Poor oral health in the older population is a condition which needs to be looked into seriously. Dry mouth can have a devastating impact on an individual's day-to-day life, especially in the elderly population since they are more likely to suffer from this condition due to a number of factors discussed in this literature review. There is in-depth discussion in this literature review of the relationship between xerostomia, salivary gland hypofunction and medication use, along with a comparison of two versions of the xerostomia inventory, in order to identify which is more effective and efficient, followed by treatment options for xerostomia. Further, xerostomia is associated with dental caries and periodontal disease. Periodontal and dental caries may cause tooth loss if left untreated, resulting in edentulism. Conversely, some oral diseases are associated with systemic illnesses. Additionally, this thesis examines the prevalence of oral lesions with regard to age, gender, medication use, systemic diseases and smoking. In regards to their health, the elderly face many challenges on a daily basis including cognitive, physical, emotional, and social issues. The elderly who cannot maintain independent living are often supported by domiciliary care services or admitted to nursing homes. Preventive and curative dental care is necessary for them continuously. The entanglement of oral health status, oral mucosal lesions, systemic diseases, and the general use of multiple medications make older people more vulnerable to oral problems than younger age groups, even more so in those who are cognitively impaired. Aim: The aim of this thesis is to present a literature review assessing the oral health issues of care home geriatric patients and older patients living alone and discussing possible recommendations, guidelines and solutions to prevent a potential new geriatric syndrome developing. Objectives : 1. To evaluate oral and dental problems of older adults, 2.To evaluate the association between xerostomia, hyposalivation and medication use in the elderly population, 3.To assess the diagnostic suitability of a xerostomia questionnaire, the shortening of the xerostomia inventory and the types of treatment for xerostomia, 4.To review the oral health care guideline for older people living alone and in care homes, 5.To assess supervised versus non-supervised implementation of an oral health care guideline in care homes. For this literature review, articles were retrieved from PubMed, Google Scholar, Australian dental journal, The National Institute for Health and Care Excellence, Commissioning better oral health for vulnerable older people-public health England. The search strategy was based on keywords and medical subject heading terms (MESH) The main keywords used during the search were “Geriatric syndrome”, “Oral health”, “Tooth loss”, “supervised and non-supervised oral care”, “frailty”, “polypharmacy”, “geriatric assessment”, “older people”, “literature review”, “Salivary gland hypofunction”, “poor oral health in elderly”, “oral health in elderly”, “care dependency”, “disability”, “multimorbidity”, “geriatric dentistry”, “oral health and general health”. Conclusion : Older adults are at increased risk for oral diseases such as periodontal disease, dental caries, and precancerous and cancerous lesions due to systemic diseases and medications. For dentists to meet the increasing need for elderly dental care, they should update their knowledge on geriatric dentistry. Periodic oral examinations for detection of precancerous and cancerous lesions are very critical and essential, especially among the elderly population, smokers and with patients who use dentures. Dry mouth is common in the elderly and has a significant impact on the daily lives of affected people. Xerostomia is difficult to treat and requires an interdisciplinary approach. Xerostomia is more prevalent in women in the night. Women also reported higher progression than men. In the comparison between supervised and non-supervised implementation of an oral health care guideline in care homes, it is seen that oral hygiene has enhanced and improved by the supervised implementation intervention. Denture plaque benefited the most, followed by tongue plaque and dental. Based on all the data mentioned above, one can definitely say that in this modern time and age, with an increasing number of elderly individuals living a longer life than before, poor oral health in the geriatric population is increasing and oral health care should be taken seriously by individuals themselves and their family members, followed by care home staff, nurses and physicians
Prasta vyresnio amžiaus žmonių burnos sveikata yra būklė, kurią reikia rimtai išnagrinėti. Burnos džiūvimas gali turėti pražūtingos įtakos kasdieniam žmogaus gyvenimui, ypač vyresnio amžiaus žmonėms, nes jie dažniau kenčia nuo šios būklės dėl daugelio šioje literatūros apžvalgoje aptartų veiksnių. Dėl susipainiojusios burnos sveikatos būklės, burnos gleivinės pažeidimų, sisteminių ligų ir bendro kelių vaistų vartojimo vyresnio amžiaus žmonės yra labiau pažeidžiami burnos problemų nei jaunesnio amžiaus žmonės, ypač tiems, kurie turi pažinimo sutrikimų. Tikslas: Šio baigiamojo darbo tikslas – pateikti literatūros apžvalgą, įvertinančią globos namų geriatrijos pacientų ir vyresnio amžiaus pacientų, gyvenančių vieni, burnos sveikatos problemas ir aptariamos galimos rekomendacijos, gairės ir sprendimai, kaip užkirsti kelią galimo naujo geriatrinio sindromo išsivystymui. Uždaviniai: 1. Įvertinti vyresnio amžiaus žmonių burnos ir dantų problemas, 2. Įvertinti ryšį tarp kserostomijos, hiposalivacijos ir vaistų vartojimo vyresnio amžiaus populiacijoje. 3. Įvertinti kserostomijos klausimyno diagnostinį tinkamumą, kserostomijų inventoriaus sutrumpinimą. ir kserostomijos gydymo būdus. 4. Apžvelgti burnos sveikatos priežiūros gaires senyvo amžiaus žmonėms, gyvenantiems vieni ir globos namuose, 5. Įvertinti burnos sveikatos priežiūros gairių globos namuose prižiūrėtą ir neprižiūrimą įgyvendinimą.
