Darbo tikslas: įvertinti pacientų, sergančių lėtiniu širdies nepakankamumu, kepenų funkcijos sutrikimus. Darbo uždaviniai: 1. Įvertinti kepenų funkcijos rodiklius, sergant lėtiniu širdies nepakankamumu. 2. Ištirti ALT, AST ir bendro bilirubino koncentracijos pokyčių priklausomybę nuo NYHA širdies nepakankamumo klasės. 3. Įvertinti NT-proBNP padidėjimo ir kepenų funkcijos rodiklių koreliaciją. 4. Nustatyti ryšį tarp kepenų pažeidimo klinikinių požymių ir kairiojo skilvelio išmetimo frakcijos bei NYHA funcinės klasės. Tyrimo metodika: analizuotos 85 pacientų, kurie 2017 metais buvo gydomi Kauno klinikinės ligoninės (KKL) kardiologijos skyriuje dėl lėtinio širdies nepakankamumo, ligos istorijos. Atrinkti pacientai, kurių diagnozė stacionarizavimo metu buvo stazinis širdies nepakankamumas. Iš ligos istorijų išrinkti reikalingi duomenys buvo išanalizuoti naudojantis SPSS 24.0 paketu ir Microsoft Excel 2013 programomis. Pasirinktas reikšmingumo lygmuo p=0,05. Rezultatai: Vidutinis pacientų amžius 73,99 ± 11,278 metai. Sumažėjusi kairiojo skilvelio išmetimo frakcija (KSIF) (<45%) nustatyta 67,1% pacientų. 50,6% pacientų priklauso IV funkcinei klasei pagal NYHA klasifikaciją, 41,2% pacientai – III funkcinei klasei, 8,2% pacientai – II funkcinei klasei. Padidėjusi NT-proBNP reikšmė nustatyta 90,59% pacientų. Padidėjusi bendro bilirubino koncentracija kraujyje nustatyta 78,8% pacientų ir dažniau padidėja, kuomet KSIF yra sumažėjusi. Ascitas buvo diagnozuotas 20% pacientų, kepenų padidėjimas - 55,3% pacientų. Išvados: 1. 78,8% pacientų, sergančių LŠN, nustatyta padidėjusi bendro bilirubino koncentracija kraujyje. Kepenų fermentų ALT ir AST koncentracijos padidėjimas nustatomas rečiau (atitinkamai 3,53% pacientų ALT ir 7,05% - AST). 2. Bendro bilirubino bei kepenų fermentų ALT ir AST koncentracija nepriklauso nuo NYHA funkcinės klasės (p>0,05). 3. Padidėjusi bendro bilirubino koncentracija dažniau nustatoma, kai NT-proBNP koncentracija yra padidėjusi (p=0,034). Kepenų fermentų ALT ir AST koncentracija nepriklauso nuo NT-proBNP koncentracijos (p>0,05). 4. Kepenų pažeidimo klinikiniai požymiai (ascitas ir padidėjusios kepenys) dažniau pasireiškia, kai KSIF sumažėjusi (p<0,05). Taip pat nustatyta, jog ascitas pasireiškia dažniau IV NYHA funkcinės klasės pacientams nei III (p<0,0001), tačiau kepenų padidėjimo dažnis nepriklauso nuo NYHA funkcinės klasės (p>0,05).
Author of the Paper: Neringa Užkurnienė. Title of the Paper: Hepatic Impairment in Patients with a Chronic Heart Failure. Aim of the Paper: To assess hepatic dysfunctions in patients suffering from a chronic heart failure. Objectives of the Paper: 1. To assess hepatic function indicators when suffering from a chronic heart failure. 2. To examine the dependence of the changes of ALT, AST and total bilirubin concentrations on the NYHA class of heart failure. 3. To assess the correlation between NT-proBNP increase and hepatic function indicators. 4. To determine the relationship between clinical peculiarities of hepatic impairment, left ventricular ejection fraction and the NYHA functional class. Research methodology: Case-histories of 85 patients, who were treated in the Department of Cardiology of Kaunas Clinical Hospital (KCH) during the year 2017 from a chronic heart failure, were analysed. The patients whose diagnosis during hospitalization was a congestive heart failure were selected. The necessary data extracted from case-histories were analysed by using SPSS 24.0 package and Microsoft Excel 2013. The chosen significance level was p=0.05. Results: The average age of the patients was 73.99 ± 11.278 years. A reduced left ventricular ejection fraction (LVEF) (<45%) was identified in 67.1% of the patients. 50.6% of the patients were included in the functional class IV under the NYHA classification, 41.2% of the patients belonged to the functional class III, while 8.2% of the patients were involved in the functional class II. An increased value of NT-proBNP was found in 90.59% of the patients. Further, a total bilirubin concentration in the blood was found in 78.8% of the patients and it increased more frequently when LVEF was decreased. Ascites was diagnosed to 20% of the patients, while liver enlargement to 55.3% of the patients. Conclusions: 1. An increased total bilirubin concentration in a blood was found in 78.8% of the patients suffering from CHF (chronic heart failure). An increase of the concentration of liver enzymes ALT and AST was found less frequently (3.53% of the patients - ALT and 7.05% of the patients – AST, respectively). 2. The concentration of total bilirubin and liver enzymes ALT and AST does not depend on the NYHA functional class (p>0.05). 3. An increased concentration of total bilirubin is found more frequently when NT-proBNP concentration is increased (p=0.034). The concentration of liver enzymes ALT and AST does not depend on NT-proBNP concentration (p>0.05). 4. Clinical peculiarities of hepatic impairment (ascites and enlarged liver) are more frequent when LVEF is decreased (p<0.05). Moreover, it was also ascertained that ascites is more frequent among patients of the NYHA functional class IV than patients of class III (p<0.0001). However, the liver enlargement rate does not depend on the NYHA functional class (p>0.05).
