SIRT1 geno polimorfizmų ir Ki-67 proliferacijos indekso sąsajos su hipofizės adenoma

Tyrimo tikslas: Įvertinti SIRT1 rs4746720 ir rs3740051 polimorfizmų ir Ki-67 proliferacijos indekso sąsajas su hipofizės adenomos pasireiškimu. Uždaviniai: 1. Nustatyti SIRT1 vieno nukleotido polimorfizmų genotipų dažnius sergantiems hipofizes adenoma ir sveikiems asmenims bei įvertinti jų įtaką hipofizės adenomos pasireiškimui. 2. Nustatyti SIRT1 vieno nukleotido polimorfizmų sąsajas su hipofizės adenomos pasireiškimu, atsižvelgiant į lytį. 3. Nustatyti SIRT1 vieno nukleotido polimorfizmų sąsajas su hipofizės adenomos pasireiškimu, atsižvelgiant į hipofizės adenomos recidyvą, invazyvumą ir hormoninį aktyvumą. 4. Nustatyti SIRT1 polimorfizmų ir Ki-67 proliferacijos indekso (PI) sąsajas. Tyrimo metodai ir dalyviai: Tyrime dalyvavo 142 asmenys, kuriems buvo nustatyta hipofizės adenoma (HA) ir 826 sveiki tiriamieji. Iš tiriamųju buvo paimtas kraujas, iš kurio buvo išskirta DNR, naudojant DNR skyrimo rinkinius. Realaus laiko polimerazės grandinės reakcijos metodu buvo atliktas genotipavimas. Rezultatai apskaičiuoti naudojant ,,IBM SPSS Statistics‘‘ programą. Rezultatai: Atlikus SIRT1 rs4746720 genotipavimą visiems tiriamiesiems buvo nustatytas laukinio tipo genotipas (A/A), todėl toliau į statistinę analizę šis polimorfizmas nebuvo įtrauktas. Išanalizavus SIRT1 rs3740051 polimorfizmo dažnį, sergantiems hipofizės adenoma ir kontrolinėje grupėse, statistiškai reikšmingų skirtumų nustatyta nebuvo. Taip pat, statistiškai reikšmingų rezultatų negavome atlikę analizę pagal tiriamųjų lytį. SIRT1 rs3740051 G/A genotipas, lyginant su G/G genotipu galimybę susirgti neinvazyvia HA didina 2,6 kartus (p=0,003), o lyginant su G/G ir A/A genotipais kartu, šią galimybę didina 2,5 kartus (p=0,004). Taip pat, G/A ir A/A genotipai kartu, lyginant su G/G genotipu, susirgti neinvazyvia HA galimybę didina 2,7 kartus (p=0,001). Išvados: Nustatėme, kad SIRT1 rs3740051 A/A genotipas mažina galimybę susirgti HA su recidyvu, o G/A ir G/G genotipai bei kiekvienas G alelis didina galimybę susirgti neinvazyvia HA. Vertinant SIRT1 rs3740051 polimorfizmo ir Ki-67 proliferacijos indekso sąsajas, statistiškai reikšmingų skirtumų nenustatėme.
The Purpose of the Thesis: To evaluate the influence of SIRT1 rs4746720 and rs3740051 polymorphisms and Ki-67 labelling index manifestations of pituitary adenoma. The Tasks: 1. To determine the frequencies of SIRT1 single nucleotide polymorphisms in patients with pituitary adenoma and for control group, also evaluate their influence on the manifestation of pituitary adenoma. 2. To determine the association between SIRT1 single nucleotide polymorphisms and manifestation of pituitary adenoma, based on gender. 3. To determine the association between SIRT1 single nucleotide polymorphisms and manifestation of pituitary adenoma in recurrence, invasiveness and hormonal activity. 4. To determine the association between SIRT1 single nucleotide polymorphisms and Ki-67 labelling index (LI). Materials and methods: Study group included patients with 142 pituitary adenoma (PA) and 826 control group of healthy people. The DNA was isolated with DNA purification kit from blood. The genotyping was performed using the real-time polymerase chain reaction method (RT-PCR). The results were calculated using the statistical analysis method of “IBM SPSS Statistics“. Results: Genotyping of SIRT1 rs4746720 revealed that only wild-type genotype (A/A) was observed in all study subjects, therefore this polymorphism was not included in the further statistical analysis. Genotyping results of SIRT1 rs3740051 polymorphism did not show statistically significant differences between overall PA group and control had no relation to gender, as well. Further, analysis showed that SIRT1 rs3740051 G/A genotype increases odds of having non-invasive PA by 2.6-folds (p = 0.003), compared with G/G and A/A genotype and by 2.5-folds (p = 0.004), compared with G/G genotype and by 2.7-folds (p = 0.001), compared with the G/G genotype. Conclusions: Study revealed that SIRT1 rs3740051 A/A genotype decreases odds of recurrence PA development. SIRT1 rs3740051 G/A and G/G genotypes and each G allele increase the odds of non-invasive PA development. No statistically significant results were found for SIRT1 rs3740051 polymorphism and PA activity. No statistically significant associations between SIRT1 rs3740051 and Ki-67 were found, as well.
