Lyginamieji viršūninio apydančio būklės ir šaknų kanalų gydymo kokybės tyrimai, taikant kūginio pluošto kompiuterinę tomografiją bei skaitmeninę rentgenografiją : daktaro disertacija : medicinos ir sveikatos mokslai, odontologija (M 002)Item type:Publication, [Comparison Study on Dental Periapical Status and the Quality of Root Canal Treatment by Cone Beam Computed Tomography and Digital Periapical Radiography]doctoral thesis[2023][R1][M002][94]; ; Kaunas :: Lietuvos sveikatos mokslų universiteto Akademinė leidyba,, 2023-03-31Radiologinis tyrimas yra vienas iš pagrindinių viršūninio apydančio patologijos nustatymo metodų. Periapical and Endodontic Status Scale).
48 41 The Changes in size of periapical lesions after root canal treatments assessed by digital periapical radiography and cone-beam computed tomography: a 2-years prospective clinical studyItem type:Publication, journal article[2022][S1a][M002][10]; ;Hellén-Halme, KristinaMedicina. Kaunas ; Basel : LSMU ; MDPI, 2022, vol. 58, no. 10., 2022-10-12, p. 1-10.Background and Objectives: There is limited information regarding comparison of long-term dynamics of periapical bone destruction estimated by digital periapical radiography (DPR) and by cone-beam computed tomography (CBCT). This study aimed to compare the radiographically assessed periapical changes of endodontically treated teeth over 2 years of follow-up and to analyse disagreements in periapical lesion size estimates around the same roots using DPR and CBCT. Materials and Methods: A total of 176 endodontically treated teeth, of 128 patients with apical periodontitis, were assessed by DPR and CBCT, at baseline and after 2 years. All periapical radiolucencies were categorised by severity (S0, S1, S2, S3) concerning their size. Descriptive statistics were used to report distribution of the radiolucencies at baseline and at follow-up, and their size transitions over 2 years. Site-specific comparison of the radiolucencies identified by two methods was performed using Z test and Pearson’s chi-square test. Results: majority of the detected radiolucencies were scored as S0: 65% and 68% at baseline; 89% and 83% at follow-up, by DPR and CBCT, respectively. Site-specific score comparison showed that disagreements comprised 18% and 20% of the total number of radiolucencies detected by DPR and CBCT, respectively. There were more disagreements between DPR and CBCT within categories S1 and S2 + S3 compared to S0: at baseline, they comprised 17–33% and after two years 62–95% of all detected radiolucencies within the category. 65% of non-matching score transitions over two years occurred between S0 and S1. The CBCT-based evaluation resulted in negative treatment outcomes for 10 more root canals than the DPR-based result. Conclusions: Most remarkable disagreement between DPR and CBCT recordings was observed within the radiolucency categories S2 and S3. However, the diagnostic accuracy of both radiographic methods was questionable as it resulted in a high proportion of non-matching S0-S1 lesion transitions over 2 years.

[Kaunas : Stilus optimus], 2020, vol. 11, no. 2., 2020-08-20, p. 1-9.Objectives: To compare selected anatomical and treatment-related diagnostic parameters estimated by cone-beam computed tomography and by digital periapical radiography in teeth with apical periodontitis, and to evaluate reliability of different examiners in interpretation of images obtained by both methods. Material and Methods: Teeth with apical periodontitis were evaluated independently by 2 endodontists and 1 radiologist based on 128 cone-beam computed tomography (CBCT) and 162 digital periapical radiography (DPR) images. Anatomical (size, relation with root, location of periapical radiolucency) and treatment-related (canal obturation length, homogeneity, coronal seal) parameters were assessed. Fleiss kappa reflected inter-observer agreement while intra-examiner agreement was estimated by Cohen’s kappa. McNemar and McNemar-Bowker tests served for evaluation of differences between CBCT- and DPR-based estimates. Results: Cohen’s kappa ranged from 0.62 to 1 for all examiners. Fleiss kappa values were nearly perfect for majority of parameters. Diagnostic discrepancy between methods was found for size of radiolucency that in 15 - 17% cases was larger, and in 25 - 28% smaller in DPR than in CBCT images. DPR revealed 20% of root canals scored as non-obturated while in CBCT - obturation present. Canal obturation was rated as homogenous by CBCT, while absent or non-homogenous by DPR, in 17 - 23%, and 11 - 14% of cases, respectively. Radiologist detected more root perforations in CBCT than in DPR images. Conclusions: Good intra- and inter-examiner agreement for anatomical and treatment-related diagnostic parameters was achieved using cone-beam computed tomography and digital periapical radiography methods and demonstrated similar diagnostic capability, although variation regarding root perforations and canal obturation quality was observed.
17 Diagnostic performance of Core Beam Computed Tomography and Digital Periapical Radiography, in adult patients with apical periodontitisItem type:Publication, conference paper[2018][T2][M002][1]; 16th European Congress of DentoMaxilloFacial Radiology : Lucerne, Switzerland, 14-16th June 2018 / European Academy Dentomaxillofacial Radiology. 2018., 2018-06-14, p. 1-1 : pav., lent.Introduction. Interpretation of the periapical structures in a radiographic image plays a significant role in the diagnosis, treatment planning and outcome evaluation. Based on the selected radiographic methods, different criteria of diagnosis of the periapical status can be used. Aim. To compare estimates of selected anatomical and treatment-related parameters of teeth with apical periodontitis (AP) using Cone Beam Computed Tomography (CBCT) and Digital Periapical images (DPI) based on Periapical and Endodontic Status Scale (PESS)*. Materials and methods. Study participants: 51 patient diagnosed with apical periodontitis, age range 27-78 yrs. Study subject: teeth with radiographically detected signs of periapical bone loss. Outcome measurements: evaluated according to the PESS variables: periapical radiolucency (size and relation with root), length of the root canal filling, in addition - the root thickness and length in alveolar bone (the smallest measurements) (Figure 3, 4, 5). Methods of assessment: CBCT (iCat scanner), the best quality images, and DPI (Kodak), parallel technique. Assessment criteria: PESS consisting of the complex periapical index (COPI)(figure 1) designed for the identification and classification of periapical bone lesions in cases of AP, and the endodontically treated tooth index (ETTI) (figure 2), designed for endodontic treatment quality evaluation .[...].
15 Evaluation of prognostic value of Periapical and Endodontic Status Scall (PESS), based on two years observation of endodontically treated teeth with apical periodontitisItem type:Publication, conference paper[2017][T2][M002][2]4th junior meeting EADMFR : Porto, Portugal, 5-8 February 2017 / European Academy DentoMaxilloFacial Radiology. 2017., 2017-02-05, p. 20-21.Study aims: to evaluate the survival rate of the endodontically treated teeth with apical periodontitis (AP) over two years, based on Cone Beam Computed Tomography (CBCT) and on digital periapical intra-oral images (DPI), and to estimate the prognostic value of the newly proposed Periapical and Endodontic Status Scale (PESS). Objectives: 1. To compare efficacy of the CBCT and of DPI, for evaluation of the periapical changes in the endodontically treated teeth over two years follow up. 2. To evaluate the impact of the selected anatomical and treatment-related parameters for survival and healing of the endodontically treated teeth, over two-year follow up period. 3. to estimate the prognostic value of Periapical and Endodontic status scale, based on the observed periapical changes over 2 years time. Materials and methods: Prior to the study, the approval of Regional Bioethics committee of Kaunas, Lithuania, will be obtained. The intended beginning of the study - December, 2016, duration - 2 years. The sample of this longitudinal, prospective clinical study will be adult patients (age from 18 to 60 years), referred for treatment to specialist in endodontology. Sample size calculation is based on the data from a clinical study reported by Patel et al, 2012. Thus, with the power of 0,9 and α = 0.05, the estimated study sample should comprise a minimum of 150 teeth. A total number of 165 teeth would allow for a 10 % drop out over the two years period of the study. [...].
