Abstract
Darbo autorius: Inesa Galinaitytė Darbo pavadinimas: Radiologinių ir klinikinių tyrimų sąsajos sergant antinksčių navikais Tyrimo tikslas: Įvertinti pacientų, kuriems nustatyti antinksčių navikai, klinikinius, laboratorinius ir radiologinius tyrimų duomenis. Tyrimo uždaviniai:
- Įvertinti antinksčių navikais sergančių pacientų bendro ištyrimo duomenis (anamnezė, gyvybinių rodiklių).
- Nustatyti dažniausius radiologinius radinius, esant antinksčių navikams ir pakitusioms hormonų koncentracijoms.
- Įvertinti sąsajas tarp radiologinių radinių ir hormonų tyrimų. Tyrimo dalyviai: Pacientai, kuriems 2017-2018 m. buvo diagnozuoti antinksčių navikai LSMUL KK. Tyrimo metodika: Tyrime nagrinėti retrospektyvūs ligos istorijų duomenys. Naudojantis LSMUL KK duomenų baze, buvo atrinkti pacientai, kuriems 2017-2018 m. buvo diagnozuoti antinksčių navikai. Statistinė duomenų analizė buvo atlikta ,,IBM SPSS Statistics 29.0“ kompiuterine programa. Skirtumai tarp grupių laikyti statistiškai reikšmingais, kai p < 0,05. Tyrimo rezultatai: Tyrime dalyvavo 40 vyrų ir 40 moterų. Vyrų amžiaus vidurkis buvo 59,48 ± 12,45. Moterų amžiaus vidurkis buvo 63,83 ± 14,92. Trečdalis pacientų (35 %) turėjo viršsvorį, o nutukimas buvo stebimas dar dažniau, net 41,3 %. 66 (82 %) pacientams arterinis kraujo spaudimas buvo padidėjęs. Vidutinis širdies susitraukimų dažnis tarp moterų buvo 77,48 ± 11,43 k./min, o tarp vyrų – 78,53 ± 13 k./min. Dažniausias nusiskundimas buvo aukštas kraujo spaudimas (63,8 %). Dažniausia gretutinė liga buvo arterinė hipertenzija (42,5 %). Dažniausias antinksčių navikas buvo adenoma (42,5 %), o rečiausias antinksčių navikas buvo aldosteronoma (3,8 %). Dažniausiai pasitaikantis naviko dydis buvo 3 cm (25 %). Nustatyta, kad kontrastą kaupia 37 navikai, likusieji 30 kontrasto nekaupia. Nagrinėjant vienpusių ir dvipusių navikų palyginimą tarp lyčių buvo rasta, kad vyrų (30 %) žymiai didesnė dalis nei moterų (5 %) turėjo dvipusius navikus (p < 0,05). Analizuojant imties pasiskirstymą pagal naviko tipus ir hormonų koncentracijas kraujyje, nustatyta, kad feochromocitomos atveju padidėjimo procentais, kai tiriamas metanefrinas ir noretanefrinas, žymiai didesnis nei kitų navikų atvejais. Išvados: 1. Įvertintus antinksčių navikais sergančių pacientų bendro ištyrimo duomenis, nustatyta, kad KMI, kraujo spaudimas ir ŠSD skirtumai tarp lyčių nebuvo statistiškai reikšmingi. Dažniausias pacientų nusiskundimas buvo aukštas kraujo spaudimas. Analizuojant nusiskundimus, tarp vyrų ir moterų reikšmingų skirtumų nebuvo gauta. 2. Dažniausi antinksčių navikai buvo adenomos, o adenomų hormonų koncentracijos pakitimai nebuvo statistiškai reikšmingi. Adenomos ir feochromocitomos naviko dydis statistiškai reikšmingai skyrėsi, feochromocitomos navikai žymiai didesni. Feochromocitomos statistiškai reikšmingai daugiau kaupia kontrastą nei visi kiti antinksčių navikai. 3. Nustatyta, kad feochromocitomos atveju hormonų, tokių kaip metanefrinas ir normetanefrinas, padidėjimo procentas žymiai didesnis nei esant kito tipo antinksčių navikams.
Author: Inesa Galinaitytė Title: Radiological and Clinical Interfaces of Adrenal Tumors Aim of the study: To evaluate the clinical, laboratory and radiological data of patients diagnosed with adrenal tumors. Objectives of the study:
- To evaluate the physical examination data of patients with adrenal tumors (anamnesis, vital signs).
- To identify the most common radiological findings in adrenal tumors and altered hormone concentrations.
- To evaluate correlations between radiological findings and changes of hormones. Subjects: Patients who were diagnosed with adrenal tumor in 2017-2018 at the Hospital of LUHS KK. Methods: Retrospective data from medical histories were examined. Using the database of the Hospital of LUHS KK, patients who were diagnosed with adrenal tumor in 2017-2018. Statistical analysis was performed using “IBM SPSS Statistics 29.0“ computer program. All differences were considered statistically significant at p < 0,05. Results: This study included 40 men and 40 women. The mean age of the men was 59.48 ± 12.45. The mean age of the women was 63.83 ± 14.92. One third of the patients (35 %) were overweight, and obesity was observed even more often (41,3 %). Arterial blood pressure was elevated in 66 (82 %) patients. It was found that the average heart rate among women was 77.48 ± 11.43 heart rate per minute, and among men it was 78.53 ± 13 heart rate per minute. The most common complaint was high blood pressure (63,8 %). The most common comorbidity was arterial hypertension (42,5 %). The most common adrenal tumor was adenoma (42,5 %) and the least common adrenal tumor was aldosteronoma (3,8 %). The most common tumor size was 3 cm (25 %). Analyzing the contrast enhancement of the investigated tumors, it was found that 37 tumors enhance contrast. When comparing unilateral and bilateral tumors between gender, a significantly higher proportion of males (30 %) than females (5 %) had bilateral tumors. Analyzing the distribution of the adrenal tumor types and blood hormone concentrations, it was found that the percent increase in metanephrine and noretanephrine was significantly higher in pheochromocytoma than in other tumors. Conclusions: 1) After evaluating the data of the physical examination of patients with adrenal tumors, it was found that the differences between gender in BMI, blood pressure and heart rate were not statistically significant. The most common complaint was high blood pressure. No significant differences were found between men and women in the analysis of complaints. 2) The most common adrenal tumors were adenomas, and the changes in the hormone concentration of adenomas were not statistically significant. Adenoma and pheochromocytoma tumor sizes differed statistically significantly, pheochromocytomas being significantly larger. Pheochromocytomas enhance the contrast statistically significantly more contrast than all other adrenal tumors. 3) It has been found that in pheochromocytoma, the percentage of increase in hormones such as metanephrine and normetanephrine is significantly higher than in other tumors.
