This study examined how far current national guidelines and good practices address these domains, and where important gaps remain. The objective was to identify unmet needs in national guidelines and good practices on self-care, education and training, and digital tools for people with dementia and their caregivers.
6 1 Beyond Algorithmic Oversight: Internal Morality of Medicine and Meaningful Human Control in AI-Assisted CareItem type:Publication, journal article[2026][S1][M004,H001][21]; ; ; Healthcare, 2026-06-10, vol. 14, no. 12, p. 1-21Background/Objectives: Artificial intelligence reshapes clinical practice, and its effect on the clinician–patient relationship requires reconsideration of the frameworks that have shaped modern medical ethics. When clinicians delegate expertise to algorithms they cannot verify, it becomes unclear who bears clinical responsibility. Methods: This article applies a theoretically grounded normative approach to explore the ethical conditions under which artificial intelligence can be integrated into clinical practice without compromising the moral foundations of medicine. The analysis is primarily based on Pellegrino and Thomasma’s concept of the internal morality of medicine and the clinician’s act of profession. It further draws on Kantian ethics of human dignity, Levinasian relational ethics, virtue ethics, and Vallor’s concept of technomoral wisdom. Results: AI systems do not satisfy the conditions under which moral responsibility can be ascribed to them. Clinical moral agency lies in the capacity to bear three distinct responsibilities—epistemic, relational, and phronetic—none of which can be fulfilled by AI. The implementation of AI in healthcare, therefore, must occur strictly under the condition of Meaningful Human Control, rather than as a technical function of human oversight over algorithmic outputs. To ensure that MHC can function as an effective and ethically grounded safeguard, we propose five normative requirements: primacy of clinical judgement, prohibition of forced automation, traceability and explainability, transparency towards patients, and retaining clinical authority. Dialogue between clinicians and patients should remain the foundation of clinical decision-making. The proposed normative requirements aim to preserve the internal morality of medicine in a form that harmoniously combines both technological progress and established medical ethics.

Jų paplitimas, sudėtinga etiologija ir ilgalaikės pasekmės paciento gyvenimo kokybei lemia būtinybę nuosekliai tobulinti šių ligų valdymą, ypač pirminės sveikatos priežiūros grandyje. Šiame leidinyje sistemiškai nagrinėjami pagrindiniai lėtinių ligų valdymo aspektai, apimantys epidemiologiją, patogenezę, klinikinę praktiką, sveikatos priežiūros organizavimą ir pacientų priežiūros tęstinumą. Analizuojamos tiek konkrečių ligų bei jų rizikos veiksnių problemos, tiek kompleksinės situacijos, susijusios su dauginėmis lėtinėmis ligomis, pacientų patirties valdymu, gyvensenos medicina, psichikos sveikata bei ilgalaike priežiūra. Akcentuojama integruotos, į pacientą orientuotos priežiūros svarba, tarpdisciplininio bendradarbiavimo reikšmė bei įvairių sveikatos priežiūros specialistų indėlis į visapusišką paciento gerovę. Šio vadovėlio turinys atspindi šiuolaikinės medicinos kryptį - pereiti nuo siauro, į ligą orientuoto požiūrio prie holistinio, žmogų vertinančio kaip visumą. Todėl ypatingas dėmesys skiriamas ne tik gydymui, bet ir prevencijai, sveikatos stiprinimui, paciento lūkesčių valdymui bei efektyviai komunikacijai tarp paciento ir sveikatos priežiūros specialistų. Šis universitetinis vadovėlis skiriamas medicinos, slaugos ir kitų sveikatos mokslų studentams, rezidentams bei praktikuojantiems pirminės sveikatos priežiūros specialistams. Demencija ir kitos neurodegeneracinės ligos yra viena pagrindinių ne- galios ir priklausomybės priežasčių vyresniame amžiuje bei reikšminga visuomenės sveikatos problema Europoje ir Lietuvoje. Projektas, įgyvendinamas kaip tarptauti- nės iniciatyvos Joint Actions Addressing Dementia and Health (JADE Health) dalis, siekia sistemiškai stiprinti ankstyvą diagnostiką, optimizuoti paciento kelią po dia- gnozės ir diegti inovatyvius, į asmenį orientuotus priežiūros modelius. Metodai. Pirmasis projektas orientuotas į ankstyvą neurokognityvinių sutrikimų atpažinimą pirminėje sveikatos priežiūroje. Bus vertinamas standartizuotų atrankos instrumentų (MoCA, GPCog) diagnostinis tikslumas rizikos grupėse bei rengiamos rekomendacijos šeimos gydytojams. Tai prisidės prie savalaikės diagnostikos gerini- mo ir efektyvesnio pacientų nukreipimo specializuotai pagalbai. Antrasis projektas skirtas demencijos sindromu sergančių asmenų ir jų globėjų integruotam priežiūros keliui po diagnozės nustatymo. Bus diegiamas struktūruo- tas stebėsenos modelis, organizuojami globėjų mokymai, vertinami gyvenimo ko- kybės, pasitenkinimo paslaugomis ir priežiūros naštos pokyčiai. Remiantis gautais duomenimis, bus parengtos įrodymais pagrįstos rekomendacijos paslaugų plėtrai Lietuvoje. Trečiasis projektas nagrinėja skaitmeninių technologijų taikymą ankstyvose demencijos stadijose. Bus vertinamas nuotolinės fiziologinių rodiklių stebėsenos (kraujospūdis, fizinis aktyvumas, miegas ir kt.) priimtimumas, klinikinė nauda ir po- veikis ligos valdymui bei globėjų naštai. Laukiami rezultatai. Projektų rezultatai turės mokslinę vertę diagnostinių stra- tegijų, integruotos priežiūros modelių ir skaitmeninės sveikatos srityse bei praktinę reikšmę kuriant tvarią, į asmenį orientuotą demencijos priežiūros sistemą Lietuvoje. Ateityje numatoma plėsti modelių taikymą nacionaliniu mastu ir integruoti juos į sveikatos politikos sprendimus.

Are those features in some unique ways related to women’s socio-cultural position(s) or roles? How are they catalyzed in/by women? We argue that women are open, tend to take risks, are able to reflect, and experience excitement in life. The ongoing project ‘Inclusive Wellbeing Through Arts and Culture in the Baltics (WITAC, 2024-2026)’ contributes to debates about risks, novelty, and innovation: authors in WITAC assume the roles of experts, researchers, lecturers, mentors, and coordinators, which require tolerance for risk and uncertainty. The study aims to highlight the unique role of women in the project, taking into account the following specific objectives: a) introduce the ideas and main activities of WITAC project; b) reflect on women’s engagement and their role(s) in the project; c) discuss, how women build inclusiveness and sustainability in the ecosystem “human – culture”; and d) analyze the sociocultural phenomenon of ‘vulnerability’ in the context of innovations. Through a methodological approach based on observation and (self)reflection, we find that the WITAC project represents a strategic partnership connecting the cultural and health sectors across Baltic higher education institutions (HEIs). This interdisciplinary model seeks to strengthen HEIs’ role in promoting equity and inclusion through an innovative lifelong learning program for professionals in cultural, health, and social work, and fosters inclusive wellbeing for vulnerable groups via arts and cultural engagement. The WITAC case has demonstrated that women create new ecosystems at the intersection of health and culture. These innovative processes require qualities such as courage, risktaking, passion, curiosity, and creativity. Among the conclusions, we argue that women are often involved in vulnerable, sensitive, and innovative activities, such as social, cultural, or healthcare work. In the WITAC project, participants seek inclusive well-being for vulnerable groups via arts and cultural engagement. Women are open, (self)reflexive, goal oriented, take risks, and demonstrate personal involvement. They participate in shaping the future of culture as a ‘care’ profession.

The collaboration of the two sectors is recognized and promoted by leading global and EU-level organisations in the fields of culture and health. The EU Work Plan for Culture 2023–2026 states that “participation in culture and cultural heritage, creativity and the arts have a positive impact on people of all ages and backgrounds; it enhances people’s quality of life and improves the health and overall well-being of individuals and communities.”2 Moreover, the recently published EU report “Culture and Health: Time to Act”, developed by the European Union Open Method of Coordination (OMC) Group for Culture and Health, sets out practical, evidence-based actions to help EU Member States better integrate culture into health and well-being policies.3 The World Health Organisation’s Sector Brief on Arts (2019) stresses that “working together, the two sectors can provide holistic health care that draws on the arts to address complex mental and physical health needs and to support the broader promotion of health by harnessing the social, educational, behavioural and communicative potential of engagement in the arts.”4 [...].

Vyrų balsams įgarsinti pasirinkta fenomenologinė kokybinio tyrimo strategija. Duomenys, gauti naudojant pusiau struktūruoto interviu ir vizualizacijos metodą „Gyvenimo kelionė“, parodė, kad ligos priėmimo procesas susideda iš savęs ir savo ligos pažinimo, santykio su liga formavimo, ligos priėmimo proceso reflektavimo ir specialistų pagalbos. Šis procesas nėra baigtinis.

;Jaskyte, Kristina ;Hunter, AshleyNonprofit Management and Leadership, 2025-11-27, vol. 00, no. 00, p. 1-11This research note draws on theory and empirical research from the broader fields of management, organizational capacity, and innovation to examine how specific organizational capacities relate to innovation in a sample of 107 rural nonprofit organizations in a Southeastern U.S. state. While six capacities – human resources, financial management, revenue development, planning and development, collaboration and stakeholder engagement, and adaptive capacity – were positively correlated with innovation, only adaptive capacity and collaboration/stakeholder engagement emerged as significant predictors in a multivariate model. These findings highlight the potential relevance of internal and external capacities to innovation in resource-constrained rural settings. The note concludes with implications for practice and outlines limitations and directions for future research.
