The relationship between the angiotensin-converting enzyme 2 (ACE2)/renin-angiotensin system (RAS) axis and eicosanoid metabolism is still not fully understood. This study aimed to investigate whether changes in circulating ACE2 and 20-hydroxyeicosatetraenoic acid (20-HETE) reflect dysregulation of a combined ACE2/RAS-eicosanoid axis in ACS and to evaluate the diagnostic and functional significance of 20-HETE. We studied 122 patients with ischemic heart disease (including stable coronary artery disease (SCAD) and ACS) and 54 controls. Levels of circulating ACE2 and 20-HETE were measured using ELISA. Additionally, in vitro experiments with cell culture models were conducted to evaluate how pharmacological modulation of the ACE2/RAS axis affects nitric oxide (NO) production, inflammatory signaling, and CYP4F2 expression. Both ACE2 and 20-HETE levels were significantly lower in ACS compared to SCAD and controls (p < 0.001). Circulating 20-HETE effectively distinguished ACS from SCAD with an AUC of 0.91. In endothelial cells, altering the ACE2/RAS axis impacted NO availability and inflammatory gene expression, correlating with changes in CYP4F2 levels. 20-HETE affected cellular metabolic activity and GPR75 signaling, indicating a functional connection between RAS signaling and eicosanoid pathways. Lower circulating 20-HETE levels are associated with disruption of the integrated ACE2/RAS-eicosanoid axis, which is linked to endothelial dysfunction in ACS. These results suggest that 20-HETE may serve not only as a biomarker but also as part of a broader vascular signaling network related to acute coronary instability.

Gal-3 enhances platelet aggregation and thrombosis via activation of Dectin-1 receptors, contributing to cardiovascular risk, particularly in high-risk patients. Purpose This study aimed to evaluate the effects of a Galectin-3 inhibitor (Gal-3I) on platelet aggregation in patients with acute coronary syndrome (ACS), stable angina, and metabolic risk factors, investigating the role of Gal-3 in modulating platelet hyperreactivity. Methods Platelet aggregation was conducted using a two-component assay with classical agonists (ADP and epinephrine) alone and in combination with Gal-3I, on venous and arterial blood samples from patient cohorts. The study included assessments across gender, age, and diabetes status. Statistical analyses adjusted for confounders were performed to evaluate aggregation responses. Mann-Whitney and KruskalWallis tests were used. Results ACS patients exhibited significantly (p<0.05) lower platelet aggregation in response to ADP (venous blood) when combined with Gal-3I compared to stable angina and high-risk groups (median (min-max), 51 (11- 94)%agr vs 60 (6-90)%agr vs 64 (0-91)%agr, respectively).. No significant gender differences were found, though a trend of declining platelet function with age was noted in females. Gal-3I normalized platelet aggregation levels between patients with diabetes and control subjects, supporting its role in reducing platelet hyperreactivity linked with metabolic disorders. Conclusion(s) Galectin-3 is a critical modulator of platelet function, particularly in acute cardiovascular events and metabolic disease contexts. Inhibition of Gal-3 attenuates platelet hyperactivity and may represent a promising therapeutic avenue for reducing thrombotic risk in ischemic heart disease. Further research is warranted to clarify underlying pathways and optimize therapeutic strategies.

Pastarasis dešimtmetis medicinoje išsiskyrė įrodymais grįstos medicinos įsigalėjimu, diagnostikai bei gydymui naudojamų technologijų sudėtingumu ir įvairove, labai greitu mokslo bei technikos naujovių įdiegimu į kasdieninę klinikinę praktiką. Todėl tikslinga atnaujinti kardiologijos vadovėlį, kuris taptų ne vien priemone studentams mokyti, bet ir kiekvieno gydytojo rezidento, vidaus ligų gydytojo, kardiologo parankine knyga. Laikas bėga labai greitai, todėl net rengiant šį vadovėlį keitėsi daugelio ligų diagnostikos bei gydymo rekomendacijos, nes greita mokslo pažanga ir nauji duomenys nuolat keičia mūsų požiūrį į daugelį dalykų, kurie atrodė aiškūs ir nusistovėję. Todėl siūlome skaitytojui priimti skaitomą medžiagą kaip geriausią šiuo metu turimą informaciją ir kardiologinės patologijos sampratą, tačiau kartu palikti protą atvirą naujovėms, kurios gali ateiti plečiantis mūsų žinioms visose medicinos srityse. The main cause of IHD is atherosclerosis, which is a multifactorial inflammatory disease of blood vessels. Studies show that bacteria might have a significant impact on the pathogenesis of atherosclerosis and plaque rupture. This study aimed to evaluate the complexity of interactions between bacteria and the human body concerning metabolites and bacterial genes in patients with ischemic heart disease. Methods: Bacterial 16S rDNA and wcaF, papC, and sdhC genes were detected in whole blood using a real-time PCR methodology. An enzyme-linked immunosorbent assay was used to measure the concentration of the LL-37 protein. An analysis of ARA in blood plasma was performed. Results: Bacterial 16S rDNA was detected in 31% of the study patients, and the genes wcaF and sdhC in 20%. Enterobacterales genes were detected more frequently in patients younger than 65 years than in patients aged 65 years and older (p = 0.018) and in patients with type 2 diabetes (p = 0.048). Concentrations of the human antimicrobial peptide LL-37 and 12S-HETE concentrations were determined to be higher if patients had 16S rDNA and biofilm-specific genes. Conclusions: The results of this study enhance the understanding that Enterobacterales bacteria may participate in the pathogenesis of atherosclerosis and IHD. Bacterial DNA and host metabolites in higher concentrations appear to be detected.
42 Ūminiu miokardo infarktu sergančių pacientų diagnostikos ir gydymo algoritmai COVID-19 infekcijos metu : metodinės rekomendacijosItem type:Publication, book[2024][K2c][M001][49]; ; ; ; ; ; ; ; ; ; ; ; ; ; Kaunas : Lietuvos sveikatos mokslų universiteto Akademinė leidyba, 2024-02-26Kol kas mažai žinoma apie sunkaus ūminio respiracinio sindromo, kurį lemia koronaviruso (SŪRS-CoV-2) sukelta liga (toliau - COVID-19), įtaką ūminiams vainikinių arterijų sindromams (ŪVAS) atsirasti. Manoma, kad aterosklerozinės plokštelės destabilizavimą lemia keli su virusinėmis ligomis susiję patogenetiniai mechanizmai. COVID-19 atvejais pastebėti pokyčiai - aktyvus sisteminis uždegiminis atsakas, pasireiškiantis citokinų audra, imuninių ląstelių poliarizacijos pokyčiais. Dviejų iš penkių pacientų, sirgusių sunkiu ūminiu respiraciniu sindromu (SŪRS), mirties priežastimi pripažintas ūminis miokardo infarktas (MI). Svarbu žinoti, kad 2 tipo MI yra dažniausias sergant virusinėmis infekcijomis, todėl perodinės vainikinių arterijų intervencijos (PVAI) nauda ribota. Dėl šios priežasties pandemijos metu būtina pasverti intervencinio ir medikamentinio gydymo naudą bei žalą. Taip pat pastebėta, kad COVID-19 infekcijų atvejais nustatomas MI be obstrukcinės vainikinių arterijų (VA) pažaidos - tai siejama su gausia širdies pericitų angiotenziną konvertuojančiojo fermento (AKF) 2 raiška (sin. ekspresija). Be abejo, citokinų audra taip pat turi jau žinomą įtaką endotelio disfunkcijos patogenezei. Pacientai, sergantys ūminiais miokardo išemijos sindromais (TLK-10-AM kodai: 120.0,121,122), gydomi vadovaujantis Lietuvos Respublikos sveikatos apsaugos ministro 2014 m. vasario 17 d. įsakymu Nr. V-244 „Dėl asmens sveikatos priežiūros paslaugų įtariant ar diagnozavus ūminį miokardo infarktą, kai ST segmentas pakilęs, teikimo tvarkos aprašo patvirtinimo“, 2010 m. gruodžio 17 d. įsakymu Nr. V-1078 „Dėl ūminio miokardo infarkto, kai ST segmentas pakilęs, diagnostikos ir gydymo tvarkos aprašo patvirtinimo“ ir 2017 m. rugpjūčio 3 d. įsakymu Nr. V-940 „Dėl sveikatos priežiūros paslaugų, teikiamų ūminio miokardo infarkto be ST segmento pakilimo (TLK-10 AM I 21.4) atveju, prieinamumo gerinimo ir šia liga sergančių pacientų srautų valdymo reikalavimų patvirtinimo“, taip pat pagal Europos kardiologų draugijos širdies ir kraujagyslių ligų diagnostikos ir gydymo taktikos COVID-19 pandemijos metu rekomendacijas (1) parengtas pacientams, sergančių ūminiais miokardo išemijos sindromais (TLK-10-AM kodai: 120.0,121, 122), kuriems yra įtariama ir (ar) nustatyta COVID-19 liga (koronaviruso infekcija).

Hoboken, New Jersey : John Wiley & Sons, Ltd, 2022, vol. 39, no. 11., 2022-11-08, p. 1439-1445.Aims: This study sought to examine the feasibility, accuracy and reproducibility of a novel, fully automated 2D transthoracic echocardiography (2D TTE) parasternal long axis (PLAX) view aortic measurements quantification software compared to board-certified cardiologists in controlled clinical setting. Methods and results: Aortic Annulus (AoA), Aortic Sinus (AoS), Sinotubular Junction (STJ) and Proximal Ascending Aorta (AAo) diameter measurements were performed retrospectively on each of 58 subjects in two different ways: twice using a fully automated software (Ligence Heart version 2) and twice manually by three cardiologists (ORG) and one expert cardiologist (EC). Out of 58 studies AoA was measured in 54 (93%), AoS in 55 (95%), STJ in 55 (95%) and AAo in 54 (93%) studies. Automated measurements had a stronger correlation with EC when compared toORGwith the largest correlation difference of .1 for STJ measurements and lowest difference of .01 for AoS measurements. Automated software was in higher agreement with ground truth intervals (ORGmeasurementsmean +- SEM) in three out of four measurements. Conclusion: Fully automated 2DTTE PLAX view aortic measurements using a novel AIbased quantification software are feasible and yield results that are in close agreement with what experienced readers measuremanually while providing better reproducibility. This approach may prove to have important clinical implications in the automation of the aortic root and ascending aorta assessment to improve workflow efficiency.

Basel : MDPI, 2022, vol. 12, no. 19., 2022-10-06, p. 1-15.Abstract: (1) Background: The aim of this study was to determine clinical and genetic factors predicting the development of dyspnea in patients receiving ticagrelor. (2) Methods: A total of 277 patients with acute myocardial infarction (with and without ST-segment elevation), who underwent coronary angiography and PTCA with stent implantation and treated with antiplatelet drugs (ticagrelor and aspirin), were enrolled in this study. Platelet aggregation (induction with high-sensitivity ADP, ADP HS) testing was performed using a MULTIPLATE analyzer and reagents for the determination of P2Y12 receptor activity. Venous blood samples were collected for genotyping. (3) Results: Patients experiencing ticagrelor-related dyspnea had lower ADP HS. ROC curve analysis showed that an ADP HS cut-off of ≤19.5 U was associated with the development of dyspnea. The ADP HS value of ≤19.5 U and any dose of atorvastatin lower than 80 mg (or no atorvastatin) increased the risk of dyspnea by more than 4 and 2 times, respectively (OR = 4.07, p ≤ 0.001 and OR = 2.25; p = 0.008). (4) Conclusion: A lower ADP HS value possibly indicates greater ticagrelor activity and a higher plasma concentration of this drug. Atorvastatin might have an impact on the occurrence of ticagrelor-related dyspnea by affecting ticagrelor metabolism. No impact of any genetic variant on the development of dyspnea was determined.
