Hepatito B paplitimas tarp medikų ir jo prevencija skiepais Kauno ir Marijampolės apskričių gydymo įstaigose

Atvejo-kontrolės tyrimas, norint nustatyti hepatito B (HB) paplitimo ypatumus tarp Kauno ir Marijampolės apskričių medikų. Tyrime dalyvavo sąlyginai sveiki hepatitu B nesirgę asmenys. Pagrindinėje tyrimo grupėje buvo 2349 medikai, I kontrolinėje grupėje – 310 Kauno medicinos universiteto studentų, II kontrolinėje grupėje – 771 asmuo. HB žymenys tirti imunofermentiniais ELISA metodais. HB persirgimo dažniai lyginti tarp medikų profesijų, amžiaus grupių, apskričių, darbo stažo ir t.t. Statistiškai reikšmingai nustatyta (a=0,05; b=0,1), kad slaugytojos ir felčeriai beveik dvigubai dažniau, o laborantai 2,3 karto dažniau (a =0,05; b=0,15) suserga HB lyginant su I kontroline grupe. Medikų ir kitų profesijų gyventojų atsitiktinėje atrankoje persirgto HB dažniai statistiškai nesiskiria (p>0,6). Persirgus HBV nešiojimo tikimybė amžiaus grupėje iki 35 metų yra 0,11 didesnė nei grupėje, kur tiriamieji vyresni nei 50 metų (a=0,05; b=0,1). 2147 hepatitu B nesirgę medikai ir studentai skiepyti skiepu Euvax B. 356 asmenims tirtas poskiepinis imunitetas, 96,45 proc. medikų ir 98,36 proc. studentų rastas pakankamas imunitetas po HB skiepų.. Galima teigti, kad vidurinysis medicinos personalas turi žymiai didesnę tikimybę užsikrėsti HB negu gydytojai. Darbo stažas, amžius didina tikimybę užsikrėsti HB. Pastebima akivaizdi nepakankamo imuniteto susidarymo tikimybės priklausomybė nuo amžiaus po HB skiepų.
Case control investigation to determine the peculiarities of hepatitis B (HB) prevalence among the health care workers of Kaunas and Marijampolė districts. HB markers were detected by serological tests. 2349 healthy medical workers of these two districts were participating in the main research group,116 medical workers refused to participate in the research, anti HBcor were found in 365 (16.34%) and HBsAg – 21 (0.94%). 310 students of Kaunas University of Medicine were included into the first control group of the research, anti HBcor were found in 31 (10%), HBsAg – 1 (0.32%). 1868 medical workers and 279 medical students (having no HB markers) were vaccinated with HB vaccine Euvax B. Enough immunity after vaccination was 98.36% in students and 96.45% in medical workers. 771 residents these districts (not medical workers) formed second control group (prevalence of HB among population), anti HBcor were found in 132 (17.11%). Frequency of being ill on HB infection was compared among medical specialities, age groups, districts, work record and etc. Statistically reliably determined (a=0.05; b=0.1) that nurses and midwifes twice frequently and laboratory technicians get ill 2,3 times more frequently (a=0.05; b=0.15), if compared with I control group. Frequency of have already being ill with HB does not change in the group of medical workers and local population. Probability of carrying HB virus in the age group up to 35 years is 0.11 times higher than in the age group over 50 years (a=0.005; b=0.1). We can affirm that length of service, age increases chances to be infected with HB. We can obviously observe a dependence of probability of formation immunity upon the age after HB vaccination.
