10th International Health Sciences Conference IHSC : March 5th-6th, 2026 : Abstract book / Edited by Beatrice Ziulyte, Karina Zerr, Gabija Varkuleviciute & Ignas Jusis, 2026-03-05, p. 104-105

Introduction Chronic cough and related upper airway symptoms may persist despite standart management, reflecting underlying laryngeal hypersensitivity or neurogenic dysfunction that lowers the threshold for sensory activation of the cough reflex (1,2). This heightened sensorimotor response- called Irritable Larynx Syndrome (ILS)- arises from a peripheral and central sensitization mechanism, often triggered by inflammation, reflux or irritant exposure and represents a functional rather than structural disorder of the larynx (3,4). Case Presentation A 59-year-old woman presented with hoarseness and phonation-induced coughing fits after a history of unsuccessful treatments, multiple specialist evaluations and ineffective reflux therapy. Examination revealed smooth, reddish laryngeal mucosa with symmetrical vocal fold movement and no structural abnormalities. Patient was diagnosed with dysphonia due to suspected laryngeal hypersesitivity. A superior laryngeal nerve-directed treatment using lidocaine and dexamethasone was initiated, alongside reflux control and speech therapy recommendations. After two injections, symptoms and vocal fold closure improved, confirming a functional rather than structural etiology. Follow-up evaluation were scheduled at three and nine months. Discussion ILS underlies chronic cough and dysphonia, affecting up to 10% of adults with peristent upper airway symptoms (3). Peripheral and central sensitization produce exaggerated laryngeal reflexes to innocuous triggers, often following inflammation or irritation (1,4). Clinically patients present with hoarseness and phonation-triggered cough despite normal laryngoscopy (3). Diagnosis is confirmed by clinical findings after excluding structural, post-viral, reflux and pulmonary causes. Management combines neuromodulators, neural-directed therapy (e.g., SLN/lidocaine-steroid injections), modern reflux control and speech therapy (1,2). Early recognition and trigger avoidance favor symptom reversal and improved prognosis (2,4). Conclusions Dysphonia and chronic cough without structural changes likely reflect laryngeal hypersensitivity (2,4). Improvement after superior laryngeal nerve block confirms the benefit of neural-targeted therapy (1). Early recognition enables effective management and better outcomes (2,3).

Objectives: The aim of this study was to develop the Lithuanian version of the Pediatric Voice Handicap Index (pVHI) and to assess its psychometric characteristics. Methods: The Lithuanian version of the pVHI was prepared according to the standard requirements for the questionnaire translation and adaptation procedure. It also included forward and backward translations of the original questionnaire, as well as a committee review and psychometric testing of the final version of the pVHI. A total of 126 children between 4 and 13 years of age were included in the study. Two study groups were formed: a group consisting of children with dysphonia (n = 70) and a control group of children without any alterations in their voice (n = 56). The questionnaire was always completed by their parents. The results were statistically analysed to assess the reliability, validity, sensitivity and specificity of the Lithuanian version of the pVHI. Results: High internal consistency of the Lithuanian version of the pVHI was determined (Cronbach's α = 0.938). The test-retest analysis also showed a high correlation between the two tests (r = 0.949) for both the dysphonic and control groups. The mean overall pVHI score was significantly higher (P < 0.001) in the dysphonic group than in the control group: 26.16 ± 11.84 and 2.43 ± 2.61, respectively. Similar results were obtained for all domains of pVHI. The most sensitive and specific limiting value for discrimination between the clinical and control groups was the sum of 8 points. Conclusions: The Lithuanian version of the pVHI is a reliable and valid instrument for the evaluation of the Voice Handicap Index in the pediatric population and can be easily applied in daily clinical practice.

Rijimo sutrikimai po ankstyvųjų stadijų gerklų vėžio chirurginio gydymo
[Swallowing disorders in patients surgically treated for early laryngeal cancer]
