A significant numbers of patients undergo Carotid Endarterectomy (CEA) experience cognitive chan- ges in the early postoperative period. The middle cerebral artery (MCA) is susceptible to fluctuations in blood flow velocity (BFV) changes during carotid cross-clamping. BFV can be measured in real time using Transcranial Dopp- ler (TCD) ultrasonography. Postoperative hypoperfusion syndrome is a proven risk factor for delayed neurocognitive recovery (dNCR), intraoperative reductions in cerebral blood flow velocity may contribute to postoperative cogni- tive decline. Aim. The aim of this study was to evaluate the association between MCA BFV fluctuations and delayed neurocogni- tive recovery and identify risk factors for dNCR. Methods. 27 patients without prior neurocognitive disor- ders undergoing CEA in LUHS Hospital, Kaunas Clinics, were enrolled in the study. Adenbrooke cognitive testing was performed prior to surgery and 1 day after surgery. TCD ultrasonography was performed 1 day before surgery and during CEA. Results. 21 patients were examined; 15 (71.4%) male. Ac- cording to the ACE-III test results, patients were divided into two groups: with dNCR (dNCR group) and without dNCR (non-dNCR group). dNCR developed for seven pa- tients (33.3%). ACM BFV was consistently higher in the postoperative dNCR group, with a statistically significant difference observed before carotid cross-clamping (median [min–max]: 43.1 [11.2–75.1] vs 26.4 [11.9–40.0] cm/s; p = 0.043). No statistically significant differences were ob- served at the remaining time points (p < 0.05). Bias-re- duced logistic regression showed that preoperative cogni- tive impairment was the dominant predictor of postoperative dNCR (penalised â » +3.8). In models exclud- ing preoperative cognitive impairment, preoperative demen- tia (â » +1–4), higher pre-clamping ACM BFV (standard- ized â » +0.6), and lower end-of-surgery rSO? (â » –0.04 to –0.17 per 1% increase) showed consistent directional asso- ciations. No predictor reached statistical significance due to limited sample size. Conclusions. Postoperative dNCR occurred in one-third of patients. Higher pre-clamping MCA BFV was associated with postoperative dNCR, suggesting that altered baseline cerebrovascular haemodynamics may play a role in cogni- tive decline. Elevated pre clamping MCA BFV in carotid stenosis reflects a compensatory hyperdynamic flow from collateral circulation. dNCR was most strongly predicted by preoperative cognitive impairment. When preoperative dNCR was excluded from analysis, dementia, higher pre clamping MCA blood flow velocity, and lower end of sur- gery rScO? showed consistent but non-significant associa- tions, likely due to the limited sample size.
10 The attitude of family physicians to the quality of patient teleconsultationsItem type:Publication, conference paper[2023][T1d][M001,M004][2]; ; Introduction The number of teleconsultations increased significantly during the COVID-19 pandemic (since2019) [1]. To reduce the flow of patients in medical institutions and to allow patients to consult a doctor, primary care providers in many countries have started to consult patients remotely [2,3]. Teleconsultations have created new opportunities for patient care. Remote consultations can be available in a variety of ways: video calls, telephone or other [4]. Despite the increasing number of teleconsultations in primary health care, more scientific data about the quality of remote consultations are needed. Aim To assess the attitude of the family physicians working in private and public institutions on the quality of patients’ teleconsultations’. Methods An anonymous questionnaire created by the authors, consisting of 16 questions (7 questionswere used to find out the attitude of family physicians about the quality of remote consultations).The study was conducted from February to June 2022, anonymous questionnaire was sent tothe email addresses of family physicians specified on public websites and shared on socialnetwork “Facebook” in medical groups in which family physicians participated. 243 familyphysicians working in private and public personal health care institutions participated in thestudy. The statistical data analysis was performed using “Microsoft Office Excel’’ 365 and SPSSVersion 27.0 software packages. The difference was considered statistically significant whenp<0.05. Research was approved by the Bioethics Center (No. BEC-MF-249) of the LithuanianUniversity of Health Sciences.ResultsOut of 243 family physicians, 54.3% were doctors from private institutions and 45.7% physiciansfrom public institutions. The attitude of family physicians about the duration of teleconsultationsdiffered as follows: 55.3% of doctors from private institutions chose the most appropriateduration of remote consultation was from 10 to 20 min., 55.9% of doctors from public institutionschose shorter duration - until 10 min. (p=0.04). Most respondents from each group indicated thatthe accuracy of diagnosis during teleconsultation depends on the complaints and contactconsultation is needed to inform patients about bad test results, also the teleconsultations affectthe quality of treatment and increase occurrence of medical errors, the most importantcommunication difficulty is informing patients in teleconsultations, the main advantage ofteleconsultation is avoiding contact with infectious diseases. The attitude of both groups did notdiffer statistically significantly in these aspects.ConclusionsThis study revealed that the attitude of family physicians about the appropriate duration ofteleconsultations differed depending on the institution where they worked. Based on otherquality criteria, the majority attitude of respondents coincided in this survey.

Kaunas : Vitae Litera, 2023, t. 27, Nr. 1., 2023-01-16, p. 10-17Nuotolinės pacientų konsultacijos yra neatsiejama pirminės asmens sveikatos priežiūros įstaigų (PSPĮ) paslaugų dalis, kuri ypatingą reikšmę įgijo COVID-19 pandemijos laikotarpiu. Šis laikotarpis sąlygojo nuotolinių konsultacijų plėtrą bei sukėlė diskusijas tarp privačiose ir valstybinėse įstaigose dirbančių šeimos gydytojų. Tyrimo tikslas. Palyginti šeimos gydytojų, dirbančių privačiose ir valstybinėse įstaigose, nuomonę apie pacientų nuotolines konsultacijas. Tyrimo metodai. Tyrimas buvo vykdomas 2022 m. vasario-2022 m. birželio mėn. Respondentų nuomonei apie nuotolinių konsultacijų vykdymo tvarką, jų kokybę ir plėtrą išsiaiškinti buvo sudarytas originalus straipsnio autorių klausimynas, skirtas šeimos gydytojams, dirbantiems privačiose, valstybinėse ir abiejose PSPĮ. Tyrime dalyvavo 243 respondentai. Visų surinktų duomenų statistinei analizei atlikti naudojamos „Microsoft Office Excel 365“ ir „SPSS 27.0“ statistinės programos. Skirtumas laikytas statistiškai reikšmingu, kai p < 0,05. Rezultatai. Visų trijų respondentų grupių nuomonė apie nuotolinių pacientų konsultacijų vykdymo tvarką (laiką nuo paskutinio vizito, pakartotinį nuotolinių konsultacijų teikimą, pacientų identifikavimą ir geriausią nuotolinės konsultacijos būdą) statistiškai reikšmingai nesiskyrė. Šeimos gydytojų nuomonė, nepriklausomai nuo darbovietės, apie nuotolinių konsultacijų kokybę skyrėsi dėl nuotolinių konsultacijų trukmės (p = 0,01). Nuomonė tarp šeimos gydytojų, dirbusių privačiose įstaigose, ir šeimos gydytojų, dirbusių abiejose įstaigose, dėl nuotolinės konsultacijos tinkamumo informuoti pacientą apie blogus tyrimo rezultatus statistiškai reikšmingai išsiskyrė (p = 0,047). Statistiškai reikšmingas skirtumas šiuo aspektu nustatytas ir tarp valstybinėse bei abiejose įstaigose dirbusių šeimos gydytojų (p = 0,02). Pagal kitus kriterijus (ligų diagnozavimo tikslumą, gydymo kokybę, klaidų tikimybę, bendravimo sunkumus bei nuotolinių konsultacijų privalumus) Šeimos gydytojų nuomonės sutapo. Daugelis respondentų nurodė, kad nuotolinių konsultacijų plėtra yra reikalinga ir SMS žinutė yra vienas iš tinkamiausių plėtros būdų. Išvados. Šeimos gydytojai, dirbantys tik privačiose įstaigose, nurodė ilgesnę nuotolinės konsultacijos trukmę, kaip veiksmingą nei Šeimos gydytojai, dirbantys valstybinėse arba abiejose. Šeimos gydytojai, dirbantys abiejose įstaigose, dažniau pažymėjo, kad skirtumo tarp nuotolinių ir kontaktinių konsultacijų nėra, informuojant pacientą apie blogus tyrimo rezultatus, o privačiose ir valstybinėse įstaigose dirbantys šeimos gydytojai - rečiau.
