Arterinės hipertenzijos paplitimo ir mirtingumo nuo galvos smegenų insulto pokyčiai tarp vidutinio amžiaus Kauno gyventojų 1983-2006 m. = : Trends in arterial hypertension and mortality from stroke among middle-aged Kaunas population during 1983 to 2006

Tyrimo tikslas – nustatyti arterinės hipertenzijos paplitimo ir mirtingumo nuo galvos smegenų insulto (GSI) pokyčius tarp 35–64 metų Kauno gyventojų 1983–2006 m. Metodika. 35–64 metų Kauno gyventojų keturios atsitiktinės imtys, kurių sveikatos patikrinimai buvo atlikti 1983–1984, 1986–1987, 1992–1993 ir 2001–2002 metais. Ištirti atitinkamai 2413, 1762, 1231, 1403 asmenys. Tyrimai atlikti vykdant tarptautinę MONICA studiją (Multinational Monitoring of Trends and Determinants in CArdiovascular Disease). Mirtingumo nuo GSI rodikliai apskaičiuoti 100 000 gyventojų per metus ir standartizuoti pagal amžių. Rodiklių pokyčių kryptys buvo analizuojamos logaritminės regresijos metodu, naudojant statistinės analizės „Joinpoint 3.4.2“ regresinę programą. Rezultatai. Per tiriamąjį laikotarpį sistolinio arterinio kraujospūdžio vidurkiai vyrų ir moterų sumažėjo, o diastolinio arterinio kraujospūdžio vidurkis sumažėjo tik moterų. Tačiau arterinės hipertenzijos paplitimas sumažėjo tik tarp moterų – 11,9 proc. (nuo 51,3 iki 39,4 proc., p<0,001), o tarp vyrų nepakito. Kauno m. GSI registro duomenimis, 1986–2006 m. 35–64 metų vyrų Kauno gyventojų vidutiniai mirtingumo nuo GSI rodikliai buvo 99,0/100000 vyrų ir 50,6/100000 moterų ir per analizuojamąjį laikotarpį statistiškai reikšmingai mažėjo: vyrų – 2,8 proc./m., p=0,004, moterų –2,1 proc./m., p=0,02. Išvada. 1983–2002 m. arterinės hipertenzijos paplitimo pokyčiai sumažėjo tik 35–64 metų Kauno moterų, o vyrų nepakito. 1986–2006 m. Kauno 35–64 metų tiek vyrų, tiek moterų mirtingumo pokyčių rodikliai nuo GSI reikšmingai sumažėjo.
Th e aim of study was to evaluate the prevalence of arterial hypertension among Kaunas population aged 35–64 in 1983–2002 and to assess stroke mortality trends during 1986–2006. Methods. Four independent surveys in 1983–1984, 1986–1987, 1992–1993, and 2001–2002 were carried out in random samples of men and women aged 35–64 involving 2413, 1762, 1231 and 1403 persons, respectively. Th e risk factors were defi ned within the framework of the WHO MONICA study (Multinational Monitoring of Trends and Determinants in Cardiovascular Disease). Th e age-standardized rates were calculated by the direct method and using the Segi’s world population as a standard. Trends were analyzed using the method of regression on logarithms of the age-standardized annual rates using the Joinpoint regression program. Results. During the analyzed period the mean values of systolic blood pressure decreased among men. Among women decreased both mean values of systolic and diastolic blood pressure, however the prevalence of hypertension has decreased only among women by 11.9% (from 51.3% to 39.4%, p<0.001) with no signifi cant changes among men. According to the stroke register data, during the study period, the average rate of mortality from stroke was 99.0/100,000 for men and 50.6/100,000 for women. During the study period trend analysis showed a signifi cant decline in mortality rates from stroke among Kaunas population by -2.8%/yr., p=0.004 in men and -2.1%/yr., p=0.02 in women. Conclusions. Th e prevalence of arterial hypertension decreased in Kaunas middle-aged women during 1983–2002. In 1986–2006 mortality rates from stroke among Kaunas men and women aged 35–64 signifi cantly decreased.
