Antrinių peritonitų mikrobiologinė diagnostika, sukėlėjų spektras ir atsparumas priešmikrobiniams vaistams

Tyrimo tikslas. Išanalizuoti antrinio peritonito sukėlėjus ir jų atsparumą priešmikrobiniams vaistams LSMUL KK stacionaro skyriuose. Tyrimo uždaviniai: 1. Nustatyti antrinio peritonito sukėlėjų spektrą bei dažnį. 2. Įvertinti dažniausiai išskirtų sukėlėjų atsparumą priešmikrobiniams vaistams. 3. Įvertinti polimikrobinių infekcijų dažnį tirtiems pacientams. 4. Įvertinti pacientų, kuriems buvo diagnozuotas antrinis peritonitas, išeitis. Tyrimo objektas ir metodai. Buvo analizuojami 2019 metais Lietuvos sveikatos mokslų universiteto ligoninės Kauno klinikose nuo antrinio bakterinio arba grybelinio peritonito gydytų pacientų duomenys. Atlikta retrospektyvinė 138 atvejų analizė, vertinant mikrobiologinių pasėlių ir pacientų elektroninių ligos istorijų demografinius bei klinikinius duomenis. Rezultatai. 138 pacientų, gydytų nuo antrinio peritonito, pasėliuose nustatyti 244 sukėlėjai. 51,6 proc. (n=126) sukėlėjų sudarė enterobakterijos, 20,2 proc. (n=49) enterokokai, 8,2 proc. (n=20) bakteroidai, 6,6 proc. (n=16) Pseudomonas aeruginosa, 5,3 proc. (n=13) Candida spp. 65,0 proc. (n=13) K. pneumoniae padermių pasižymėjo dauginiu atsparumu vaistams. Visos Enterobacterales padermės buvo jautrios meropenemui (n=126). Vankomicinui atsparios buvo 67,9 proc. (n=19) E. faecium ir 5,0 proc. (n=1) E. faecalis padermių. Enterococcus gentyje nustatyta E. faecium padermė atspari linezolidui (2,0 proc.; n=1). Dvi arba daugiau padermių buvo nustatyta 45,6 proc. (n=63) tiriamųjų pasėliuose. 79,1 proc. (n=34) Enterococcus spp. padermių, taip pat 84,6 proc. (n=11) Proteus spp. ir 59,0 proc. (n=36) E. coli padermių nustatyta polimikrobinių infekcijų atvejais. Tirtų pacientų imtyje pasveiko 68,1 proc. (n=94) pacientų. Reikšmingai rečiau pasveiko 65 metų arba vyresni (53,0 proc.; n=35), turėję dvi arba daugiau gretutines ligas (20,9 proc.; n=10) pacientai. Neišgyvenusių pacientų imtyje 70,5 proc. (n=31) atvejų nustatyta bent viena dauginiu atsparumu vaistams pasižymėjusi padermė. Pasveiko 66,2 proc. (n=45) tirtų vyrų ir 70,0 proc. (n=49) moterų. Išvados: 1. Esant antriniam peritonitui, 80 proc. nustatytų sukėlėjų priklausė normaliai žarnyno mikrobiotai: Enterobacterales šeimos, Enterococcus ir Bacteroides genčių bakterijos. Rečiau buvo nustatomos Pseudomonas sp. ir Candida spp. padermės. 2. Tarp Enterobacterales šeimos antrinio peritonito sukėlėjų dauginiu atsparumu vaistams dažniausiai pasižymėjo K. pneumoniae padermės. Karbapenemams atsparių enterobakterijų nustatyta nebuvo. Tarp Enterococcus spp. sukėlėjų E. faecium buvo patikimai dažniau atsparus ampicilinui ir vankomicinui lyginant su E. faecalis. Nustatyta viena E. faecium padermė atspari linezolidui. 3. Beveik pusei tirtų pacientų nustatytas daugiau nei vienas sukėlėjas. Reikšmingai dažniau infekcija buvo polimikrobinė, kai nustatyti Enterococcus spp., E. coli ir Proteus spp. sukėlėjai. 4. Du trečdaliai pacientų, kuriems buvo nustatytas antrinis peritonitas, pasveiko. Reikšmingai rečiau pasveiko pacientai, kurie buvo 65 metų ir vyresni bei turėjo gretutinių ligų. Neišgyvenusių pacientų imtyje reikšmingai dažniau buvo nustatytos dauginio atsparumo vaistams padermės. Vertinant antrinio peritonito išeitis, reikšmingo skirtumo tarp lyčių nenustatyta.
Aim of the study. To analyse the spectrum of pathogens causing secondary peritonitis and their resistance to antimicrobial drugs in inpatient departments. Objectives of the study: 1. To determine the spectrum of pathogens causing secondary peritonitis. 2. To evaluate the antimicrobial resistance of most commonly isolated pathogens. 3. To evaluate the occurrence of polymicrobial infections among studied patients. 4. To evaluate the outcomes of patients who were diagnosed with secondary peritonitis. Object and methods of the study. All patients of the study received treatment for secondary bacterial or fungal peritonitis in the Hospital of Lithuanian University of Health Sciences Kaunas Clinics in 2019. A retrospective analysis of 138 cases was performed including data of microbiological cultures and patients’ electronic medical records. Results. Among 138 patients treated from secondary peritonitis 244 pathogens were isolated. 51,6% (n=126) of them were Enterobacterales family bacteria, 20,2% (n=49) were Enterococcus spp., 8,2% (n=20) Bacteroides spp., 6,6% (n=16) Pseudomonas aeruginosa and 5,3% (n=13) Candida spp. 65,0% (n=13) of K. pneumoniae isolates were multidrug-resistant. All Enterobacterales strains were carbapenem-susceptible (n=126). Vancomycin-resistant were 67,9% (n=19) of E. faecium and 5,0% (n=1) of E. faecalis isolates. In the genus Enterococcus, one E. faecium strain was found to be linezolid-resistant (2,0%; n=1). Two or more strains were found in 45,6% (n=63) of evaluated microbiological cultures. 79,1% (n=34) of Enterococcus spp., 84,6% (n=11) of Proteus spp. and 59,0% (n=36) of E. coli isolates were detected in cases of polymicrobial infection. 68,1% of patients treated from secondary peritonitis recovered. The recovery rate was significantly lower among patients who were 65 years or older (53,0%; n=35) and had two or more comorbidities (20,9%; n=10). In non-survivors, the presence of multidrug-resistant species was 70,5% (n=31). The recovery rate was 66,2% (n=46) in males and 70,0% (n=49) in females. Conclusions: 1. 80% of isolated microorganisms causing secondary peritonitis belong to normal intestinal microbiota: Enterobacterales family pathogens, Enterococcus spp. and Bacteroides spp. strains. Other isolates included Pseudomonas sp. and Candida spp. 2. Among the Enterobacterales family pathogens of secondary peritonitis, K. Pneumoniae strains were most often found to be multidrug-resistant. None of analysed Enterobacterales family pathogens were carbapenem-resistant. Significantly higher resistance to ampicillin and vancomycin was determined in E. faecium isolates compared to E. faecalis. One strain of E. faecium was found to be linezolid-resistant. 3. In almost half of analysed cases two or more pathogens were detected. Polymicrobial infection was found significantly more often when pathogens were Enterococcus spp., E. coli and Proteus spp. 4. Two-thirds of patients treated from secondary peritonitis recovered. A significantly lower recovery rate was observed when patients were 65 years or older and had comorbidities. In non-survivors, the presence of multidrug-resistant species was significantly more common. No significant sex‐related difference of outcomes was found.
