Išanalizuoti veiksnius, lemiančius pacientų perioperacinio gydymo trukmę širdies chirurgijos skyriuje. Uždaviniai. 1. Nustatyti ir įvertinti priešoperacinio laikotarpio veiksnius ir jų reikšmę iki operacinio laikotarpio stacionaro skyriuje trukmei. 2. Nustatyti ir įvertinti veiksnius ir jų reikšmę paciento gydymo trukmei intensyvios terapijos skyriuje po atliktos širdies operacijos. 3. Nustatyti ir įvertinti veiksnius lemiančius pooperacinio gydymo trukmę. Metodai. Atlikta retrospektyvinė miokardo revaskuliarizaciją ir/ar vožtuvų korekcinį gydymą patyrusių pacientų ligos istorijų analizė. Surinkti 2019 m. sausio – birželio mėnesiais LSMUL KK Širdies, krūtinės ir kraujagyslių chirurgijos klinikoje planine tvarka gydytų pacientų dokumentiniai duomenys, perioperacinio gydymo trukmės, patirtos komplikacijos. Statistinė duomenų analizė atlikta naudojant Microsoft Excel 2016 ir IBM SPSS Statistics 26 programas. Tyrimo rezultatai. Priešoperacinė hospitalizacija truko ≤ 1 d.d. 29,2% tirtųjų (n = 87), > 1 d.d. 70,8% (n = 187) tirtųjų. Priešoperacinė hospitalizacija stacionare reikšmingai nepriklausė pagal EuroSCORE II įvertį, amžių, lytį, kairiojo skilvelio funkcinę būklę. Tarp pacientų, prieš operaciją hospitalizuotų ilgiau nei 1 d.d., papildomam ištyrimui dažniausiai prireikdavo vieno iš neinvazinių vaizdinių tyrimų (25,8%), rečiau prireikdavo invazinės radiologijos (14,5%) ar gydytojo specialisto konsultacijos iš kitos klinikos (5,4%), kitiems priešoperacinio papildomo ištyrimo neprireikė ir nepavyko nustatyti prailgėjusios priešoperacinės hospitalizacijos priežasties (54,3%). ITS po chirurginio gydymo ≤ 2 d. (A grupė) praleido 27,8% tirtųjų, > 2 d. (B grupė) – 72,2% tirtųjų. Pastarajai, ilgesnės hospitalizacijos ITS grupei, reikšmingai dažniau buvo priskirti tiriamieji su bloga KSIF (kai IF – 21-30%, B – n = 21, A – n = 6) nustatytą prieš operaciją, terminaliniu inkstų funkcijos nepakankamumu (B – n = 5, A – n = 0), atliekamu vožtuvų korekciniu gydymu (B – n = 173, A – n = 18) ir vožtuvų korekciniu su miokardo revaskuliarizacija vienos operacijos metu (B – n = 67, A – n = 20), patyrę pakartotine operacija (B – n = 40, A – n = 2), pooperacinėmis komplikacijomis: šokas (B – n = 70, A – n = 5), KFN (B – n = 62, A – n = 7), delyru (B – n = 45, A – n = 1), ŪIFN (B – n = 29, A – n = 2). Pooperacinė hospitalizacija ≤ 10 d. truko 39,1% tirtųjų (A grupė) , > 10 d. ligoninėje praleido 60,9% dalis tirtųjų (B grupė). Ilgesnės pooperacinės hospitalizacijos grupei, reikšmingai dažniau buvo priskirti tiriamieji su ilgesne operacijos trukme (B – 198,5 ± 47,0 min, A – 185,4 ± 41,8 min), aukštesniu EuroSCORE II įverčiu (B – 4,6 ± 9,3, A – 2,8 ± 4,2), priešoperacine hospitalizacija klinika Kardiologijos klinikoje (B – n = 45, A – n = 1), bloga KSIF ( kai IF – 21-30% B – n = 20, A – n = 7), terminaliniu inkstų funkcijos nepakankamumu (B – n = 4, A – n = 1), tęstiniu CNS deficitu (B – n = 5, A – n = 1), vienos operacijos metu atliekamu miokardo revaskuliarizacijos ir vožtuvų korekcijos gydymu (B – n = 63, A – n = 24), giliuoju krūtinės žaizdos drenavimu (B – n = 15, A – n = 0), pooperacinėmis komplikacijomis: pneumonija (B – n = 61, A – n = 8), KFN (B – n = 61, A – n = 8), insultu/CNS deficitu (B – n = 21, A – n = 0), ŪIFN(B – n = 26, A – n = 5), šoku (B – n = 63, A – n = 12), mediastinitu (B – n = 15, A – n = 0). 171 (41,0%) tirtųjų nepatyrė tyrime išskirtų pooperacinių komplikacijų arba jų nepavyko nustatyti retrospektyvinio tyrimo metu, vis dėlto 60,2% (n = 103) dalis turėjo ilgesnę nei 2 d. pooperacinę hospitalizaciją ITS ir ilgesne nei 10 d. pooperacinė hospitalizacija pasižymėjo 33.9% (n = 58) komplikacijų išvengusių pacientų dalis. Nustatytos reikšmingos teigiamos koreliacijos: tarp EuroSCORE II įverčių ir pooperacinės hospitalizacijos trukmės; tarp operacijos trukmės ir lovadienių skaičiaus po operacijos ITS ir pooperacinės hospitalizacijos trukmės; aortos užspaudimo trukmės ir šoko pasireiškimo dažnio; aortos užspaudimo trukmės ir insulto/CNS deficito pasireiškimo dažnio; ITS praleisto lovadienių skaičiaus ir pooperacinės delyro atsiradimo bei užfiksuoto delyro trukmės. Išvados. 1. Daugiau nei du trečdaliai tyrimo dalyvių operacinio gydymo laukė ilgiau nei vieną darbo dieną, daugiau nei pusei jų nebuvo nustatyta operacijos atidėjimo priežastis. 2. 72,2% po širdies operacijos pooperacinis gydymas intensyvios terapijos skyriuje truko > 2 dienas, šios grupės tiriamieji dažniau pasižymėjo aukštesniu EuroSCORE II įverčiu, bloga kairiojo skilvelio išstūmio frakcija. Nustatyta, kad ilga operacijos trukmė, bloga kairiojo skilvelio išstūmio frakcija, patiriamas vožtuvų korekcinis gydymas, pooperacinės komplikacijos: sepsis, pneumonija, kvėpavimo funkcijos nepakankamumas, insultas/CNS deficitas, delyras bei šokas reikšmingai ilgina pacientų gydymo trukmę intensyvios terapijos skyriuje po atviros širdies operacijos. 3. 60,9% tyrimo dalyvių po atviros širdies operacijos pooperacinis gydymas klinikoje truko > 10 dienų. Nustatyta, kad priešoperacinė hospitalizacija Kardiologijos klinikoje, aukštas EuroSCORE II įvertis, tęstinis CNS deficitas, ilga operacijos trukmė, gilusis operacinės žaizdos krūtinėje drenavimas, pakartotinė operacija, pooperacinės komplikacijos: sepsis, pneumonija, kvėpavimo sistemos nepakankamumas, insultas/CNS deficitas, delyras, krūtinplėvės komplikacijos, ūmus inkstų funkcijos pablogėjimas, mediastinitas bei šokas reikšmingai ilgina pacientų gydymo trukmę klinikoje po atviros širdies operacijos. Rekomendacijos. 1. Užtikrinti, kad pacientai planiniam operaciniam gydymui atvyktų jau turėdami atliktus visus reikiamus tyrimus. 2. Jei nėra komplikuotos eigos rekomenduojama pacientus dėl delyro išsivystymo rizikos į stacionaro skyrių iš intensyvios terapijos skyriaus grąžinti ne vėliau kaip 3 parą po operacijos. 3. Siekti kuo trumpesnės operacijos ir aortos užspaudimo trukmės, nes nustatytas tiesioginis ryšys – ilgėjant operacijos trukmei, ilgėja pooperacinio gydymo trukmė intensyvios terapijos skyriuje ir ligoninėje, ir netiesioginis ryšys – ilgėjant aortos užspaudimo trukmei, didėja pooperacinių komplikacijų, insulto ir šoko atsiradimo rizika.
Author. Emilija Želvytė. Title. Factors Determining the Duration of Perioperative Treatment of Elective Cardiac Surgery. Aim. To analyse the factors determining the duration of perioperative treatment of patients in the cardiac surgery department. Objectives. 1. To determine and analyse the factors of the preoperative period and their significance for the duration of the preoperative period in the inpatient ward. 2. To determine and evaluate the factors and their significance for the patient's duration of treatment in the intensive care unit after cardiac surgery. 3. To determine and evaluate the factors determining the duration of postoperative treatment. Methodology. A retrospective analysis of case histories of patients who underwent coronary artery bypass graft and/or heart-valve surgery. Documentary data, complications, and perioperative period duration of patients, who underwent surgery at the Department of Cardiovascular and Thoracic Surgery in the Hospital of Lithuanian University of Health Sciences Kaunas Clinics in 2019 January – July, were collected. Statistical analysis was performed using Microsoft Excel 2016 and IBM SPSS Statistics 26 programs. Results. Preoperative hospitalization lasted ≤ 1 W/D 29.2% of the subjects (n = 87) and > 1 W/D 70.8% (n = 187) of the subjects. Preoperative hospitalization was not significantly related to EuroSCORE II, age, gender, and left ventricular function. Those who were hospitalized longer than 1 W/D, usually required one of the non-invasive imaging (25.8%), less often – invasive radiology (14.5%), or consultation with a specialist from another clinic (5.4%) for additional examination. For those who had no additional preoperative examination, the cause of the prolonged preoperative hospitalization could not be determined (54.3%). 27.8% of the patients stayed in the ICU for ≤ 2 days (Group A) and 72.2% of the patients stayed in the ICU for > 2 days (Group B). In the group with a prolonged length of stay in the ICU patients were statistically more likely to have a poor LVEF (B – n = 21, A – n = 6) measured before surgery, a terminal renal failure (B – n = 5, A – n = 0), a valve surgery (B – n = 173, A – n = 18), valve correction and CABG in a single surgery (B – n = 67, A – n = 20), a reoperation (B – n = 40, A – n = 2), postoperative complications: shock (B – n = 70, A – n = 5), respiratory insufficiency (B – n = 62, A – n = 7), delirium (B - n = 45, A – n = 1), renal insufficiency (B – n = 29, A – n = 2). 39,1% of the patients stayed in the hospital for ≤ 10 days (Group A) and 60,9% of the patients stayed in the hospital for > 10 days (Group B). In the group with a prolonged length of stay in the hospital patients were statistically more likely to have a longer surgery time (B – 198.5 ± 47.0 min, A – 185.4 ± 41.8 min), higher EuroSCORE II score (B – 4.6 ± 9.3, A – 2.8 ± 4.2), preoperative hospitalization at the Cardiology Clinic (B – n = 45, A – n = 1), poor LVEF (B – n = 20, A – n = 7), terminal renal failure (B – n = 4, A – n = 1), previous stroke (B – n = 5, A - n = 1), valve correction and CABG in a single surgery (B – n = 63, A – n = 24), deep drainage of surgical wound in the chest (B – n = 15, A – n = 0), postoperative complications: pneumonia (B - n = 61, A – n = 8), respiratory insufficiency (B - n = 61, A – n = 8), stroke (B – n = 21, A – n = 0), renal insufficiency (B – n = 26, A – n = 5), shock (B - n = 63, A - n = 12), deep sternal wound infection (B – n = 15, A - n = 0). 171 of the subjects did not experience the postoperative complications or could not be detected in this retrospective study, however, 60.2% (n = 103) of them stayed in the ICU for > 2 days, and 33.9% (n = 58) of the patients without complications stayed in the hospital for ≤ 10 days. Significant positive correlations were found: between EuroSCORE II score and postoperative length of stay in the hospital; between the duration of the surgery and postoperative length of stay in the ICU and the hospital; between duration of aortic cross-clamping and frequency of shock; between duration of aortic cross-clamping and frequency of stroke; between the length of stay in the ICU and duration of delirium recorded, and frequency of delirium. Conclusions. 1. More than two-thirds of the patients in the study had been waiting for surgical treatment for more than one working day, and more than half of them were not found a reason to delay surgery. 2. 72.2% of patients after cardiac surgery, postoperative treatment in the intensive care unit lasted for > 2 days. Factors significantly prolonging the length of stay in the intensive care unit: prolonged duration of surgery, poor left ventricular ejection fraction, corrective valve therapy, postoperative complications (sepsis, pneumonia, respiratory failure, stroke, delirium, and shock). Factors significantly prolonging the length of stay in the hospital after surgery: preoperative hospitalization at the Cardiology Clinic, high EuroSCORE II score, previous stroke, long duration of surgery, deep drainage of surgical wound in the chest, reoperation, postoperative complications (sepsis, pneumonia, respiratory insufficiency, stroke, delirium, renal insufficiency, deep sternal wound infection and shock). Recommendations. 1. Ensure that patients arrive for elective surgery after all necessary examinations have been completed. 2. If the patient does not experience complications, it is recommended that patients be evicted from the intensive care unit the third day after surgery due to the risk of developing delirium. 3. Aim for the shortest duration of surgery and aortic cross-clamping time, as a correlation is established – the duration of surgery correlates with the postoperative length of stay in the intensive care unit and hospital, and increasing cross-clamping time – there is an increased risk of postoperative complications, a stroke, and a shock.
