Abstract
Tikslas: išsiaiškinti depresijos simptomų cukriniu diabetu ir/ar širdies ir kraujagyslių ligomis sergantiems pacientams įtaką gydymo būdų pasirinkimui. Uždaviniai:
- Įvertinti poliligotų pacientų ir sergančių tik viena chronine liga alternatyviosios bei papildomosios medicinos pasirinkimus ir pacientų nurodomas naudojimo priežastis.
- Įvertinti depresijos simptomų dažnumą tarp cukriniu diabetu ir/ar širdies ir kraujagyslių ligomis sergančiųjų.
- Įvertinti pacientų depresijos simptomų sąsajas su papildomų ir alternatyvių gydymo būdų naudojimu. Metodika: Tyrimas atliktas VšĮ Antakalnio poliklinikoje Šeimos gydytojų skyriuje. Tiriamieji - pacientai ≥ 18 m., sergantys cukriniu diabetu ir/ar širdies ir kraujagyslių ligomis. Apklausta 200 pacientų anonimine anketavimo apklausa. Duomenų statistinė analizė atlikta taikant „SPSS 22.0“ programą, taikytas Chi–kvadrato požymių homogeniškumo kriterijus. Stebėti skirtumai laikyti statistiškai reikšmingais, kai p < 0,05. Rezultatai: Iš viso 162 (81,0 proc.) respondentai nurodė besinaudojantys bent viena iš papildomosios ir alternatyviosios medicinos (PAM) paslaugų. 91,0 proc. apklaustųjų gydosi pas tradicinės medicinos (TM) gydytoją ir tik 9,0 proc. respondentų kartu gydosi ir pas vieną iš PAM specialistų. 75,0 proc. tiriamųjų nurodė, jog vartoja ar vartojo per pastaruosius 12 mėnesių nors vieną PM produktą. 37,5 proc. respondentų nurodė praktikuojantys vieną ar kelias savigydos praktikas. Poliligoti pacientai rinkosi daugiau PAM gydymo metodų nei respondentai, turintys tik vieną ligą. Poliligotų pacientų PAM gydymo metodų naudojimo vidurkis 1,3 (0-3; mediana 1,0), o turinčių tik vieną ligą – 0,9 (0-2; mediana 1,0) (p < 0,05). Taip pat poliligoti pacientai dažniau nei turintys tik vieną ligą (n=77, 46,4 proc. palyginus su n=8, 23,5 proc.) vartojo vitiminus/mineralus (p < 0,05). 114 (57,0 proc.) tiriamųjų turėjo depresijos požymių. Pacientai, turintys gretutinių ligų dažniau nei neturintys (n=98, 86,0 proc. palyginus su n=58, 67,4 proc.), turėjo depresijos požymių (p < 0,05). Respondentai su depresijos požymiais dažniau nei neturintys depresijos simptomų (n=98,0, 86,0 proc. palyginus su n=64, 74,4 proc.) naudojosi bent viena PAM gydymo paslauga (p < 0,05). Be to tiriamieji, kuriems galima įtarti depresiją dažniau nei neturintys depresijos simptomų (n=54, 55,1 proc. palyginus su n=25, 39,1 proc.) vartojo bent vieną PAM produktą (ypač vitaminus/mineralus bei homeopatinius vaistus) (p < 0,05). Išvados: 81,0 proc. respondentų nurodė naudojantys PAM. Dažniausiai pasirenkamos priemonės – vaistažolės, rečiau vitaminai/mineralai. Vaistažoles nurodė besirenkantys 63 proc. respondentų, o vitaminų/mineralų preparatus - 42,5 proc. Trys pagrindinės priežastys, dėl kurių respondentai vartojo vaistažoles buvo profilaktika, miegui gerinti, stiprinti nervų sistemą. Tuo tarpu vitaminų/mineralų – silpnas imunitetas, stiprinti nervų sistemą, kaulams. 57,0% pacientų buvo galima įtarti depresiją. Depresijos simptomų turintys pacientai dažniau naudoja PAM gydymo būdus bei produktus (ypač vitaminus/mineralus ir homeopatinius vaistus).
Purpose: elucidate the impact of depression symptoms among patients with diabetes mellitus and/or cardiovascular diseases on the choice of treatments. Objectives:
- evaluate the choice and the reasons of usage of the alternative and complementary medicine among patients with multimorbidity and patients who have one chronic disease.
- evaluate the rate symptoms of depression among patients with diabetes mellitus and /or cardiovascular diseases.
- evaluate the link between depression symptoms and the usage of complementary and alternative medicine. Methods: The study was carried out in the department of General Doctors, in the hospital Antakalnis. Subjects - patients ≥ 18 years with diabetes mellitus and/or cardiovascular diseases. It was interviewed 200 patients by anonymous questionnaire. The statistical analysis of the data was performed using the program of SPSS 22.0, the homogeneity criteria of the Chi-square attributes were applied. Observed differences were considered statistically significant when p < 0.05. Results: There was 81,0% of respondents who used service of complementary and alternative medicine (CAM). Most of the patients (91,0% ) went to the doctor of conventional medicine and only 9,0 proc. went to the doctor of conventional medicine and specialist of CAM. Three quarters (75,0 proc.) of respondents consumed at least one product of CAM through the last 12 months. There was 37,5% of respondents who did one or several self-care practice. Patients with multimorbidity choosed more methods CAM’s treatment than respondents with one chronic disease. Also patients with multimorbidity used more vitamins/minerals than patients with one chronic disease (n=77, 46,4 proc. versus n=8, 23,5 proc.) (p < 0,05). 57,0% of patients had the depression symptoms. Patients who had comorbidities diseases more often had depression symptoms than patients without these diseases (n=98, 86,0 proc. versus n=58, 67,4 proc.) (p < 0,05). Respondents who had depression symptoms used more at least one method CAM’s treatment than patients without depression symptoms (n=98,0, 86,0 proc. versus n=64, 74,4 proc.) (p<0,05). Also patients with the depression symptoms used more at least one product of CAM (especially vitamins/minerals) than patients without depression symptoms (n=54, 55,1 proc. versus n=25, 39,1 proc.) (p<0,05). Conclusions: 81,0% of respondents used CAM. The most common options were medical herbs, less often was vitamins/minerals. The medical herbs used 63% of respondents and the vitamins/minerals – 42,5%. Three main reasons why respndents used these herbs were prophylaxis, to improve the sleep and strengthen the nervous system. Meanwhile the main reasons of usage the vitamins/minerals were weak immune system, strengthen the nervous system and for the bones. 57,0% of patients had the symptoms of depression. Patients who have the sypmtoms of depression used more CAM treatment and products (especially vitamins/minerals and homeopathy medicine).
