissues, liver, skin, and gastrointestinal tract. Case description. A 31-year-old male patient with jaundice, vomiting, fever up to40℃ and abdominal pain was admitted to the hospital. Elevated inflammatory markers, hepatic enzymes, hyperbilirubinemia, and hepatosplenomegaly were observed, as wellas, pancytopenia, high levels of triglycerides and ferritin. Bone marrow puncture – biopsy confirmed the diagnosis of hemophagocytic lympho histiocytosis. Systemic corticosteroid therapy was effective and the patient was discharged. After 3 weeks, the patient develope dfever, abdominal pain, rash, dark urine, and pale stools. CT scan showed signs of pleuritisand pericarditis, infiltration in the left lung. Ultrasound showed enlarged right testicle withheterogenous zone, hepatosplenomegaly, heterogeneous, dysmetabolic liver with visiblehypoechogenic, heterogeneous masses with suspected air and fluid inclusions, and enlargedlymph nodes in the portal area of the liver. MRT showed signs of the malignant process:multiple contrast accumulating masses in liver, spleen, testicles, and prostate and pathologicallymph nodes in abdominal and groin area. Biopsy of the liver and testicle was performed, anddiagnosis of aggressive peripheral T-cell lymphoma was confirmed. Before starting etiologicaltreatment, the patient died of cardiopulmonary insufficiency, an autopsy was not performed.Summary. A patient complained of abdominal pain, fever, skin rash, pale stools, anddark urine. Tests showed signs of possible malignancy in the liver, spleen, testicles, prostateand lymph nodes. Biopsy confirmed the diagnosis of aggressive peripheral T-cell lymphoma.Unfortunately, the patient died before the etiological treatment was started.Conclusions. This case study shows that T-cell lymphoma may rarely arise in the liver andcause focal hepatic lesions, so it is important to diagnose this condition immediately due tothe aggressive course of the disease.
