Darbo tikslas: Nustatyti TNF-863A/C (rs1800630), TNF-308A/G (rs1800629) ir TNF-238A/G (rs361525) sąsajas su hipofizės adenoma ir jos klinikinėmis išraiškomis. Darbo uždaviniai: 1. Nustatyti TNF-863A/C (rs1800630), TNF-308A/G (rs1800629) ir TNF-238A/G (rs361525) genotipų ir alelių pasiskirstymą, pacientams, sergantiems hipofizės adenoma bei kontrolinės grupės asmenims. 2. Nustatyti TNF-863A/C (rs1800630), TNF-308A/G (rs1800629) ir TNF-238A/G (rs361525) polimorfizmų įtaką hipofizės adenomos pasireiškimui. 3. Nustatyti TNF-863A/C (rs1800630), TNF-308A/G (rs1800629) ir TNF-238A/G (rs361525) sąsajas su hipofizės adenoma, atsižvelgiant į tiriamųjų lytį. Metodika: Tyrime dalyvavo HA sergantys 109 pacientai: 67 moterys ir 42 vyrai bei 218 kontrolinės grupės dalyvių – 152 moterys ir 66 vyrai. LSMU Oftalmologijos laboratorijoje tyrimai buvo atliekami taikant DNR išskyrimo metodiką ir druskų nusodinimo metodą. Statistinė analizė atlikta naudojant IBP SPSS 29.0 programas, statistinis reikšmingumas vertintas Chi kvadrato kriterijumi, atlikta dvinarė logistinė regresija, statistinio reikšmingumo lygmuo (p mažiau 0,05). Rezultatai: TNF-308A/G (rs1800629) polimorfizmo genotipas AG yra statistiškai reikšmingai dažnesnis hipofizės adenoma sergančių vyrų grupėje, lyginant su kontroline grupe (31,0 proc. vs. 15,2 proc., p=0,023). Statistiškai reikšmingai dažnesnis A alelio pasireiškimas taip pat nustatytas hipofizės adenoma sergančių vyrų grupėje, lyginant su kontroline grupe (20,2 proc. vs. 9,0 proc., p=0,006). HA sergančių ir sveikų TNF-863A/C (rs1800630) ir TNF-238A/G (rs361525) genotipų ir alelių dažnis statistiškai reikšmingai (p daugiau 0,05) nesiskyrė nei moterų, nei vyrų grupėse. Dvinarės logistinės regresijos rezultatai rodo, kad vyrų grupėje TNF-308A/G (rs1800629) AG genotipas, lyginant su AA genotipu, mažina HA pasireiškimo galimybę 3,7 karto (GS=0,2696; 95 proc. PI: 1,049-6,928; p=0,039), AG+GG genotipai, lyginant su AA genotipu šią galimybę didina 3,1 karto (GS=3,111; 95 proc. PI: 1,237-7,827; p=0,016. Nustatyta, kad kiekvienas A alelis didina HA pasireiškimo galimybę 3,1 karto (GS=3,123; 95 proc. PI: 1,316-7,413; p=0,010). Atlikta TNF-863A/C (rs1800630) ir TNF-238A/G (rs361525) genų polimorfizmų dvinarė logistinė regresija parodė, kad nei moterų, nei vyrų grupėse tarp pacientų, kuriems nustatyta HA, ir referentinės grupės dalyvių, statistiškai reikšmingi (p daugiau 0,05) skirtumai nenustatyti Išvados: Įvertinus HA sergančių ir kontrolinės grupės asmenų TNF-863A/C (rs1800630), TNF-308A/G (rs1800629) ir TNF-238A/G (rs361525) genotipų ir alelių pasiskirstymą, statistiškai reikšmingų skirtumų nenustatėme. TNF-863A/C (rs1800630), TNF-308A/G (rs1800629) ir TNF-238A/G (rs361525) polimorfizmų sąsajų su HA adenomos pasireiškimu nenustatyta. Nustatyta, kad TNF-308A/G (rs1800629) polimorfizmo genotipas AG ir A alelis statistiškai reikšmingai dažnesni HA sergančių vyrų grupėje nei kontrolinėje grupėje. Vyrų grupėje TNF-308A/G (rs1800629) AG genotipas, lyginant su AA genotipu, mažina HA pasireiškimo galimybę 3,7 karto, AG+GG genotipai, lyginant su AA genotipu šią galimybę didina 3,1 karto, o kiekvienas A alelis didina HA pasireiškimo galimybę 3,1 karto.

Title: Associations of TNF-863A/C (rs1800630), TNF-308A/G (rs1800629) and TNF-238A/G (rs361525) with pituitary adenomas and their clinical expression. Aim: To determine the associations of TNF-863A/C (rs1800630), TNF-308A/G (rs1800629) and TNF 238A/G (rs361525) with pituitary adenomas and their clinical manifestations. Objective: 1. To determine the distribution of genotypes and alleles of TNF-863A/C (rs1800630), TNF 308A/G (rs1800629) and TNF-238A/G (rs361525) in patients with pituitary adenoma and in the control group. 2. To determine the influence of TNF-863A/C (rs1800630), TNF-308A/G (rs1800629), and TNF 238A/G (rs361525) polymorphisms on the occurrence of pituitary adenomas. 3. To determine the influence of TNF-863A/C (rs1800630), TNF-308A/G (rs1800629), and TNF-238A/G (rs361525) polymorphisms on the occurrence of pituitary adenomas in relation to sex. Methods: The study included 109 patients with HA: 67 women and 42 men and 218 control participants
- 152 women and 66 men. The studies were performed in the Ophthalmology Laboratory of LUHS using the DNA extraction method and the salt precipitation method. Statistical analysis was performed using the program IBP SPSS 29.0, statistical significance was evaluated using the chi-square criterion, binary logistic regression was performed, statistical significance level (p less than 0.05). Results: TNF-308A/G (rs1800629) polymorphism AG genotype and A allele were statistically significantly more frequent in the group of men with pituitary adenoma compared to the control group (31.0% vs. 15.2%, p=0.023; 20.2% vs. 9.0%, p=0.006; respectively). The frequency of genotypes and alleles of TNF-863A/C (rs1800630) and TNF-238A/G (rs361525) was not statistically significantly different between PA patients and the control group in either the female or male group (p more than 0.05). The results of binary logistic regression show that in the male group, the TNF-308A/G genotype (rs1800629) AG reduced the incidence of PA by 3.7-fold compared with the AA genotype (OR =0.2696; 95% CI: 1.049-6.928; p =0.039), AG +GG genotype, compared to AA genotype, increased the probability of occurrence of PA by 3.1-fold (OR =3.111; 95% CI: 1.237-7.827; p=0.016). Each A allele was found to increase the probability of occurrence of PA by 3.1-fold (OR =3.123; 95% CI: 1.316-7.413; p=0.010). Binary logistic regression of the TNF-863A/C (rs1800630) and TNF-238A/G (rs361525) gene polymorphisms did not reveal statistically significant (p more than 0.05) results between patients diagnosed with PA and participants in the control group. Conclusions: The genotypes and allele distributions of TNF-863A/C (rs1800630) and TNF-238A/G (rs361525) did not differ between the PA group and the control group. Binary logistic regression also did not yield statistically significant results between the groups. The genotype of TNF-308A/G (rs1800629) polymorphism AG and the A allele were statistically significantly more frequent in the group of men with PA than in the control group of men. Binary logistic regression showed that the TNF 308A/G (rs1800629) AG genotype compared with the AA genotype decreased the probability of occurrence of PA by 3.7-fold, the AG +GG genotype compared with the AA genotype increased the probability by 3.1-fold, and each A allele increased the probability of occurrence of PA by 3.1-fold.
