Tyrimo tikslas. Nustatyti anemija sergančių pacientų baigtis po kelio arba klubo sąnario endoprotezavimo operacijų ir anemijos korekciją perioperaciniu laikotarpiu. Tyrimo metodika. Atliktas retrospektyvusis tyrimas. Tiriamieji – 407 Kauno klinikų pacientai, kuriems 2013 m. buvo endoprotezuotas kelio arba klubo sąnarys. Analizuoti pacientų ligos istorijų duomenys, registruota anemijos diagnozė, gydymas, komplikacijos po operacijos, baigtys. Skirtumas laikytas statistiškai reikšmingu, kai p<0,05. Rezultatai. Anemijos prieš operaciją (toliau – PrA) dažnis – 26 proc. Patologija po operacijos nustatyta 5,6 proc. pacientų. PrA grupėje komplikacijos nustatytos 9,4 proc. pacientų, kontrolinėje grupėje – 4,7 proc. Iš 146 pacientų, kurie nukraujavo >300 ml, 0,7 proc. paūmėjo IŠL, 2 proc. nustatyta širdies aritmija. EM transfuzija atlikta 41,5 proc. pacientų PrA grupėje ir 16,9 proc. – ne PrA grupėje (p<0,05). 95 pacientams, kuriems atlikta EM transfuzija, 2,1 proc. paūmėjo IŠL, 4,2 proc. – širdies aritmija, 1,1 proc. – pjūvio infekcija, 1,1 proc. mirė. Išvados. Ketvirtadaliui pacientų nustatyta PrA. Tarp anemijos ir kontrolinės pacientų grupių komplikacijų po operacijos ir mirčių dažnis statistiškai reikšmingai nesiskiria, tačiau EM transfuzija statistiškai reikšmingai dažniau atliekama anemijos grupės pacientams. Iki operacijos nustatytas mažesnis hemoglobino kiekis susijęs su didesne išemijos ir širdies aritmijos rizika.
The aim of the study. To determine perioperative anemia (PrA) outcomes after knee or hip replacement surgeries & perioperative management of anemia. The methods of the study. A retrospective analysis included 407 patients (pts) who underwent elective hip or knee surgery in Hospital of Lithuanian University of Health Sciences during the 2013 year. Registered diagnosis of anemia, treatment, postoperative complications and outcomes were analyzed using patient case histories. Results with values of p<0,05 were considered statistically significant. The results of the study. The rate of PrA was 26 pct of cases. Postoperative morbidity was identified in 5.6 pct. of cases. Complications were diagnosed for 9.4 pct. pts of PrA group versus 4.7 pct of the control group. From 146 pts who lost over 300 ml of blood during surgery 0.7 pct. had IHD, 2 pct. pts. had cardiac arrhythmias. 41.5 pct. received blood transfusions. (BT) in PrA group compared to 16.9 pct. of non PrA group (p<0.05). From 95 pts who received BT 2.1 pct. pts had IHD, 4.2 pct. - arrhythmias, 1.1 pct. – wound infection and 1.1 pct. – death. Conclusions. One quarter of pts are diagnosed with PrA. Anemia and control groups are comparable in terms of the rate of postoperative complications and mortality. However, a lower preoperative Hblevel is associated with increased risk of blood transfusion, ischaemic events and cardiac arrhythmias.
