Vidutinių vainikinių arterijų stenozių aterosklerozinių plokštelių sandaros, nustatytos kompiuterinės tomografijos metodu, diagnostinė bei prognozinė vertėItem type:Publication, [Diagnostic and Prognostic Value of Atherosclerotic Lesions Causing Intermediate Coronary Artery Stenosis Assessed by Computed Tomography Angiography]doctoral thesis[2026][M001][120]; ; ; ; ; ; ; ;Badarienė, JolitaRadikė, MonikaVainikinių arterijų liga (VAL) išlieka viena svarbiausių sergamumo ir mirtingumo nuo širdies ir kraujagyslių ligų priežasčių.
3 3 Stellate ganglion block as a bridge to definitive ganglionectomy in the management of refractory ventricular tachycardiaItem type:Publication, [Žvaigždinio mazgo blokada kaip tarpinė priemonė prieš galutinę širdies simpatinės nervų sistemos denervaciją gydant refrakterinę ventrikulinę tachikardiją]journal article[2026][S4][M001][8]; ;Ramanauskaitė, Gabrielė ;Klišmontaitė, AugustėSveikatos mokslai = Health sciences in Eastern Europe, 2026-09-17, vol. 36, no. 6, p. 125-132Electrical storm (ES) is a critical cardiac emergency characterized by recurrent, life-threatening ventricular arrhythmias occurring within a short period, most commonly in patients with structural heart disease, particularly those who have an implantable cardioverter-defibrillator (ICD) for secondary prevention, as the device records recurrent ventricular arrhythmias that characterize electrical storm. Despite advances in antiarrhythmic therapy and catheter ablation, some patients remain refractory to conventional management, resulting in significant morbidity and mortality. Recent evidence highlights the potential role of autonomic modulation, particularly stellate ganglion block (SGB), as an adjunctive or rescue therapy in these cases. This article presents the clinical case of a 75-year-old patient with ischemic cardiomyopathy and recurrent episodes of sustained ventricular tachycardia refractory to standard pharmacologic therapy (amiodarone, lidocaine, beta-blockers) and ICD interventions. This case emphasizes the importance of recognizing SGB as a viable, minimally invasive, and potentially life-saving intervention for patients with ES who do not respond to conventional treatment. Furthermore, as its effects are transient, SGB may serve as a diagnostic and bridging step toward definitive sympathetic modulation through surgical cardiac sympathetic denervation in recurrent or refractory cases.
10 Sinkopės rizikos vertinimo skaičiuoklių apžvalga: kriterijų analizė ir klinikinė reikšmėItem type:Publication, [Evaluation of syncope risk assessment scores: analysis of criteria and clinical relevance]journal article[2026][S4][M001][6] ;Simanauskaitė, Fausta ;Abou Amra, Alya ;Miežinytė, AustynaLietuvos bendrosios praktikos gydytojas, 2026-09-16, vol. San Francisco Syncope Rule, SFSR) bei EGSYS balą (angl. Evaluation of Guidelines in Syncope Study score).
Be to, šiame tyrime detaliai įvertintas ir intraoperacinių arterinio kraujo dujų rodiklių poveikis minėtoms komplikacijoms.
74 25 Electrocardiographic and Hemodynamic Changes During Tilt-Table Testing in Patients With SyncopeItem type:Publication, Article[2026][S4][M001][12] ;Stirkaitė, UgnėCureus, 2026-05-04, vol. 18, no. 5, p. 1-12Background Syncope is a common clinical condition with diverse underlying mechanisms, making its evaluation challenging. Tilt-table testing is widely used to assess autonomic and cardiovascular responses; however, the role of electrocardiographic (ECG) parameters in differentiating syncope types remains unclear. Methodology This retrospective study analyzed patients who underwent tilt-table testing for syncope evaluation in 2023-2024. A total of 59 patients were included and divided into three groups: orthostatic hypotension (n = 20), vasovagal syncope (n = 19), and negative test results (n = 20). ECG parameters (P-wave duration, PR interval, QRS duration, QTc interval, and T-wave duration) and hemodynamic variables (systolic and diastolic blood pressure and heart rate) were assessed at five time points. Linear mixed models were used to evaluate the effects of group, time, and their interaction. Results Most ECG parameters showed significant changes over time; however, no consistent group-time interactions were observed, indicating similar temporal patterns across groups. P-wave duration and PR interval differed between groups. In contrast, both systolic and diastolic blood pressure demonstrated significant group-time interactions (p < 0.001), reflecting distinct hemodynamic response patterns between syncope types. Heart rate increased significantly during orthostatic testing but did not differ between groups. Conclusions ECG parameters reflect general autonomic adaptation but lack specificity for differentiating syncope etiology. In contrast, hemodynamic responses, particularly blood pressure changes, provide greater diagnostic value and should be the primary focus during orthostatic testing.
38 29 Arrhythmogenic Mitral Valve Prolapse and Mitral Annular Disjunction: a Literature Review and Case-Based Perspective (Russian Translation)Item type:Publication, 12, no. 2, p. 41-51Mitral valve prolapse (MVP) is among the most frequently diagnosed valvular heart diseases, with the majority of patients being asymptomatic. However, MVP may also present with ventricular arrhythmias (VA) or sudden cardiac death (SCD). The arrhythmic form of MVP and the complicated course of this condition are closely associated with mitral annular disjunction (MAD). MAD alters the anatomical integrity of the mitral annulus and is thought to contribute to leaflet degeneration and myocardial fibrosis, creating a substrate for malignant arrhythmias. The most common complications associated with MVP and MAD include progressive mitral valve regurgitation, left ventricular hypertrophy and an increased risk of SCD. Cardiac magnetic resonance (CMR) imaging is the most sensitive modality for detecting MAD and myocardial fibrosis and is thus essential for early diagnosis and risk stratification. This review aims to enhance recognition of MAD, summarize current diagnostic and risk assessment strategies, and provide clinical context through two cases of patients with arrhythmic MVP and MAD.
14 3 Structural and Echocardiographic Abnormalities in Congenital Long QT Syndrome: A Review of the LiteratureItem type:Publication, review article[2026][S1][M001][16]; ; ; ; Medicina, 2026-04-27, vol. 62, no. 5, p. 1-16Congenital Long QT Syndrome (LQTS) is a hereditary cardiac channelopathy defined by delayed ventricular repolarization and an elevated risk of life-threatening ventricular arrhythmias. Recent echocardiographic studies using speckle-tracking and strain imaging have identified subtle abnormalities in ventricular and atrial mechanics among LQTS patients, including reduced global longitudinal strain, impaired diastolic function, enlarged left atrial volumes and a consistently negative electromechanical window. These findings challenge the traditional concept of LQTS as solely an electrical disease and support evolving evidence of a subclinical cardiomyopathic phenotype. Left atrial remodeling, although less studied, may represent an underrecognized component of LQTS with potential implications for arrhythmia vulnerability and diastolic dysfunction. This review summarizes current evidence on electromechanical and structural cardiac involvement in congenital LQTS, highlights its diagnostic and clinical implications, and outlines future directions for research in this evolving field.
34 52 Wide-Complex Tachycardia as the Initial Presentation of Arrhythmogenic Cardiomyopathy: A Case ReportItem type:Publication, conference output[2026][T1e][M001][2] ;Stirkaitė, Ugnė ;Sėlenytė, Milda10th International Health Sciences Conference IHSC : March 5th-6th, 2026 : Abstract book / Edited by Beatrice Ziulyte, Karina Zerr, Gabija Varkuleviciute & Ignas Jusis, 2026-03-05, p. 530-531Introduction Arrhythmogenic cardiomyopathy (ACM) is an inherited heart muscle disease with an estimated prevalence of 0.078%. [1] It exhibits a distinctive propensity to develop scar-related ventricular arrhythmias, reflecting the principal pathological feature of the disease. [2] Case Presentation A 42-year-old man presented with palpitations and dull chest discomfort. ECG showed widecomplex tachycardia, initially interpreted as supraventricular tachycardia (SVT) with aberrancy due to termination with the Valsalva maneuver. Given recurrent arrhythmias and elevated troponin I level, further evaluation was indicated. Coronary angiography excluded myocardial ischemia. Electrophysiological study ruled out SVT or accessory pathways and confirmed ventricular tachycardia; marked dilatation of the right cardiac chambers was observed, prompting further structural assessment. Transthoracic echocardiography was inconclusive, whereas cardiac magnetic resonance imaging demonstrated a normal-sized right ventricle, subepicardial late gadolinium enhancement in the lateral left ventricular wall and mild systolic dysfunction (LVEF 48%). The findings fulfilled one major and two minor criteria of arrhythmogenic left ventricular cardiomyopathy (ALVC). An implantable cardioverterdefibrillator was placed to mitigate the risk of sudden cardiac death (SCD), and the patient was referred for genetic counselling. Discussion The diagnosis of ACM can be challenging due to phenotypic heterogeneity, including rightdominant, biventricular, and left-dominant forms, that may overlap with other cardiac conditions. [2,3] Patients with ALVC often experience a diagnostic delay of > 2 years and have higher mortality rates. [4] They more frequently experience “hot phases“, characterized by episodes of troponin elevation, chest pain, and ECG abnormalities in the absence of coronary artery disease. [5] Therefore, obtaining a detailed family history of SCD, cardiomyopathy, or myocarditis is essential. [6] Conclusions Ventricular tachycardia with chest discomfort, elevated troponin levels, and no coronary obstruction should prompt evaluation for structural heart disease. Cardiac magnetic resonance, when performed early, supports detection of myocardial involvement and guides SCD prevention.

;Ramanauskaitė, Gabrielė ;Klišmontaitė, Augustė10th International Health Sciences Conference IHSC : March 5th-6th, 2026 : Abstract book / Edited by Beatrice Ziulyte, Karina Zerr, Gabija Varkuleviciute & Ignas Jusis, 2026-03-05, p. 258-259Introduction Electrical storm (ES) is a life-threatening arrhythmic emergency that often determines immediate survival in patients with structural heart disease and carries an in-hospital mortality of 20–40% [1]. Beyond its high lethality, ES is associated with frequent hospitalizations, impaired quality of life, and accelerated progression of heart failure [1,2]. Despite guidelinedirected antiarrhythmic therapy and device optimization, a subset of patients remains refractory to conventional management. Increasing evidence supports the use of stellate ganglion block (SGB) as a targeted intervention to mitigate sympathetic overactivity in these patients. Case Presentation A 75-year-old man with cardiovascular comorbidities was hospitalized multiple times in 2025 due to recurrent ventricular tachycardia (VT). Initial episodes were treated with antiarrhythmic therapy, implantable cardioverter-defibrillator (ICD) interventions. Ongoing arrhythmia recurrence prompted invasive electrophysiological evaluation - three-dimensional electroanatomic mapping. Pace mapping and radiofrequency ablation were performed. Despite optimized pharmacological therapy, device reprogramming, and catheter-based intervention, VT recurred. An autonomic modulation strategy was therefore pursued. An ultrasound-guided left stellate ganglion block (SGB) at the C6 level was performed using local anesthetic, resulting in immediate suppression of VT, confirmed by transient Horner’s syndrome. No further ventricular arrhythmias were observed during hospitalization. Discussion This case illustrates the role of SGB as an adjunctive therapy in patients with recurrent VT. Previous studies have demonstrated that SGB can rapidly suppress VT when conventional pharmacological and electrical therapies fail. The prompt arrhythmia termination observed in this patient is consistent with published experience and supports autonomic modulation as a targeted, mechanism-based intervention. Although SGB does not provide definitive long-term control, it may serve as an effective stabilizing measure. Conclusions Stellate ganglion block can provide effective short-term suppression of electrical storm by reducing sympathetic activity. This case highlights its usefulness as a temporary therapeutic option when conventional treatments fail.

; 10th International Health Sciences Conference IHSC : March 5th-6th, 2026 : Abstract book / Edited by Beatrice Ziulyte, Karina Zerr, Gabija Varkuleviciute & Ignas Jusis, 2026-03-05, p. 566-567Introduction Reflex syncope is transient loss of consciousness triggered by reduced cerebral perfusion, which can result from two components: vasodepressor or cardioinhibitory mechanisms or combination of both [1]. Case Presentation A 62-year-old female patient with a history of arterial hypertension, type 2 diabetes mellitus, spinal canal stenosis and rheumatoid arthritis presented with recurrent dizziness and a prior episode of transient loss of consciousness for a tilt table test (TTT) at our hospital. Previously, the patient underwent additional investigations including echocardiography: fibrodegenerative changes of the aortic valve, left ventricular hypertrophy. Electrocardiogram, Holter monitoring and coronary computed tomography angiography without significant changes. The patient was consulted by an endocrinologist for a potential association with diabetes, however, glycemic control was adequate. Neurological evaluation, including carotid ultrasonography demonstrated no clinically significant stenosis. During TTT, after 10 minutes, passive phase without spontaneous syncope, 400 µg sublingual glyceryl trinitrate was administered. At the 2 minutes of head-up tilt (70°), the patient developed presyncopal symptoms, including weakness, blurred vision and nausea (symptoms coincided with those experienced in real life), accompanied by a significant hypotensive response, with blood pressure decreasing from 146/90 mmHg (67 bpm) at the baseline to 86/60 (100 bpm), leading to discontinuation of the test. Reflex syncope (VASIS III) was confirmed. The patient remained hemodynamically stable after the test. Discussion In cases of reflex syncope with a predominant vasodepressor component, treatment is conservative and pacemaker implantation is not indicated. This case also highlights that although vasovagal syncope is more common in younger patients, it can occur in older individuals as well. Conclusions Diagnosis of reflex syncope remains challenging due to a variety of triggering factors and nonspecific symptoms. Although this condition does not increase mortality, it is associated with higher risk of injury and reduced quality of life [2].
