Uždaviniai: 1. Įvertinti osteosintezės, atliktos intramedulinėmis elastinėmis stabiliomis vinimis, kokybę vaikams, patyrusiems apatinės galūnės ilgųjų kaulų diafizių lūžius, priklausomai nuo pasirinktų implantų parametrų. 2. Įvertinti osteosintezės, atliktos intramedulinėmis elastinėmis stabiliomis vinimis, kokybę vaikams, patyrusiems apatinių galūnių ilgųjų kaulų diafizių lūžius, priklausomai nuo pasirinktos operacinės technikos. 3. Nustatyti galimas komplikacijas po osteosintezės, atliktos intramedulinėmis elastinėmis stabiliomis vinimis, vaikams, patyrusiems apatinių galūnių ilgųjų kaulų diafizių lūžimus. 4. Įvertinti uždaros ir atviros repozicijos įtaką osteosintezės, atliktos intramedulinėmis elastinėmis stabiliomis vinimis, kokybei vaikams, patyrusiems apatinių galūnių ilgųjų kaulų diafizių lūžimus. Metodai: tyrimo tipas yra retrospektyvinis. Duomenys rinkti ligoninės informacinėje sistemoje ir centriniame medicininių vaizdų archyve MedDream nagrinėjant pacientų ligos istorijas, operacinius protokolus, po ir priešoperacinius rentgenologinius vaizdus, ambulatorinius apsilankymus. Ieškota informacijos apie naudotus implantus, operacinę techniką, repozicijos būdą ir nustatytas komplikacijas. Rezultatai: Ištirti 56 pacientai, kuriems atlikta OIESV. Vidutinis pacientų amžius buvo 10,8 ± 3,6 metų. Jauniausiam pacientui buvo 3, o vyriausiam 17 metų. Lyginant konstrukcijos nestabilumo komplikacijų pasireiškimą tarp titaninių ir nerūdijančio plieno implantų šlaunikaulyje atitinkamai nustatyti 35% ir 100% komplikacijų dažniai. Statistiškai reikšmingų skirtumų nerasta χ 2 = 1,706; Ils=1, fišerio p=0,381. Lyginant implantacijos laikotarpį tarp titaninių ir nerūdijančio plieno implantų blauzdikaulyje nustatyti atitinkamai 7,69 ir 9 mėn. vidutiniai rangai. Statistiškai reikšmingų skirtumų nerasta t=0,681; p=0,501. Lyginant konstrukcijos nestabilumo komplikacijų pasireiškimą tarp titaninių ir nerūdijančio plieno implantų blauzdikaulyje atitinkamai nustatyti 15% ir 67% komplikacijų dažniai. Statistiškai reikšmingų skirtumų nerasta χ 2 = 4,659; Ils=1, fišerio p=0,090. Lyginant osteosintezių kokybę šlaunikaulyje prikslausomai nuo implanto skersmens pasirinkimo vidutiniai elastinių vinių pašalinimo rangai osteosintezėse, atliktose per mažais ir reikiamo dydžio implantais, siekė atitinkamai 9,81 ir 7,19 mėn. Bet statistiškai reikšmingo skirtumo nerasta; U= 21,500; p=0,246. Lyginant konstrukcijos nestabilumo sukeliamų komplikacijų dažnius šlaunikaulyje, tinkamo ir netinkamo diametro grupėse nustatyti atitinkamai 27% ir 50% komplikacijų dažniai. Tačiau statistiškai reikšmingų skirtumų nerasta χ 2 = 1,147, Ils=1, fišerio p=0,387. Lyginant osteosintezių kokybę priklausomai nuo implantų skersmens blauzdikaulyje, vidutiniai rangai tinkamo ir netinkamo skersmens pasirinkimo grupėse siekė atitinkamai 7,75 ir 7,7 mėn. Statistiškai reikšmingo skirtumo nerasta t=0,04; p=0,969. Konstrukcijos stabilumo komplikacijų dažniai blauzdikaulyje tinkamo ir netinkamo diametro grupėse buvo atitinkamai po 19% ir 20%. Statistiškai reikšmingų skirtumų nerasta χ 2 = 0,009; Ils=1, fišerio p=1,000. Vertinant implanto ilgio palikimo rekomendacijų nesilaikymą, komplikacijų dažniai tarp 1-2 cm ir > 2 cm grupių buvo atitinkamai 20% ir 23%. Statistiškai reikšmingų skirtumų nerasta χ 2 = 0,060; Ils=1, fišerio p=1,000. Vertinant operacinę techniką distaliniams šlaunikaulio lūžimams, rezultatai prastesni retrogradinės bipoliarinės osteosintezės atvejyje. Po šlaunikaulio operacijų komplikacijos nustatytos 67% pacientų. Dažniausiai, 29% atvejų pacientai susidūrė su nepakankama kaulinių fragmentų repozicija ir netaisyklinga jų padėtimi. Kojų ilgio skirtumai rasti 14% pacientų. Sulėtėjusi kaulinių fragmentų konsolidacija nustatyta 10% tirtųjų. Sąnarių kontraktūros implantų įvedimo vietoje nustatytos 19% pacientų. Minkštųjų audinių dirginimas rastas 14% tirtųjų. 10% pacientų odos pažeidimai buvo sukelti implantų migracijos. Paviršinė minkštųjų audinių infekcija nustatyta 5%. Po blauzdikaulio OIESV komplikacijos nustatytos 33% atvejų. Dažniausiai pacientai skundėsi komplikacijomis dėl per ilgų implantų galų. Sąnarių kontraktūros nustatytos 14%, o minkštųjų audinių dirginimas 3% klinikinių atvejų. Netaisyklinga kaulinių fragmentų padėtis diagnozuota 8% atvejų. Sulėtėjusi konsolidacija rasta 6% atvejų. Kaulinių fragmentų nesugijimas diagnozuotas 3% blauzdikaulio lūžių. Kojų ilgio skirtumai rasti 6%. Vertinant repozicijos įtaką, vidutinis implanto pašalinimo rangas atviros ir uždaros repozicijos grupėse atitinkamai siekė 8,5 ir 7,96 mėn. Statistiškai reikšmingo skirtumo nerasta U=6,500; p=0,904. Vertinant galimą komplikacijų dažnį atviros repozicijos grupėje, jokių gijimo komplikacijų nenustatyta. Atviros repozicijos grupėje tokių pacientų buvo 10%. Statistiški reikšmingų skirtumų nerasta χ 2 = 0,527; Ils=1; fišerio p=1,000. Išvados: 1. Per mažo skersmens implantų pasirinkimas, ilgesnių nei 2 cm galų palikimas ir implanto medžiagos tipas statistiškai reikšmingos įtakos osteosintezės kokybei neturėjo. 2. Distaliniam šlaunikaulio lūžimui gydyti visuomet turėtų būti pasirenkama antegradinė unipoliarinė operacinė technika. 3. Po šlaunikaulio OIESV dažniausiai nustatomos netaisyklingos kaulinių fragmentų padėties komplikacijos, o po blauzdikaulio OIESV dažniausiai nustatomos atsikišusių implantų galų sukeliamos komplikacijos. 4. Kaulinių fragmentų repozicijos būdas statistiškai reikšmingos įtakos osteosintezės kokybei neturėjo. Rekomendacijos: 1. Renkantis implantą siekti, jog jo skersmuo būtų lygus 40% intramedulinio kanalo diametro. Ypač tai svarbu atliekant šlaunikaulio osteosintezes. Stabili ir tvirta konstrukcija padeda išvengti kaulinių fragmentų poslinkių, kuriuos sukelia stiprių aplinkinių raumenų trauka. 2. Prieš užsiuvant įvedimo taško žaizdą, implantai turėtų būti patikimai įkalami į metafizę naudojant specialų smogtuvą, siekiant išvengti migracijos. Likę išlindę galai turėtų būti sutrumpinami iki 1-2 cm ir prišliejami prie metafizės. 3. Atliekant OIESV implantų įvedimo taškus reikėtų formuoti kuo toliau nuo lūžio vietos. Proksimaliniams ir vidurinio trečdalio šlaunikaulio lūžimams reikia rinktis retrogradinę bipoliarinę techniką. O distaliniam trečdaliui – antegradinę unipoliarinę techniką. Atliekant blauzdikaulio operaciją viduriniam ir distaliniam trečdaliui fiksuoti reikėtų rinktis antegradinę bipoliarinę techniką. Proksimalinio tračdalio lūžimams reikėtų palikti retrogradinę bipoliarinę operacinę techniką. 4. Nors tarp atviros ir uždaros repozicijos kokybiniai skirtumai nedideli, vis tiek reikėtų laikytis minimaliai invazinės chirurgijos principų ir stengtis atlikti repoziciją uždaru būdu.

Aim: To evaluate the factors that may have contributed to the quality of osteosynthesis, performed with intramedullary elastic stable nails in children who underwent fractures of the long bones in lower extremity Objectives: 1. To evaluate the quality of osteosynthesis performed with intramedullary elastic stable nails in children who have undergone fractures of the long bone’s diaphysis in lower extremity, depending on the selected implant parameters. 2. To evaluate the quality of osteosynthesis performed with intramedullary elastic stable nails in children who have undergone fractures of the long bone’s diaphysis in lower extremity, depending on the chosen surgical technique. 3. To identify possible complications after osteosynthesis performed with intramedullary elastic stable nails in children who have undergone fractures of the long bone’s diaphysis in lower extremity. 4. To evaluate the influence of closed and open reduction on the quality of osteosynthesis performed with intramedullary elastic stable nails in children who underwent diaphyseal fractures in lower extremity. Methods: This type of study is retrospective. Data was collected in the hospital’s information system and in the central medical image archive ‘MedDream’. This encompassed examining patients' medical histories, surgical protocols, post-operative and pre-operative radiological images, and outpatient visits. Data regarding implant parameters, surgical techniques, methods of reduction, and identified complications were collected. Results: A total of 56 patients were studied. The mean age of the patients were 10.8 ± 3.6 years. The youngest patient was 3 years old and the oldest was 17 years old. When comparing the incidence of structural instability, complications between titanium and stainless steel implants in the femur had incidence rates that were 35% and 100%, respectively. No statistical significant differences were found χ2 = 1.706; Ils = 1, fisher’s p = 0.381. When comparing the implantation period between titanium and stainless steel implants in the tibia, the average ranks were 7.69 and 9 months, respectively. No statistical significant differences were found, t = 0.681; p = 0.501. When comparing the incidence of structural instability, complications between titanium and stainless steel implants in the tibia concluded that the incidence rates were 15% and 67%, respectively. No statistical significant differences were found χ 2 = 4,659; Ils = 1, fisher’s p = 0.090. When comparing the quality of osteosynthesis in the femur, (depending on the choice of implant diameter) the average removal rates of elastic nails in osteosynthesis performed with implants that were too small and of the required size were 9.81 and 7.19 months, respectively. But no statistical significant difference was found; U = 21,500; p = 0.246. The frequencies of complications due to structural instability in the femur in the right and wrong diameter groups were found to be 27% and 50%, respectively. However, no statistical significant differences were found χ 2 = 1.147, Ils = 1, fisher’s p = 0.387. Comparing the quality of osteosynthesis, (depending on the diameter of the implants in the tibia) the mean ranked in the right and wrong diameter selection groups were 7.75 and 7.7 months, respectively. No statistical significant difference was found at t = 0.04; p = 0.969. The rates of structural stability complications in the tibia in the right and wrong diameter groups were 19% and 20%, respectively. No statistical significant differences were found χ 2 = 0.009; Ils = 1, fisher’s p = 1,000. The rates of complications between the 1-2 cm and > 2 cm groups were 20% and 23%, respectively, when assessing non-compliance with implant length retention recommendations. No statistical significant differences were found χ 2 = 0.060; Ils = 1, fisher’s p = 1,000. When evaluating the surgical technique for distal femoral fractures, the results were worse in the case of retrograde bipolar surgical technique. Complications after femoral surgery were reported in 67% of patients. In most cases, 29% of patients experienced insufficient repositioning of bone fragments and malunion. Leg length discrepancy was found in 14% of patients. Slower consolidation of bone fragments were found in 10% of the subjects. Joint contractures at the implant site were identified in 19% of patients. Soft tissue irritation was found in 14% of subjects. Skin lesions were caused by implant migration in 10% of patients. Superficial soft tissue infection was detected in 5%. Complications in the tibia were reported in 33% of cases. Patients most often complained of complications associated with too long of implant ends. Joint contractures were observed in 14% and soft tissue irritation in 3% of clinical cases. Irregular positions of bone fragments were diagnosed in 8% of cases. Slower consolidation was found in 6% of cases. Failed bone fragment healing was diagnosed in 3% of tibial fractures. Leg length discrepancy was found in 6%. Assessing the effect of reduction, the mean implant removal rank in the open and closed reduction groups were 8.5 and 7.96 months, respectively. No statistical significant differences were found, U = 6,500; p = 0.904. No healing complications were observed in the open reduction group. There were 10% of such patients in the closed reduction group. No statistical significant differences were found χ 2 = 0.527; Ils = 1; fisher’s p = 1,000. Conclusions: 1. The choice of implants that were too small in diameter, leaving an end longer than 2 cm, and the type of implant material did not statistically have a significant effect on the quality of osteosynthesis. 2. The antegrade unipolar surgical technique should always be used to treat distal femoral fractures. 3. Malunion is the most common complication in the femur, while complications caused by protruding implant ends are most common in the tibia. 4. The method of reduction of bone fragments did not statistically have a significant effect on the quality of osteosynthesis. Recommendations: 1. When selecting an implant, aim for its diameter. The 40% rule should be respected. This is especially important during femoral osteosynthesis. This stable and strong construction helps prevent the displacement of bone fragments caused by the traction of strong surrounding muscles. 2. Before suturing the insertion point wound, the implants should be securely hammered into the metaphysis using a special impactor to prevent migration. The remaining protruding ends should be shortened to 1-2 cm. and left near metaphysis. 3. Implant insertion points should be formed as far away from the fracture site as possible. Proximal and middle-third fractures require retrograde bipolar technique. As for the distal third, the antegrade unipolar technique should be used. An antegrade bipolar technique should be used to fix the middle and distal thirds fractures of tibia. Retrograde bipolar surgical technique should be left for proximal third fractures. 4. Although the quality differences between open and closed reduction are small, the principles of minimally invasive surgery should still be followed and a closed reduction should be done.
